Job summary
As a Health Connector, you'll play a key role in the PCNs social prescribing service, supporting people across West Mendip to improve their health, wellbeing and independence through non-clinical, community based support.
Minimum of 22.5 hours per week, with potential of up to 37.5 hours per week in line with service requirements.Please get in touch with the individual listed in the contact section if you would like to discuss this further.
This role is included within the extended hours provision and may involve working an evening and/or Saturday on a rota basis.

Main duties of the job
You'll work directly with individuals and groups, offering time, space and practical support to help people identify what matters to them and access services, activities and resources that can make a positive difference. This includes one-to-one support, facilitating group based wellbeing activity, and helping people take small, achievable steps towards improving their physical, emotional and social health.
The role involves managing a varied caseload, working closely with GP practices and the wider PCN team, and contributing to group sessions that focus on wellbeing, building healthier habits and supporting people with ongoing or pain related conditions. You'll also help strengthen local connections by promoting community resources, contributing to the development of new opportunities, and supporting a prevention focused, neighbourhood approach.
This role forms part of the PCNs social prescribing service, supported by the locally commissioned Health Connections model.
About us
Primary Care Networks (PCNs) were established in July 2019 as part of the NHS Long Term Plan to improve the quality and accessibility of care by encouraging collaboration among various healthcare services and community organisations.
One of 13 PCNs in Somerset, we are a collaboration of five GP practices in West Mendip: Glastonbury Health Centre, Glastonbury Surgery, Wells City Practice, Wells Health Centre and Vine Surgery Partnership (Street).

- Help people to improve their health, wellbeing and social welfare by connecting them to a wide range of local community services, groups, projects and resources
- Encourage people to increase their active involvement in their local community, helping to strengthen personal and community resilience and reduce health inequalities
Health and Wellbeing Coaching
- Provide one-to-one and group health and wellbeing coaching for people who would benefit from support to improve their health and wellbeing, based on what matters to them
- Support people to build confidence, motivation and skills to better manage their own health and wellbeing and improve their health outcomes
- Use behaviour change approaches, including goal-setting and motivational techniques, to support people to make sustainable changes
- Manage and prioritise a caseload of people, recognising when a person's needs are beyond the scope of the role and referring back to, or on to, appropriate health professionals or services as required
Community Outreach and Service Development
- Deliver PCN projects, programmes and activities within the community, including group-based support, outreach, and engagement initiatives, in line with service priorities - this includes existing or newly developed projects and services in response to local need, and working in partnership with local organisations/services and communities where appropriate
- Represent the PCN at community events, outreach activities and informal settings (for example coffee mornings) to engage people and promote PCN support and projects
- Work collaboratively with colleagues across the PCN and wider system to support joined-up, person-centred care
- Attend and contribute to team, GP practice and PCN meetings and events as required
- Make use of regular supervision, reflective practice and continual learning to support safe, effective and person-centred practice.
- Identify gaps in local service provision through conversations with people and communities, working with your line manager, colleagues and community stakeholders to explore and develop potential responses - this may include supporting the development of new community groups or partnership projects
Safeguarding, Governance and Risk
- Recognise potential safeguarding concerns and follow PCN procedures, escalating to Senior SPLW or the appropriate clinician or safeguarding lead as required
- Ensure accurate, timely and confidential record-keeping in line with information governance requirements
- Work within professional boundaries and ethical practice, in line with the duty of candour and PCN policies
- Identify and raise concerns or risks encountered in day-to-day practice, escalating appropriately in line with PCN procedures
ALL POST HOLDERS ARE REQUIRED TO
- Adhere to West Mendip PCN policies and procedures, e.g. Health and Safety at Work, Equal Opportunities etc.
- Maintain personal and professional development to meet the changing demands of the job, participate in appropriate training activities and encourage and support staff development and training
- Attend statutory, essential and mandatory training
- Take part in an annual appraisal
- Respect the confidentiality of all matters relating to their employment and other members of staff. All members of staff are required to comply with the requirements of the Data Protection Act 2018
- Comply with the codes of professional conduct set out by the professional body should registration be required for the post
- Maintain the prevention and control of infection and fully comply with all current Infection Control policies and procedures
- Take responsibility for any records that they create or use in the course of their duties, in line with the Public Records Act and be aware that any records created by an employee of the NHS are public records and may be subject to both legal and professional obligations
- Perform general administrative duties such as locating of clinical files and working with the administrative team as required
- Document in patient notes in the agreed PCN format
- Report untoward incidents such as complaints, clinical emergencies or injury to senior staff

