Job summary
Care Coordinator 26 hours per week - 12-month fixed term contract
We are looking for a friendly, motivated and IT confident Care Coordinator to join our busy GP practice in Honiton. This is a patient facing and coordination role where excellent communication, accuracy and professionalism are essential.
Hours
This role includes some long working days, in line with GP practice opening hours 08:00-18:30. We are recruiting for a parttime role with the following potential working pattern:
Monday 14:00 - 18:00 (4 hours)
Tuesday 8:00 - 6:00 (9 hours)
Wednesday 14:00 - 18:00 (4 hours)
Thursday - 14:00 - 18:00 (4 hours)
Friday - 8:00 - 6:00 (9 hours)
Flexibility for holiday/sickness cover is appreciated.

Main duties of the job
As a Care Coordinator, you will support patients through their care journey, ensuring they receive the right care, at the right time, with the right clinician. You will work closely with the clinical and administrative teams to coordinate care, manage information, and support practice processes.
You will be a key point of contact for patients in person, by phone and online. Helping them understand next steps, navigate services and complete required actions.
Key responsibilities
- Care navigation using practice protocols to direct patients to the right clinician or health care service.
- Coordinating patient care for longterm conditions, followups, recalls and ongoing care plans.
- Accurate coding of information into patient records.
- Processing prescriptions safely and accurately in line with practice protocols.
- Managing patient communication via phone, online requests and facetoface contact.
- Supporting workflow including scanning, coding, tasks and administrative processes
- Liaising with clinical teams to ensure actions are completed.
- Booking appointments and contacting patients where required.
- Maintaining confidentiality and following practice procedures at all times.
- Providing general support to GPs, management team and wider practice team.

Honiton Surgery has a highly committed and friendly team of GP Partners, Salaried GPs, Advanced Nurse Practitioners, Allied Health Professionals, Clinical Pharmacists, a multiskilled Practice Nursing Team and Clinical Research Team. We have a reputation for proving excellent care to our 18,000 patients and are rated Good by the CQC.
We are the lead practice for our PCN, working collaboratively with two other practices, Sid Valley Practice and Coleridge Medical Centre. We have developed a wider range of PCN teams and roles, including, Mental Health and Wellbeing Team, First Contact Physiotherapists, Clinical Pharmacists and Pharmacy Technicians. Through the PCN we are also developing a Practice Nursing Network, led by our Lead PCN Nurse.

Scanning of electronic and paper documents utilising the SystmOne Clinical System and to accurately code to patient records according to Surgery protocols.
To process repeat prescriptions, ensuring safe, accurate and timely supply of prescribed medication to our patients. Undertaking medicines reconciliation in accordance with the practice protocols and to update patients medical records and prescriptions accordingly.
Maintain effective liaison with practice staff and outside agencies concerned with patient care with appropriate regard to confidentiality.
To provide administrative support to Clinicians and colleagues in the workflow team.
Care navigate patients requests using the correct guidelines to ensure all requests are dealt with according to Surgery protocols and in accordance with the patients needs.
Processing electronic requests to facilitate booking appointments and directing patients to the appropriate healthcare professional.
To contribute to initiatives that help the practice to achieve its targets in the Quality Outcomes Framework, PCN, DES and IIF.
Communicate with patients, colleagues and outside agencies using face-to-face, telephone or electronic methods in a professional manner referring to the appropriate GP in accordance with practice protocols.
Respond to patients in a professional, courteous manner.
Contact patients and book appointments if needed using a variety of means on behalf of Honiton Surgery.
Undertake training as directed by Management and attend team meetings as required, keeping up to date with mandatory training, practice policies, emails and surgery intranet.

HEALTH & SAFETY:

- Using personal security systems within the workplace according to Surgery guidelines.
- Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks.
- Making effective use of training to update knowledge and skills.
- Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards.
- Actively reporting of health and safety hazards and infection hazards immediately when recognised.
- Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role.
- Undertaking periodic infection control training, minimum annually.
- Reporting potential risks identified.

Job summary
We have an exciting opportunity for a Band 7 Highly Specialist Dietitian to provide a comprehensive nutrition and dietetics service in the acute hospital setting to a caseload of in-patient paediatrics, associated clinics.
Main duties of the job
The highly Specialist Paediatric Dietitian is responsible for delivering day-to-day care for children, young people, their families and carers living with dietetic needs and will manage a designated clinical caseload of children and young people with dietetic requirements. The post holder will use clinical judgement and autonomous decision-making about the specialist nursing / AHP care and management of patients, undertaking a range of specialist clinical actions across a variety of settings following assessment of patients' needs and preferences.
They will provide expert and specialist paediatric dietetic advice, education, training and support to key partners within both the community and acute settings and work collaboratively and effectively with all other health care professionals, those allied to health and those in the third sector services.
The post holder will work at an enhanced level of practice utilising highly developed post graduate experience, knowledge and skills in dietetic specialty. Could you identify if a detainee is suffering from a mental health condition? If they are uncooperative or lacking diabetes medication? If they are a substance misuser or suffering from a head injury? Our Forensic HCPS are responsible for assessing and addressing the health needs of detained persons, as well as conducting forensic medical examinations gathering evidence for use in the criminal justice system.
Main duties of the job
You will be conducting clinical assessments, identifying, and implementing appropriate interventions and collecting forensic samples. Providing advice and guidance to ensure appropriate care is maintained and keeping detailed and accurate records to ensure the health, safety and welfare of detained persons held in police custody.

