We are currently recruiting for two Practice Nurse opportunities within established NHS GP surgeries in West London.
Twickenham Park Medical Practiceis seeking a Practice Nurse on a part-time basis (2 days per week), with flexibility for the role to be offered as permanent, locum, or temporary.
Grove Village Medical Centre is seeking a Practice Nurse on a part-time temporary basis (3 days per week).
Both roles involve delivering high-quality nursing care within a GP surgery setting, including chronic disease management, cervical screening, immunisations and vaccinations, wound care, NHS Health Checks, and health promotion activities.
These are excellent opportunities to join supportive clinical teams committed to delivering outstanding patient care.
Essential Requirement: Applicants must have recent Practice Nurse experience within a UK GP surgery. Candidates without primary care/general practice experience will not be considered.
Main duties of the job
The Practice Nurse will be responsible for delivering high-quality nursing care within a GP surgery setting. Duties will include chronic disease management clinics, cervical cytology, immunisations and vaccinations, wound care, health promotion, NHS health checks, and supporting patients with long-term conditions. The successful candidate will work closely with GPs and the wider multidisciplinary team to provide safe, effective, and patient-centred care while maintaining accurate clinical records and adhering to practice protocols and NHS guidelines.

Job responsibilities
The following are the core responsibilities of the practice nurse. There may be on occasion a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels:
a. Develop, implement and embed health promotion and well-being programmes

q. Prioritise health issues and intervene appropriately

f.Continually review clinical practices, responding to national policies and initiatives where appropriate

Job summary
The Health Records Clerk will be responsible for providing an essential support service in the continuing care of patients, ensuring that the casenotes are at the right place at the right time.
To actively promote the vision for the Health Records service working towards a Digital Records future and support the delivery of modernisation plans for Health Records. This will require the post holder to support with the delivery of the Health Records Operational Plan by supporting the Health Records Co-ordinator/ Assistant Site Manager.
Main duties of the job

If so then we would like to hear from you in connection to our Health Records vacancy. The Records department provides an essential support service. This is not your typical administrative post. The duties are predominantly centred around the physical retrieval, filing and preparation of case notes in conjunction with an efficient and professional outpatient reception service.The successful candidate will assist in the provision of case notes for patient care both Outpatient and Acute ensuring that all case notes are delivered and prepared to Service Level agreements.The post holder will be expected to undertake weeding of existing case notes in line with the organisations Retention and Destruction Policy, ensuring that the Patient tracking system is accurate and up to date.This is a very busy department and we work to tight deadlines in completion of our work therefore it is essential that your application demonstrates good communication, ability to work to tight deadlines, ability to work under your own initiative and ability to prioritise your workload with a high level of accuracy (please see job description and person specification for full details).

Birchington Medical Centre is a small- medium sized practice of just under 9000 patients, many of whom have complex medical conditions and social needs. Main duties of the job
We are looking for an enthusiastic and compassionate GP who enjoys a team focus in problem solving and also enjoys sharing knowledge. You will have excellent verbal and written communication skills and you will be an excellent listener. You will be on the General Medical Council's GP register, having passed the MRCGP (or equivalent) and gained a certificate of Completion of Training (CCT). An interest in teaching would be ideal, and the development of areas of special interest would be welcomed and nurtured.
As a Salaried GP, you would provide a wide range of primary care based general medical services to our patients in a non-judgemental, respectful manner, using a patient centred and fully team-based approach. Good communication between colleagues is essential to the role, as we routinely share our experience and knowledge within the practice in order to optimise patient care.
Home visits by the GPs are rare.

A rare and exciting opportunity has arisen for a motivated and dedicated Specialist Screening Practitioner (SSP) to join the Shropshire Bowel Cancer Screening Centre.
The successful candidate will be primarily based at William Farr House but will be required to work regularly across the service's additional screening sites at The Royal Shrewsbury Hospital and The Princess Royal Hospital in Telford. The service currently operates six days a week, Monday to Saturday, with the potential to expand to a seven-day service as the programme develops.

The post holder will work collaboratively with colleagues to ensure the effective delivery of the NHS Bowel Cancer Screening Programme, maintaining high clinical standards and contributing to continuous service improvement.

Main duties of the job
- Work effectively as part of a multidisciplinary team and as an autonomous practitioner, managing caseload of patients within the Bowel Cancer Screening Programme.
- Undertake nurse-led clinics, including patient assessment, provision of information, and supporting individuals preparing for endoscopic and radiological investigations.
- Provide high-quality, patient-centred care.
- Communicate effectively with patients, relatives, and colleagues, including the delivery of sensitive and complex information such as breaking significant news.
- Collaborate closely with the wider multidisciplinary team to ensure coordinated and seamless care across all stages of the screening process.
- Contribute to achieving Trust objectives, as well as local and national targets set by the NHS Bowel Cancer Screening Programme.
- Accurately record and maintain patient data using the national Bowel Cancer Screening IT system.
- Undertake and successfully complete all required trainings
- Participate in ongoing professional development, supported by in-house training, supervision, and mentorship to meet the requirements of the role.

Job summary
The AAA Screening Programme for Shropshire, Telford & Wrekin has been established since 2012 and continues to offer an excellent service to the residents of Shropshire since its conception.
The primary purpose of this role is to undertake abdominal ultrasound scanning as part of the community AAA screening programme. The post holder will ensure abdominal ultrasound scanning is undertaken to a high standard and in accordance with national standard operating procedures.
The post holder will reassure men attending the community AAA screening and surveillance clinics and explain the screening test to them. The post holder will ensure that the men attending clinics are informed and aware of the implications of AAA screening.
Main duties of the job
The post holder will work as a screening technician, carrying out abdominal ultrasound scans within standard operating procedures. Following successful completion of the National AAA Screening Programme (NAAASP) course, the post holder will work independently and make judgements according to experience and training. Advice is available when needed for more complex situations but the post holder generally works on his/her own initiative without reference to the Line Manager.
The holder must maintain accreditation with NAAASP. Within the Shropshire, Telford and Wrekin Programme, Screening Technicians do not work on their own in clinics.