Job summary
This is an exciting opportunity to take on a part time role to improve access to heart failure services for Epsom and St Helier. We ideally would like somebody with heart failure experience that is keen to improve the access to specialised care in hospital and assisting in the transition to the community.
Main duties of the job
We are an expanding cardiology team and we are looking for an experienced heart failure specialist or cardiology nurse who wants to help shape the care we deliver.
Working closely with the lead Heart Failure specialist nurse, the Cardiology Matron and the lead Heart Failure Consultant, you will run and develop thediagnostic and treatment pathways for heart failure patients promoting high quality care across the care continuum from tertiary to primarycare, engaging with healthcare professionals across the network.
The post holder will also be required to support clinical staff in heart failure management plans; make appropriate onward referrals for follow up care; be involved in optimisation of medical therapy; provide education and support to patients, relatives and carers in both inpatient and outpatient settings with the aim being to reduce symptoms, prevent unnecessary admissions/readmission and maximise patients' quality of life. The role involves working closely with the community heart failure teams and the collection of data for local and national audits. You will have a keen interest in education, research, training and development.

We are looking for an enthusiastic and forward-thinking GP to join our friendly and supportive team at Haslemere Health Centre. Main duties of the job
This is a fixed-term contract for maternity cover. The duration is expected to be approximately 12 months but may vary depending on the return date of the substantive postholder.
The post-holder will undertake a full range of GP duties in line with the practice timetable, including:

PASS consists of a diagnostic service for Adults (PASSAT) and a post diagnostic advice and signposting service (PASS). This post sits predominantly within the post diagnostic branch. However, we work together closely, sharing opportunities and skills to learn from each other and best meet the needs of the Autistic community.
This post will provide experience of the delivery of a social model of Psychology within a multi-disciplinary team. You will have the opportunity to provide support within clinics and community settings, such as wellbeing hubs, developing relationships within the voluntary and community sector.

Main duties of the job
You will be working directly with autistic people; supporting people to find resolutions to the issues they face.
The post holder will apply psychological theory to underpin the delivery of a social model of support, working collaboratively with other agencies, VCSO's and teams within Livewell Southwest.
You will undertake needs based assessments that will support the development of formulation and contribute to broader clinical planning.
You provide 1:1 and group support within clinics, community settings and wellbeing hubs.
There may be the opportunity to co-facilitate psycho-educational workshops, with your colleagues or participation partners.
You may undertake audit and evaluation, providing data that helps the team identifying key areas for service improvement.
You will work closely with the teams Senior Autism Practitioner who has a social work background.

