We are looking for a full-time Medicine Care Coordinator to join our team at Jenner Health Centre. This is an exciting opportunity to work across Modality Lewisham Division practices as part of a multidisciplinary healthcare team, working closely with the Lead Pharmacist and PCN Manager.
You will play an important role in supporting the delivery of medicines-related services, helping the team achieve key performance indicators and targets while contributing to improvements in patient care, service delivery, and ways of working.

Main duties of the job
As a Medicine Care Coordinator, you will work closely with the Lead Pharmacist and PCN Manager to support the delivery of medicines-related services and achieve key performance indicators and targets outlined within standard operating procedures and the business plan.
This is a dynamic role that requires innovation, flexibility, commitment, and the ability to work resourcefully as part of a multidisciplinary team. You will contribute to the delivery of high-quality services while helping to identify opportunities to improve processes, patient care, and the overall patient experience.
You will work directly with key stakeholders to help harmonise ways of working, improve working practices, and contribute to sustainable improvements across Primary Care.Key responsibilities include
- Working with the Lead Pharmacist and PCN Manager to support the achievement of KPIs, targets, and performance indicators
- Contributing to the delivery and coordination of medicines-related services across the Division
- Working collaboratively as part of a multidisciplinary healthcare team

Job summary
The Queen Elizabeth the Queen Mother Hospital is a busy district general hospital close to the Kent coast and surrounded by countryside. We have 3000 deliveries per year. The labour ward has 8 delivery rooms and a dedicated obstetric emergency theatre which is present on the labour ward.
The Trust hosts medical students on attachment from GKT, UCL and Imperial Medical Schools and became a University Hospital linked to GKT in 2009.
From September 2020, we started to host an intake of medical students from the new Kent and Medway medical school which is based in East Kent.
Main duties of the job
The job plan will contain 10 Programmed Activities per week consisting of 8 direct clinical care PA's and 2 supporting professional activity PA's. The precise length of Programmed Activities may vary from week to week around the average assessment set out in the Job Plan.
There will be a Friday on call with out of hours frequency will occur every fourth week (Category A at 8%) with weekend frequency being 1:7 (Category A at 5%) as well as regular antenatal clinics, gynae clinics, operating lists and triage cover. These will either be stand alone or alongside another consultant. There will be timetabled theatre sessions for the planned caesarean birth lists.

The post holder will offer medical input to the Step up hub team South Tyneside. The Step up Hub team Provides an enhanced intensity of interventions to patients with high complex needs , usually in stepped care or supported accommodation.
Main duties of the job
The post-holders will provide Consultant clinical leadership to the service and will assume consultant psychiatrist responsibility for their proportion of patients being looked after by the team. This includes assuming Responsible Clinician status for patients under their care, including patients subject to Community Treatment Orders, taking a lead on compliance with the Mental Health Act and CQC requirements. They will participate, directly and indirectly, in the assessment of patients referred to the service and in drawing up care plans which will address the patients' psychiatric, physical, and social needs. They will carry out detailed psychiatric assessments of patients as clinically indicated to allow accurate diagnoses and contribute to their treatment plans, including pharmacological management of psychotropic medication.
The post-holder will forge good working relationships with other psychiatrists across the Directorate in order to encourage appropriate referrals and assure a cooperative approach to the care of patients. They will be linked in with other Consultants and supported to join peer groups. They will also provide consultation and advice to members of the MDT and primary care colleagues.

Main duties of the job

Job summary
The Childrens Trust supports children and young people with brain injury and neurodisability to live the best life possible. From our national specialist centre in Tadworth, Surrey, and through our community services, we provide specialist rehabilitation, education, therapy, care and family support.
Our services include:
- Specialist rehabilitation for children with acquired brain injury
- A non-maintained special school, supported by residential provision for children with complex education, health, therapy and care needs
- Community-based clinical support for children and young people with acquired brain injury
- Online information and resources for families, including our Bumps Happen hub and publications.
Our Tadworth site provides a child and family-focused environment, including residential accommodation, therapy facilities, outdoor space and specialist equipment. We work closely with the NHS, local authorities and other partners to help children transition between hospital, home, school and community life.
The Childrens Trust is also the only paediatric centre with a national specialised commissioning contract through NHS England to provide brain injury rehabilitation for children with severe brain injury. Alongside this nationally commissioned work, our services are supported through local health, care and education funding, as well as voluntary fundraising.
Main duties of the job
The role of Highly Specialist Physiotherapist is to provide specialist physiotherapy to children and young people accessing The Childrens Trust Rehabilitation Therapy Service. You will lead in delivering high quality, outcome focused interventions through evidence-based interventions and safe care, in order to promote function and participation in home, school and family life.
You will provide leadership to the allocated house-based therapy team, ensuring accountability and effective communication with other teams, creating a cohesive and collaborative multidisciplinary team. Our service delivery aims to promote outcome focussed interventions, safe care, function and participation of children and young people.