An exciting opportunity has arisen for an Assistant Practitioner with our Early Supported Discharge (ESD) Stroke team. About us
Wetmore Road Surgery is a long established town practice in the Staffordshire Brewery town of Burton on Trent. We have a list size of 12,500, operating from a single site, in a purpose built building. We were rated as Good with CQC in 2018 and consistently achieve high QOF points. We have weekly CPD meetings, monthly half day PLTs, which alternate between in-house and locality based. An on-going approach has enabled us to tailor our processes in various aspects of the everyday running of the practice with the best interests of both the patients and clinicians. Our experienced reception and admin teams work exceptionally hard directing patients to the most appropriate care ensuring they are seen by the most suitable health care professional first time.

Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. This might include risks that are patient related, medicine related, or both.
Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups.
Update and implement a practice repeat prescribing policy. Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review to the GP
Undertake Clinical Medication Reviews and Structured Medication Reviews with patients, implement required actions or produce recommendations for the GP on prescribing and monitoring as appropriate.
Work with care home staff to improve safety of medicines ordering and administration.
Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advise on treatment pathways)

About the role
You will provide high quality clinical care to our clients across a range of BPAS reproductive healthcare services including termination of pregnancy, STI testing & treatment, contraception, and vasectomy. You will be involved in all stages of the client pathway from consultation through to treatment and will work in all areas of the unit including theatre and recovery. This role will involve providing treatment and clinical information to clients both in person & over the phone.
About You
You will be a Registered Adult Nurse Level 1 or Midwife (practising) with active NMC registration and PIN. You will have clinical practice experience of at least 12 months & have a keen interest in sexual health & reproductive healthcare. Clinical knowledge & experience of safeguarding & Fraser/Gillick competencies are essential for this role.
With excellent interpersonal & communication skills you will be adept at caring for clients in a compassionate & empathetic way. You will also be comfortable using IT & MS Packages including Outlook, Word & MS Teams.
Main duties of the job
- You will work in accordance with NMC guidelines for Professional Practice & the wider regulatory frameworks of the Abortion Act 1967 & Health & Social Care Act as well as BPAS clinical policies and procedures.
- Ensure that the client understands her options in relation to the pregnancy &, if choosing termination, meets the ground of the Abortion Act.
- Provide information on STI Testing, undertake tests and provide results and also discuss, inform and confirm the clients future choice of contraception.
- Carefully check for and implement any procedure indicated on client case notes, including administering medication for Medical Abortion procedures or cervical preparation.
- Carry out post treatment check-ups by assessing the physical condition of the client and discussing any concerns
- Complete relevant paperwork and in the event of post treatment problems, make arrangements with the clinic or refer to an NHS agency if necessary.
- To care for and support clients undergoing surgical treatments. This could include general anaesthetic, conscious sedation or local anaesthetic.
We are also looking for:
- Sound & accurate administrative competence with attention to detail
- Gynecological nursing experience or relevant specialty i.e., sexual health, contraception

Job summary
Are you early on in your P&O career, working in orthotics but still wanting to practice in prosthetics? Are you keen to work and develop in the NHS with all the benefits that brings? If so, a role in our integrated in-house P&O service on the beautiful Isle of Wight may be for you.
An exciting opportunity has arisen for the right individual to join our dedicated team of clinicians, technicians and support staff working across orthotics and prosthetics. You will initially be focusing on providing care to patients in orthotics, working alongside our experienced orthotists, assistant practitioner, technicians and apprentices but you will also join our Prosthetic Rehabilitation Team and Prosthetic Team Lead in the care of new amputees.
Our service has full manufacturing capabilities, a full MDT in Prosthetics and close working relationships with services in both the community and the acute hospital.
Our aim is to provide excellent patient care according to clinical need and you can expect to have a high level of prescription freedom, a wide range of orthoses available to provide on the day and support from our experienced technical team.
We are a small specialised multidisciplinary team with plenty of opportunity to develop your skills and knowledge into more senior roles.
Main duties of the job
In this role you will provide orthotic and prosthetic care for patients of all ages with a wide variety of presentations where provision of an orthosis or prosthesis can improve function and mobility, reduce pain and enhance quality of life.
*To provide routine orthotic care to patients with long term conditions or short-term incapacity, prescribing, fitting and supplying the most appropriate orthosis to meet patients' needs.
*To provide routine prosthetic care to patients working closely with the Prosthetic Rehabilitation Team.
*To provide information and training to patients, carers and healthcare professionals involved in the care of service users.
*To provide clinical supervision and support to extended role technicians, assistant practitioners and students, working closely with senior clinicians.
*To deliver care in a variety of settings including outpatient clinics, patients' own homes and in-reaching into St. Mary's Hospital in-patient wards.

- Live register or equivalent professional registration relevant to the post
- Postgraduate degree or equivalent or be able to demonstrate equivalent knowledge or experiential learning/experience in service development, managing change, leading and managing staff, plus specialist training and short courses to Diploma level relevant to area of practice:
- Good IT skills e.g. knowledge of word and excel
- Evidence of project work that has had a positive impact on service delivery
Desirable
- Accredited leadership/management qualification or ability to demonstrate these at an advanced level
- Knowledge of managing delegated budgets
Knowledge & Experience

- Evidence of continuing professional development (CPD) relevant to the service area
- Proven leadership/management experience, able to demonstrate examples of positive impact/change within service delivery/practice at an advanced level