With the with pre-diagnostic and diagnostic services, the Post Diagnostic Autism service will be part of an integrated pathway that provides support to understand the diagnosis and steers people with a diagnosis of Autism to services and support that will help them navigate their way through life, providing coping strategies and connecting people with enduring support networks that will prevent acute episodes of poor mental health and reduce unhelpful and unnecessary hospital admissions. Further, the role will encompass a training element, providing support to other areas of service delivery within and outside of LSW in order that mainstream services are better placed to meet the needs of people with ASC
Context:
The service sits within a developing clinical pathway and part of a hub and spoke design that supports people who potentially have a diagnosis of ASC (Pre-diagnostic service) through a process of diagnosis, post diagnosis support to specialist MDT support for those with particularly complex needs.
5. MAIN DUTIES/RESPONSIBILITIES
Key Tasks and Responsibilities of the Post
- To overcome a variety of barriers to communication when working with clients who are fearful of others, unable to articulate their problems, or who think within a very different frame of reference, requiring imaginative and sensitive approaches to engagement.
- To carry out a range clinical assessments with adults with Autistic Spectrum Condition using interview skills, standardised tests and informant based measures, including screening assessments, cognitive assessments, and assessments of various emotional and/or behavioural issues. The post holder will work under supervision and undertake selected 1:1 sessions i.e. all work will be supervised but they must be able to undertake assessment and intervention work independently where needed) and group sessions.
- To work within departmental guidelines to provide input into assessments of complex psychological problems including: risk of offending; capacity; the long-term effects of childhood trauma; other complex biopsychosocial presentations.
- Be able to support the delivery of psychological interventions in 1:1 and group settings, and with families and professional carers. The caseload may include clients who have forensic backgrounds, additional physical health needs, underlying physical disability or health needs, behaviours that challenge, mental health needs and associated underlying trauma.
- To undertake joint work with other members of the multi-disciplinary team as appropriate.
- To utilise psychological theory and apply it to clinical situations in order to integrate assessment data and begin to develop psychological formulations
- To be able to communicate, by talking and writing, psychological formulations and intervention plans to the client, family members, carers and professional colleagues as appropriate.

8.4 Safeguarding Children and Adults

We are seeking an enthusiastic and experienced Consultant Neurologist to join the Headache Service at the National Hospital for Neurology and Neurosurgery (NHNN), Queen Square, on a 12-month locum basis.This 5 PA post offers an opportunity to work within a nationally and internationally recognised specialist service for complex headache and facial pain. You will help maintain continuity of care for existing patients while contributing to the development of specialist services, particularly CGRP monoclonal antibody and Gepant therapies and the urgent assessment and treatment of cluster headache and other trigeminal autonomic cephalalgias.You will work alongside experienced neurologists, specialist nurses, psychologists, neurosurgeons and neuroradiologists in a highly collaborative environment with strong links to research, education and innovation.
Main duties of the job
You will deliver three dedicated headache and facial pain outpatient clinics each week, provide care for inpatients under your responsibility and undertake the associated clinical administration and correspondence.The role has a particular focus on delivering 18-week Referral to Treatment pathways, the CGRP monoclonal antibody and Gepant therapy service, and timely specialist assessment and treatment for patients with cluster headache and other trigeminal autonomic cephalalgias.You will contribute to multidisciplinary working, including collaboration with neurosurgery and neuroradiology where required, and participate in the wider clinical and professional activities of the service. We are excited to announce that we have a vacancy for an HCPC registered specialist band 6 dietitian to join our learning disabilities and mental health team.
You will join a small, close team of dietitians providing specialist services for community learning disabilities and mental health in-patients within Bradford District Care NHS Foundation Trust (BDCFT). Experience of working within learning disabilities and/or mental health, whilst beneficial, is not an essential requirement as you will be part of a large dietetic team covering multiple specialities and your learning and development needs will be supported.
Main duties of the job
You will work across both specialities, managing an inpatient and outpatient caseload with a wide range of clinical areas including nutrition support, home enteral feeding, weight management and gastrointestinal problems. Our learning disabilities work is delivered as part of a specialist multidisciplinary team of nurses, speech and language therapists, occupational therapists, physiotherapists and psychologists.
The role involves developing and delivering training and education for staff and service users, collaborative working, project and audit work, policy development, coordinating care across services, supporting our catering team and leading on nutrition for the service.
The post holder will be expected to participate fully in the training of 'A', 'B' and 'C' Placement student dietitians.
The team is fully integrated within the BDCFT multi-disciplinary team as well as Bradford Teaching Hospitals NHS Foundation Trust's (BTHFT) Specialist Outreach dietetic team. Main duties of the job
- To carry out physiotherapy assessments and treatments; developing individualised therapeutic treatment programmes for a specialised caseload in the relevant clinical area. Patients may include those with physical and psychological problems in area of speciality; some of whom may have a level of complexity. Team leader; clinical specialist or advanced practitioners are available for clinical and managerial support.
- To ensure a high standard of care for each patient working with patients, families or carers, multi-agency teams and disciplines involved.
- To communicate with these patients, carers, agencies and other disciplines in an appropriate manner, often with sensitive information, diagnoses or outcomes.
- To supervise, teach and appraise peers, physiotherapists, physiotherapy assistants, technical instructors, students and other members of the multidisciplinary or multiagency teams.

