Job summary
We are a 26 bed gastroenterology ward looking to recruit a Discharge Coordinator to our team. This post entails arranging safe discharges for our patients when being discharged from hospital. This will need good communication with patients, relatives and multidisciplinary team members within and outside of the hospital. This will include discussion with social workers and district nurses.
Main duties of the job
The successful candidate will arrange the safe discharge out of hospital for our patients. Good communication will be needed to establish what services need to be set up to facilitate a smooth, safe transition out of hospital. Good working relationships will be needed with the multidisciplinary team to work together to determine what care and services are needed to safely discharge the patients. Good telephone skills will be needed and documenting in the patients records.

To contribute to an efficient and accurate pharmacy service to ensure the safe and effective use of medicines throughout at SHSC NHS Foundation Trust.

Under the direction of the Chief Pharmacist, Deputy Chief Pharmacist and the Senior Pharmacy Technicians, the post holder will perform whatever duties are reasonable and necessary to achieve the job purpose and overall safe provision of pharmacy services throughout the Trust.
Main duties of the job
To work closely with the senior pharmacy technician, lead pharmacist and pharmacy team in providing pharmacy services to SHSC providing the following services:
Participate in the technical support necessary for the safe and effective dispensing of prescriptions including inpatient, outpatient and discharge medications and the distribution of medicines to the designated units of SHSC under the supervision of a senior technician and pharmacist.
Undertake the SHSC system of ward top up safely and effectively to designated units.
Provide safe and effective supply of pharmaceuticals to units within SHSC and other designated units outside the Trust.
If eligible, and subject to appropriate training, to undertake accuracy checking of prescriptions.
Participate in early morning, late night, weekend, and bank holiday rota.
Give advice to staff and service users, providing information on local guidelines and policies ensuring they remain up to date.
To order any medicines required for service users and ward stocks.
To maintain continued professional development (CPD) as required by the GPhC or other registered body.

We're looking to appoint a Performance Analyst to join our friendly and well-established Performance and Analytics Team. It's a fantastic time to join Sheffield Partnership as we harness the benefits of our new electronic patient record (EPR) system and embed a new data warehouse alongside Power BI, supporting us to become an even more data-driven organisation.
We offer a range of training and education opportunities as well as on-the-job training when learning new skills or systems such as Power BI.Hybrid working: our staff work a mix of from our office, clinical sites and from home. Everyone is also welcome to work in the office full time. Time co-located with clinical teams every week is essential, and the successful candidates will need to travel to a variety of clinical sites across Sheffield as required.

Main duties of the job
The Performance and Analytics Team:

- The Care Act was the most significant change in social care legislation in 70 years. The Act introduced a number of new duties for local authorities, including the wellbeing principles and statutory responsibilities to carers and safeguarding adults at risk of harm or neglect. Job summary
The Specialist Community Mental Health Team (SCMHT) is a multidisciplinary team that serves the town of Wigan, working closely with all other mental health Teams in the Wigan directorate. Its aim is to facilitate the treatment and management of adults of working age (18+) with severe and enduring mental health disorders, with associated complexities, functional impairment and risks, in the community setting.
The Wigan SCMHT operates on a sectorised model, ostensibly divided into 2 SCMHTs each covering their own geographical area within Wigan. The SCMHTs are based at the Pennington Unit, Leigh Infirmary and Claire House, Phoenix Way, Ince in Makerfield respectively.
Each SCMHT has 2 consultant psychiatrists, supported by a full-time speciality doctor and 2 full-time resident doctor trainees (the type of which vary in each rotation). Each CMHT also has a team manager, deputy team manager, advanced nurse practitioners, CPNs, psychologists, clinical pharmacists, OTs and support workers. Social Workers input into the SCMHT from the local authority for any specific social care assessments and/or interventions.
Main duties of the job
Clinical role
This a replacement post. In line with New Ways of Working, the post holder will provide a consultative, supervisory and supportive role to the multi-disciplinary team. Expected case load is around 330. the majority of clinical work is clinic based, but service users may also be seen at home, as required. The clinic can be at different sites, within short distance from the base.
Undertake assessments of referrals/ admissions to the service
Diagnose and formulate management plans in complex presentations.
Provide management in treatment resistant and complex presentations
Assessment and management of risk including of vulnerability, suicide risk, risks to health and safety and risk of harm to others.
Preparation of Mental Health Review Tribunal and Managers' Hearings reports and attendance at subsequent hearings.
Work in a collaborative way with MDT including external partners.
Work in partnership with psychiatrists from partner providers such as Forensic and Rehabilitation Psychiatrists.
Leadership role
Lead clinical decision making in MDT to ensure the delivery of high-quality care
Develop service clinical priorities in line with national and professional guidance and standards
Engage with service transformation programmes including attendance at Consultant meetings .
Educational role
Provide clinical supervision for trainees in gaining core psychiatric experience
Provide clinical supervision for trainees in achieving competencies in core general psychiatry

Main duties of the job
The postholder will be employed by Greater Manchester Mental Health Trust (GMMH) to work across and provide clinical leadership to psychology staff within the Wigan Specialist Community Mental Health Team (SCMHT, working age adults) in relation to all aspects of direct and indirect psychological work.
The post holder will maintain oversight of the work of more junior psychologists/psychological therapists and Mental Health Wellbeing Practitioners based within the Wigan community teams. They will ensure that all psychological aspects of SCMHT service provision continue to be maintained at the highest possible standards, ensuring people who use the service, and their carers/families, receive high quality care.
The postholder will maintain an overview of non-psychology practitioners' psychosocial intervention work via the provision of co-working opportunities and case formulation, clinical supervision, reflective practice, and training sessions as needed.
They will provide expert psychological assessment, formulation, consultation, and intervention, and play a key role in the continued development of the Psychological Therapies Hub and the wider transformation of community mental health services.

Highpoint is spilt into 2 sites, the North Building has around 390 males and the South Building has around 950 males. Both sites have designated Healthcare Departments which you will be based in.

- Monday - Sunday service 37.5 hours per week.
- 7.45am - 18:15pm - NO NIGHT SHIFTS.

As a Senior Primary Care Nurse your responsibilities will vary. You will

Job summary
An opportunity has arisen within the Community Drug and Alcohol team for a Community Substance Misuse Worker to join a developing substance misuse team within the Ceredigion locality, based in Aberystwyth.
We are looking for an individual who has experience of working with substance misuse and can demonstrate core and specific competencies related to tier 3 substance misuse work or who is keen to develop their skills and experience within a tier 3 substance misuse service.
Experience in the assessment and treatment of service users with opiate dependency would be an advantage as well as a good working knowledge of harm reduction approaches and managing prescribing regimes within a recovery focused treatment framework.
Main duties of the job
The post holder will act as care manager for the caseload and any significant change in circumstances that may result in increased risk to the service user or their significant others must be discussed in the appropriate forum. This will include:
Comprehensive assessment of the service user's substance misuse problem including their health and wellbeing needs and level of motivation to change
Agreeing a care plan /aftercare / discharge plan with the service user that may include other professionals or onward referral as appropriate.
Implementation and review of the care plan and associated evaluation in line with service and Health Board policy
Consideration of the needs of significant others and the opportunity to engage them in the care plan.
Joint working with partner agencies and contributing the treatment and clinical components of the agreed care plan where the service user is subject to a treatment order through the courts