We are looking for an Administrative Assistant to join our Adult Mental Health Services based in Severn Fields Health Village, Shrewsbury. The Service offers mental health care and treatment across a variety of different age ranges and presentation. You would predominantly be based in Severn Fields, although you may be required to travel to work in other Trust locations to meet service need.

- To accurately maintain and keep secure patient health and staff records/systems in line with policy and standard operating procedure, to ensure data quality, confidentiality and compliance with the current Data Protection Law.
- Maintain strict confidentiality regarding information concerning patients, staff and sensitive Trust Business at all times.
- To carry out reception duties including meeting and greeting service users, carers, relatives and visitors, as requested by the Admin Support Supervisor and/or the Admin Team Leader.
- Handle incoming and outgoing telephone calls in a polite and efficient manner. Receive and deal with telephone enquiries referring telephone queries to a more senior colleague where appropriate. Taking accurate messages and passing them to the appropriate person in a timely manner.

- To ensure assessments are provided in line with NICE Guidance as well as other relevant local and national documentation.

The Community Children's Nursing Specialist and Family Services team has an opportunity for a Band 6Community Children's Nurse (CCN) to their team in Fenton.
The Community Children's Nurse (CCN) will manage a defined caseload of CYP with a range of acute and complex health care needs, including life limiting and life-threatening conditions requiring palliative and terminal care.
The service aims to support families in caring for their sick children at home, in order to avoid hospital admission and/or facilitate early discharge. This is done by the assessment of children and young people's healthcare needs, and the planning, development, implementation and evaluation of programmes of care which facilitate care at home and in other community settings.
In the role you will have responsibility for the management of a defined caseload and responsibility for the team caseload including out of hours/on call when required. The team provides a 7-day week service to include 24 hours on call for children receiving palliative care.

Main duties of the job
The Adult Community Nursing Service provides care to patients in their own homes, covering care homes and working closely with our GP practices and integrated services such as Physio, OT and Podiatry. As a Community Staff Nurse you will deliver a high quality service, working closely with GP's and multi-disciplinary teams in addressing a range of care issues across the spectrum of health conditions, and preventing avoidable hospital admissions. We provide wound care, palliative and end of life care, leg ulcer management, continence and catheter care, management and administration of injections such as insulin and B12.
You will work independently in a community setting, participating in the ongoing assessment, planning, implementation and evaluation of programmes of care, adhering to policies and procedures.
You must be a qualified nursing practitioner with current NMC registration. Previous experience of working in the community is not an essential requirement, however, you do need to be able to demonstrate a commitment to delivering high quality targeted personalised care.
Post holders will be required to work flexibly on a rota system across a 7 day working week.

The role of the band 4 outreach support worker is to support vulnerable people who have entered the criminal justice system. The service has a focus on supporting adults who have been released from police custody and may have additional health needs. The focus of the team is to provide support for a period of up to 12 weeks in the community to reduce risks around reoffending and also support health vulnerabilities.
Main duties of the job
The main focus of the outreach service is to support vulnerable adults to engage in the services available in the community. This may mean supporting people to health appointments, engagement in mental health and drug and alcohol services. Support with accessing online therapeutic interventions that will be available. Providing psycho social approaches, advice and guidance to help support health inequalities and support approaches to help reduce the risks of reoffending.

We are offering an exciting opportunity for a Band 3 Complex Discharge Tracker for the Home First (Discharge to Assess) in East Staffordshire to cover Maternity Leave.The Team is an integrated clinical and care and support service providing short term reablement intervention to enable people to maximise their independence and meet their health and social care needs.
You will be responsible for scheduling and allocating care calls on the Complex Discharge Tracker System and will be working in partnership with the Home First Locality Teams to ensure that all referrals are allocated care calls within the discharge to assess timescales andwill ensure that the Complex Discharge Tracker system is validated and updated within each locality teams.
We are looking for a team member who is enthusiastic, passionate and a dynamic individual. There will be a requirement to deliver face to face care/support to our community patients on an adhoc basis. The post holder is expected to commute to the office bases across South Staffordshire, if required. Shift patterns are on a rotational basis over 7 days, working early and lates shifts therefore flexibility is required for this role.
Main duties of the job
As a Complex Discharge Tracker, you will review all daily Home First referrals with the Locality Teams and ensure that all patients are allocated the appropriate care calls within the discharge to assess appropriate referral timescale.
As part of this role, you will be expected to:
- Ensure that all Home First referrals from the acute portals are immediately sourced and prioritised throughout the working day.
- Attend all CDT training and briefing sessions as the system evolves.
- Ensure that all updates recorded in CDT are accurate before entering them. Ensure that any identified mistakes are corrected immediately.
- Provide care/support to community patients in their own homes on a adhoc basis.