The position holder will be expected to work part of the week from an office in the Central Cornwall area to provide a face-to-face clinic. The other days will be completed remotely.
Main duties of the job
As a Senior Psychological Wellbeing Practitioner (SPWP) you will support the Service Lead, Professional Lead & Team Managers to develop the Step 2 service provision across the organisation.
This role provides an opportunity for experienced PWPs who are committed to developing their careers to share their interest and passion and to develop these qualities and skills in others. In addition to maintaining a PWP (Step 2) caseload, the post holders will work within the Talking Therapies service providing a range of more senior duties including:
Clinical support and guidance around clinical suitability and patient risk across the organisation.
Clinical supervision/case management of both qualified & trainee PWPs.
Clinical skills training.
Referral assessment, identifying & ensuring appropriate and current level of intensity for patients.
Clinically support/mentor less experienced PWPs.
Lead on low intensity service developments and deliver in house training workshops.
Develop, support and train innovative practice to trainee PWPs and to other workers qualified to act in low intensity worker roles.
Integrate care with physical health services to improve physical and mental health outcomes for clients with long-term conditions.

Main duties of the job

The successful candidate will be well organised and able to prioritise tasks. They will be proactive in solving problems and queries and you will see these through to completion. The post holder will need to be calm and patient, with an excellent telephone manner, communication, interpersonal and organisational skills to deal with sometimes complex and difficult issues over the phone.
Main duties of the job
You will be supporting a multidisciplinary team and will be involved with answering calls to the service, arranging appointments, audio typing letters, scanning, and uploading documentation onto appropriate systems, photocopying, minute taking, ordering stationery, health rosters, and some general administrative work as required.
This role will be challenging and varied, but ultimately a rewarding role that will provide an opportunity, for the right person, to become a key member of the team. A high degree of flexibility is required to meet the day-to-day challenges of the service whilst respecting the dignity, privacy, and confidentiality of each individual patient.

We are looking for a qualified Clinical Associate Psychologist to join the East CAMHS Team
Applicants will need to be self-motivated,ideally have prior experience of working in an acute mental health services and have completed the MSc course in Clinical Associate Psychology to be considered for this post.

We are seeking an enthusiastic and highly motivated practitioner to become part of a small, supportive multidisciplinary team providing specialist neurodevelopmental assessments for children and young people who are exploring a diagnosis of Autism Spectrum Condition (ASC), ADHD, or other neurodevelopmental differences. The successful candidate will play a key role in delivering high-quality neurodevelopmental assessments and providing expert advice and support throughout the assessment pathway. They will work collaboratively with children, young people, families, schools, and partner agencies to promote understanding, inclusion, and positive outcomes for neurodivergent young people.
Main duties of the job

As a Neurodevelopmental Practitioner, you will:
- Participate in the delivery of specialist Autism and ADHD assessments for children and young people with complex presentations.
- Contribute to the provision of high-quality neurodevelopmental diagnostic assessments, formulation, and advice.
- Support the development and delivery of short-term interventions for children, young people, and their families.
- Provide consultation, advice, and training to parents, carers, schools, health colleagues, social care teams, and voluntary sector organisations.
- Promote a neurodiversity-affirming approach across the wider professional network.
- Work collaboratively with professional leads and colleagues to develop and improve the neurodevelopmental service.
- Contribute to clinical governance, quality improvement initiatives, and service development activities within the Neurodevelopmental Assessment Team.
