To provide a specialist clinical SLT service to children and young people (CYP)with speech, language, and communication needs which may include CYP with Autism Spectrum Disorder (ASD), Social Emotional and Mental Health (SEMH) needs and Learning Disabilities (LD).
To be an integral member of the multidisciplinary team providing an integrated care pathway.
To participate in the co-ordination, development and delivery of training programmes and information for school staff, parents and carers and colleagues in health and education.
To work as part of a team with class-teachers, learning support assistants, head teacher, therapists, and the child's family / carers.
Main duties of the job
To provide an assessment and therapy service for children with speech and language needs, and children with complex communication needs referred to the SLT service.
To manage a complex communication caseload independently, accessing clinical/managerial supervision on a regular basis and to make a differential diagnosis on the basis of evidence from assessment, seeking advice when appropriate.
To deliver care in line with the RCSLT/HCPC professional guidelines and standards.
To be responsible for the planning, and evaluation of evidence-based therapy interventions. To monitor and review the progress of therapy programmes implemented and facilitate modifications of interventions / targets as appropriate.
To maintain professional competence through personal development and training. To be responsible for maintaining own professional registration and meet HCPC Registration requirements maintaining a professional portfolio.
To reflect on clinical practice identifying strengths and needs and providing evidence of sound clinical practice.

ABOUT THE ROLE

Job summary
We are seeking a Band 4 Sport and Exercise Therapy Instructor to work across our Psychiatric Intensive Care Unit and Woodlands Inpatient Unit.
This role will work across The Tarn (Psychiatric Intensive Care Unit), Oxleas House and The Woodlands Inpatient Mental Health Unit. The post is embedded within the Occupational Therapy Structures of the Acute and Crisis Mental Health Directorate and has been designed to complement the therapeutic group work and interventions carried out by OT on the wards encouraging service user engagement with their own physical well being.
The post holder will work under the supervision of the Directorate Lead Occupational Therapist to deliver sport & exercise therapy, with support from the Trust Sport & Fitness Lead. This entails planning and implementing an evidence-based exercise prescription programme, tailored to the clinical conditions of the service user/s needs.
The Sport & Exercise Therapy instructor will work with both individuals and groups offering a range of sporting activities
We are looking for someone with experience of working in the field of exercise and fitness who has a passion for bringing these skills to our in-patient services.
Main duties of the job
Facilitate sports and fitness programmes for service users across inpatient sites, offering a range of sports and fitness activities as well as encourage access to community sports programmes to promote social inclusion.
Manage own clinical caseload.
Work autonomously under the supervision of the Directorate Lead Occupational Therapist.

Home Treatment Team is a multidisciplinary team, providing a service seven days a week, 365 days per year to individuals in mental health crisis with the principal aim of providing an effective alternative to inpatient care. Job summary
The Addison Unit is a 12 bedded Unit located at Health care wing at HMP Wandsworth.
The Addison team is a multidisciplinary team (MDT) that provides an assessment and treatment service to patients who present to the acute hospital with mental health difficulties. Referrals are received from various department within the prison and triaged by the in-reach team MDT and placed on a waiting list pending vacancy at Addison Unit. The main responsibilities of the HCA is to support the qualified staff in any way required, to deliver this service.
The HCA has a key role in gathering and recording data, carrying out therapeutic engagement with the patient, and recording any interactions about and with patients on the System one as well as initiating an ACCT and managing presenting risk to safeguard patient. HCA's are also required to support patients in crisis, with personal care and to escort patients who are being transferred to a psychiatric facility or hospital.

- Carry out delegated tasks for patients in mental health crisis or admitted to Addison Ward under the direction of qualified nursing staff.
- Support admissions by recording clinical information and completing physical observations (vital signs), including within the first 72 hours of admission.
- Undertake routine physical monitoring (e.g. NEWS2, ECGs, phlebotomy, blood glucose, BMI) and report any abnormalities promptly.
- Conduct and document daily mental state observations, escalating any deterioration or risk concerns without delay.
- Deliver and support prescribed observation levels (general, intermittent, constant/1:1), ensuring accurate documentation.
- Engage therapeutically with patients, offering emotional support and promoting recovery-focused care.

Working in the Histocompatibility & Immunogenetics (H&I) department, you will coordinate the provision of HLA selected products throughout the UK when requested. The Quality and Nursing Operations Manager has two jobs.
The first is to make sure Affinity Care can show, at any time, that its services are safe, effective, caring, responsive and well-led ready for inspection because of how the organisation runs.
The second is to manage the nursing team: its people, its capacity and the systems it works within.
This is a management post, not a clinical one, and nursing registration is not required. The postholder owns the systems, records and people processes. Clinical judgement on nursing practice, competence and clinical risk sits with the Lead Nurse, who work alongside this post. Knowing which is which, and bringing clinical colleagues in at the right moment, is a core part of the job.

Monday- Friday 8am-4pm
Main duties of the job
Reports to the Operations Manager, with partner support as set out below.
Clinical and professional leadership of nursing sits with the partner leads for nursing and Long term conditions, supported by a Lead Nurse who provides clinical supervision, signs off competence and is the named infection prevention and control lead.
Quality and patient safety is led at partner level by the Medical Director. This posts assurance reports go directly to the Quality and Safety Group.
Works closely with the Planned Care Manager (long-term conditions and vaccination campaigns), the Patient Services Managers (site operations), HR and learning and development, and the colleagues who run the organisations risk register, policy register and complaints process.

looks across them. Where a theme or case involves nursing practice, agree the clinical learning with the Lead Nurse, then take it back to the nursing team through team meetings, clinical supervision or learning events in a way that encourages reflection rather than blame.
Where an individual nurse is involved, hold the feedback conversation jointly with the Lead Nurse the manager covering process and conduct, the Lead Nurse covering clinical practice and help the nurse record it as reflective evidence for revalidation.
Keep the clinical and quality risks on the organisations risk register current, with named owners and mitigations.
Keep nursing and clinical-quality policies and procedures current within the organisations policy register, with clinical sign-off from the Lead Nurse, and make sure staff know what has changed.

An exciting opportunity has arisen for a passionate Psychologist to join our thriving Older Adult Psychology Team within CTM UHB, on a permanent basis. The successful candidate will join the new Dementia Intensive Support Team (DIST).The team will provide rapid assessment and intensive support for people with dementia / or those with a working diagnosis of dementia and their carers or care providers, to manage a period of acute distress and avoid further deterioration.
The service will be provided in the community, wherever individuals call "home", including their own home, care homes (residential and nursing), and will also inreach into identified day centres.Working across CTMUHB, the successful post holder will experience the breadth and diversity of clinical work that this specialty has to offer.
We are in the position to recruit Psychologists who already have the competence and experience relevant to an 8a position, or Psychologists who meet the qualification, training and competence for a band 7 post. In the case of the latter, we are able to offer a professional knowledge and skills-based development programme that will provide you with the opportunity to acquire the appropriate skills and competencies to enable your progression into the 8a role within an appropriate timescale.
The successful candidate will be well supported by senior Older Adult colleagues, and provided with a range of opportunities for career development.
Main duties of the job
The post holder will work within the new Dementia Intensive Support Team (DIST), providing specialist psychological services to people living with dementia, wherever they call home.
They will promote the use of psychological formulation and intervention (Newcastle Model) in the care provided to people living with dementia. Their work will include psychological assessments, interventions and consultation to clients and their carers when living with or supporting complex presentations (behavioural and emotional expressions of need) in the context of dementia. It also involves providing training and supervision to other disciplines in psychological approaches. This post holder will work autonomously within professional guidelines and the overall framework of the service's policies and procedures.