Forensic Healthcare Professional Skills and Experience:

As a Healthcare Professional, you will be joining our team of clinicians who provide custody cover 24 hours a day, 365 days a year, to assess and address the health needs of detained people, as well as conduct forensic medical examinations gathering evidence for use in the criminal justice system. The role will involve conducting clinical assessments, medicines management, identifying and implementing appropriate interventions, collecting forensic samples, providing advice and guidance and maintaining detailed and accurate records to ensure the health, safety and welfare of detained people held in police custody.

Key Responsibilities
- Provide a professional, responsive, and compassionate service to detained individuals in custody, supporting Custody Sergeants and Officers in assessing healthcare needs and ensuring an effective care pathway throughout detention.

This service is centralised alongside Ophthalmology inpatient, Maxillofacial Surgery and emergency services at The William Harvey Hospital. All 3 Departments share a subspecialist Head and Neck Ward at William Harvey Hospital.
The ENT team is comprised of 13 Consultants working alongside 6 Specialty Doctors, 5 Specialist Registrar's, 6 teaching grades, 3 Trust Doctors and 4 Nurse Practitioners across the sites. We offer 27 days' annual leave and active support for study, quality improvement and patient safety work as part of our 'We care' improvement journey, and medical education and research.
Main duties of the job
Provide support to GPST2, F1, F2, CT1, CT2 in maintaining A&E Gold Standards and breach rules.
Provide clinical/observational/prescribing support to NPs (nurse practioners) working in adjacent clinics.
Elective work - day to day cover of all patients admitted under supervising Consultant.
Provide clinical support/review of any patient requiring ENT input as an Inpatient on either of the 2 acute sites
Contribute to the overall service delivery and performance of the ENT Department
Provide support and training to junior staff, but have the ability to work and manage patients independently
Confidently consult, diagnose and work up a treatment plan for cancer, adult & paediatric general ENT patients
Co-ordinate, communicate and work as a team with other available Grades within the department.
Ensure that records of patients in your care are accurate, up to date, legible and signed.

Main duties of the job
The post holder will assist in the supply of medicinal items in the Medicines Distribution Unit (MDU) to all wards and departments across Barts Health. The role will also include supporting procurement activities, receipt of stock from suppliers, and maintaining stock control systems including the Pharmacy Robot. In addition, the post holder will supervise the ATOs and rotational staff working in MDU.
Candidates must possess a minimum functional skills qualification in English and maths and a Level 2 in Pharmacy services. Experience of supervising a team and of working in a healthcare environment would be desirable but not essential as training would be provided. We welcome applications for the role of Consultant Gastroenterologist to join our team based at Yeovil District Hospital in Somerset. This is an excellent opportunity for you to bring forward fresh ideas, drive service improvement and introduce new initiatives.
We have a newly opened diagnostic centre featuring state of the art equipment, modern facilities and providing an excellent working environment. We are a JAG accredited service demonstrating a commitment to high standards and quality patient care.
The role attracts a 10 PA per week job plan and includes participation in the GI bleed on-call rota (approximately one weekday every two weeks and one weekend every eight weeks).
Within this job plan you will provide specialist diagnosis and management of gastroenterology, hepatology and general medical patients at YDH. You will maintain responsibility for inpatient and outpatient care, respond to referrals from primary care and other specialties, and offer expert gastroenterology input across the Trust. The role includes caring for acute and elective gastroenterology patients, supporting the development of nurse led endoscopy services, and managing a wide range of conditions in outpatient clinics.
You will ensure clinical practice meets governance standards, engage in annual appraisal and revalidation, maintain effective communication with colleagues and external partners, and support service development with awareness of NHS budgeting considerations.

- Diagnosis and management of patients with Gastroenterological and general medical conditions.
- Continuing responsibility for care of patients in liaison with consultant colleagues.
- Respond to and manage in- and outpatient referrals from primary care and other specialties.
- Professional supervision, management and training of junior medical staff.
- To be aware of the general and local budgeting issues within the NHS.
- To be fully involved in the development of the specialty.
- To ensure clinical practice is monitored and performed in accordance with requirements from clinical governance.

