Main duties of the job
Essential requirements of a Support Worker
- Flexibility - Your role will involve working shifts including weekends and Bank Holiday

At Care in Mind our homes are always a hub of activity and energy where the young people can feel safe, and our staff enjoy coming to work. Our young people can still be quite active at night so you will have lots of interaction and will be required to support them their wellbeing, education, household tasks and general life skills. There will also be more challenging aspects to the role such as incident management and de-escalation of challenging behaviours. You will be required to update online care records, complete medication administration and provide effective handovers to the day staff. In addition, you will be responsible for the general upkeep and tidiness of the home ensuring the young people live in homely environment. Even though you are working nights, you will have the support of another night worker or a sleep in and have access to an on-call manager for additional support.
At our home in Stockport, you will be working with young people who have a variety of mental health diagnosis including personality disorder, those who have difficult relationships with food and those who use self-harm as a strategy to cope with their emotions. As such, you will be required to be patient and resilient as the support you will be providing to our young people will include supporting them to improve their relationship with food and managing incidents of self-harm. A number of our young people are preparing for moving on positively from the home into the community, therefore you will be supporting this transition through things such as: supporting them to self-administer their own medication, getting back into education, accessing public transport and appointments independently. Our young people are very adventurous and engage in a variety of activities including festivals, walking in the Peak District, visiting places like the cinema, Trafford Centre, indoor golf as well as having a stroll on a local park. There will never be a dull moment when you are on shift.
The home is a 4-bed service which is located between Stockport town centre and Marple giving the house the best of both worlds in the sense of being close to all amenities being near a town centre but also the opportunity to explore the countryside. The house is a large 4-bedroom detached property which is well maintained and homely, making it a lovely environment to work in. The garden is another area of activity and includes a trampoline, football nets, dining area as well as flower beds and vegetable patch.
Previous care experience is not essentialand full training will be provided. You will receive ongoing support and mentoring from our friendly residential and clinical team which includes Reflective Practice, 1:1 supervisions and Group Staff support.
The job is highly rewarding and a great start to developing your career working with young people or enhancing your previous experience of mental health with the opportunity for excellent career development.

Main duties of the job

Main duties of the job
As a Intergrated care nurse your responsibilities will vary. You will
- Work within a multi-disciplinary team primarily on our inpatient that cares for both mental health and physical health.
- Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening

Job summary
To support the delivery of high-quality NHS Continuing Healthcare (CHC) and Funded Nursing Care (FNC) by undertaking and contributing to all review of care packages for individuals with complex health needs under the National Framework for NHS Continuing Healthcare.

We are a 19 bedded ward comprising of 6 en-suite cubicles, 3 four bedded bays and one HDU room
We are a general paediatric ward receiving children from 0 - 18 years dependent on condition. We have a busy day surgery area within the ward and take up to 6 surgical patients per day. Our Paediatric Assessment Unit takes up to 9 patients and is where GP's and other health professionals can refer into for admissions and / or assessment, as well as taking patients on their journey from the Emergency Department.
Our nursing associates predominately work within our main ward and day surgery areas.

Main duties of the job
The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team in general practice. An exciting opportunity has arisen for a skilled, enthusiastic and motivated Assistant Practitioner to join the Integrated Reablement Team, incorporating the Urgent Community Response (UCR) Service, Pathway 1 Discharge to Assess (D2A) and Reablement Services across Nottingham City.
Working within a dynamic multidisciplinary team of Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Pharmacy Technicians, Assistant Practitioners, Rehabilitation Support Workers and Administrative Staff, you will support patients to maximise independence through rehabilitation and crisis intervention within their own homes. The service aims to prevent unnecessary hospital admissions and facilitate timely, safe hospital discharges.
As part of the UCR service, you will support clinicians in responding to urgent referrals from emergency and out-of-hours services, providing a rapid two-hour response to crises such as falls, functional decline and acute illness. Comprehensive in-house training will be provided, including falls recovery and safe assistance from the floor.
The successful candidate will plan, deliver and evaluate person-centred, goal-focused rehabilitation programmes to adults with physical and mental health needs.
Main duties of the job
The successful candidates will be expected to work holistically alongside the multidisciplinary team. Our team provide comprehensive assessments to patients who are at risk of hospital admission, along with facilitating discharges from the Acute hospitals.
You will need to be able to use your own initiative within your own limitations to support the clinicians to provide gold standard quality care to our patients within the local community.
The ideal candidate would be someone with experience of working within the health and social care sector, who can demonstrate they have the skills and qualities to work holistically in a fast paced environment. You will be expected to complete holistic assessments, order and assess equipment and minor adaptations within your scope of practice, assess, review and set goals for patients in their own homes.
In return you will be offered a comprehensive induction, access to internal and external training, exposure to a range of clinical expertise within our team, along with regular supervision. If you do not hold the Foundation degree you will be supported to enrol on the Assistant Practitioner foundation degree to aid your future development.

Job summary
The post will incorporate the Clinical Nurse Specialist (CNS) role dimensions of expert clinical practice, education, leadership, management and research. The haematology Oncology service provides expert advice and management for patients who have been diagnosed with a haematological cancer and are having treatment for their disease.
Main duties of the job
The Clinical Nurse Specialist will be required to:
Support the Macmillan Lead Acute Oncology Nurse to deliver and evaluate the service delivery model
Support team working and collaboration within the haemato Oncology team and the wider team across the Royal Free Group.
Provide visibility as haem Onc CNS support to Multidisciplinary teams across the North Middlesex Hospital.
Deliver an CNS service across the North Middlesex Hospital.
The CNS will have a key role within the MDT acting as a liaison between all members of the multi disciplinary team,
Liaise with all key members of the multidisciplinary team to advise and support colleagues to allow for the delivery of seamless and efficient care and maintain patient flow.
Work autonomously to deliver specialist clinical care appropriate to the needs of the patient group and based on principles that are research based and which enhance the quality of patient care.
Work within sphere of confidence, and seek support and education to extend the band 7 CNS role with additional skills in Advanced Clinical Assessment