This is a unique opportunity to become one of the first Advanced Practitioners leading the delivery of the national SEND Reforms across the Central East region. Children's Services are looking to recruit a Support Worker to our Team at Roston.
Roston is a small welcoming two bedded home regulated by OFSTED which provides short breaks for children and young people with a learning disability and complex health needs.

Job summary
University Sussex Hospitals (East) are looking for two outstanding fixed term consultants for a period of 12 months to join our team of general anaesthetists, working in a fantastic area of the country. Our anaesthetic department is dynamic and has an excellent reputation for teaching, research and innovation for the benefit of our population. The successful applicants will have the opportunity to hone and expand their clinical skills in addition to developing services within University Hospitals Sussex and our partner organisations.
These posts have arisen due to temporary planned absences of consultant colleagues and offer the successful candidates the opportunity to work at both of our Hospitals: the Royal Sussex County (just a few minutes walk from the beach) and the Princess Royal set in the beautiful Sussex countryside. The department offers a mixture of tertiary general work such as Vascular, Major Trauma, Gynae Oncology and Head and Neck, as well as general anaesthetic work including Obstetrics, Colorectal, Orthopaedics and Urology.
Visiting our department prior to interview is encouraged, and if unfamiliar with the hospital, contact with the clinical lead or clinical director prior to application would also be welcomed.
Main duties of the job
Due to the nature of the work undertaken, considerable experience in the delivery of anaesthesia for a wide range of procedures is expected. The posts are flexible in nature, but offer the opportunity to expand and hone practice in a variety of subspeciality areas. The weekly list allocations are consultant led and can hence be tailored to the interests and skills of individuals.
As an expanding service, there are plenty of opportunities for leading service development and quality improvement projects and experience/enthusiasm for these areas would be welcomed.
We are large teaching department with an associated medical school so we expect all consultants to participate in the education and training of medical and non-medical colleagues. Those with a proven track record of delivering education to these groups will find plenty to occupy themselves.

Main duties of the job
The post holder leads the design, configuration, build and testing of their application module, ensuring delivery against governance, quality standards and project plans. Working with clinicians, operational managers and subject matter experts, they analyse service needs, define best practice workflows and support system optimisation aligned with Trust strategic objectives.
They develop and support analysts, collaborate across integrated EHR teams, and contribute to upgrades, testing and safe deployment of new functionality. The role includes resolving incidents, providing specialist end user support, and maintaining strong relationships with departmental leads to ensure effective, clinically aligned system use.
It is a requirement that candidates hold Epic certification or accreditation, ideally but not necessarily within the applications we are recruiting into. If you do not have certification/ accreditation within the relevant application, you will be required to undertake a period of training to obtain this. We are looking for an enthusiastic, forward thinking, compassionate GP to join our practice.6-8 session position (to include Monday, Tuesday and Friday)
Main duties of the job
The post holder will provide high-quality, patient-centred general medical services, including assessment, diagnosis, treatment and management of a wide range of conditions. You will work as part of a multidisciplinary team, contribute to clinical decision-making, and support continuity of care for patients. The role includes participation in practice meetings, service development, and quality improvement, as well as maintaining accurate clinical records and adhering to professional and regulatory standards.

We are seeking a highly motivated & experienced health or social care professional with adult secure/specialised women's pathway experience to join the Quality and Commissioning Team, at the GM Provider Collaborative (PC). The Team holds commissioning & quality responsibility on behalf of the GM ICB/NHS England for adult secure & specialised mental health services across GM. This is an exciting opportunity to work within a dynamic commissioning environment, providing clinically informed support in line with the PC's governance structure, to a complex, specialised programme. The successful candidate will ensure oversight for women's secure and specialised pathways. As part of the role, the post holder will drive forwards the Women's Secure Transformation Programme in line with local and national objectives. The post holder will act as a key contributor to the PC's quality assurance arrangements, working closely with the Senior Case Manager and Head of Quality. The role also provides a link to a range of stakeholders across NHS organisations, independent sector providers and system partners. The successful candidate will require the ability to demonstrate effective communication skills, responsiveness, resilience, confidentiality & integrity. Based at Prestwich Hospital, offering a combination of office & remote working across the footprint & wider as required. Working hours are 9am-5pm Monday to Friday, though some flexibility & out of hours visits may be needed..
Main duties of the job
- Aligned to the national Case Management function, provides expert women's secure and specialised pathway oversight and quality assurance. - Build a combination of clinical expertise and technical skills to develop strong service delivery, pathway advice and service improvements for secure/specialised services. - Specialised informed experience to ensure effective pathway oversight - Ensure patients receiving a high-quality service, free from stigma, discrimination and harm, escalating quality concerns and supporting remedial actions - Effectively fulfil 6-8 weekly host and placing commissioner oversight responsibilities - Robust oversight of patient flow - Supporting strategic leads in capacity and demand planning - Support mandated mental health targets including national LD/A programme - Deliver provision of competent, confident and responsive Case Management function against the national Standard Operating Procedure.