Job summary

Specimen Reception is a busy laboratory whose primary function is to support the Medical Biochemistry and Immunology/Haematology analytical laboratory services. The laboratory receives a range of specimens for a wide variety of clinical investigations.
Main duties of the job
The successful candidate will be involved in the receipt and checking of specimens, entry of patient and sample details into the laboratory information system, pre-analytical sample preparation and answering general telephone inquiries.The laboratory provides a 24 hour 365 day service and is a UKAS ISO 15189:2012 accredited laboratory. Once trained the successful applicants will be expected to participate in the 24/7 shift pattern.
Working with other Medical Laboratory Assistants, the successful candidate will play an essential role in the delivery of the laboratory service.
To receive, check and distribute samples for analysis received by Specimen Reception.
Complete patient identity checks and to inform supervisor of any discrepancies in labelling.
Barcode specimens/forms and enrol requests into the Laboratory Information System for tests as required.
Complete any additional pre-analytical, analytical and post-analytical duties as required e.g. centrifugation and freezing.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

We have an opportunity has arisen within the Path Links pathology services for a Phlebotomist/Medical Laboratory Assistant (MLA).
We have 1 x 30 hrs vacancies based in Scunthorpe- working at the Scunthorpe hospital site and the Scunthorpe Community Diagnostic Centre.
The Phlebotomy and MLA staff work closely within the department of Blood Sciences, acting as supporting roles for both the taking & processing of blood samples that are then tested using complex analytical platforms.
The Phlebotomy service is based in outpatients & also comprises of a daily ward service. There is also a service provided at our Community diagnostic centre.
Duties associated with the role are the support of the blood test booking line, pathology reception - meet & greet patients, taking blood samples, working in the sample reception area, loading samples on to analytical equipment.
While the role is based at Scunthorpe, there is a requirement to work cross site at Goole, as and when required.
Full training will be provided to the successful candidate. Both theory & practical competency assessments will be completed, consolidating the training received.
Main duties of the job
Applications are invited to become part of our team in the busy and friendly Pathology Department at Scunthorpe General Hospital, Community Diagnostic Centre and at Goole District Hospital. The post will be based at Scunthorpe with the flexibility to work at Goole as and when required, all being part of the Blood Sciences team.
This post is based in the Outpatients of Pathology incorporating Blood Sciences Sample Reception. The successful candidate will, after suitable training, also be expected to take blood from patients within the Pathology Department and ward areas, and undertake other laboratory duties.
Previous experience in phlebotomy and/or a medical laboratory skills would be an advantage. Good communication skills, flexibility and the ability to work within a team are essential.
The post currently requires work to be undertaken between the hours of 8:00am to 8:00pm Monday to Sunday on a rotational basis .

- To assess and make recommendations under the Mental Health Act & fulfil the duties of Responsible Clinician as appropriate.
- To provide (or ensure referral to the appropriate service) for the provision of up to date, evidence based interventions for the patients under their care.

Main duties of the job
To work as an experienced Specialist Paediatric Radiographer within a multidisciplinary team, undertaking a comprehensive range of diagnostic imaging procedures including Major Trauma, Dental, Palatal Studies, Forensic Imaging, Urodynamics, Fluoroscopy, Theatre, Mobile/Portable Imaging, and other examinations in accordance with service requirements and within the scope of professional practice.
The post holder will rotate between Birmingham Children's Hospital and Birmingham Women's Hospital, supporting imaging services across both sites and providing specialist radiographic expertise to children, young people, neonates, women, and their families.
To provide an efficient, effective, responsive, and safe imaging service across both the Children's and Women's hospital sites, supporting acute, elective, inpatient, outpatient, emergency, theatre, neonatal, and specialist care pathways.
To perform imaging examinations to a consistently high technical and professional standard, ensuring image quality is optimised and that all examinations comply with departmental protocols, radiation safety requirements, and professional standards.
The post holder will be required to participate in a 24-hour, 7-day service, including shift work, on-call duties, weekends, and bank holidays, in line with departmental rotas and service needs across both Birmingham Women's Hospital and Birmingham Children's Hospital.

You will work alongside an experienced Specialist Dietitian, helping to enhance and develop our services for adults with eating disorders. Clinical work is varied and hugely rewarding. You will work with patients through the nutritional and physical consequences of eating disorders providing advice to patients at all stages of their treatment. This includes community clinics, group education and supporting acute hospital admissions.
We welcome applications from Dietitians with experience but also sound transferable skills and an interest in this fascinating clinical field. We are a cross-county team so some work across the Suffolk locality may be required.
Main duties of the job
This role expects you to act as a source of expertise on the dietary management of eating disorders to the multi-disciplinary team, community health professionals and others involved in the care of our patients. This includes liaising with colleagues in the local acute trust.
Clinical work is varied and hugely rewarding. You will work with patients through the nutritional and physical consequences of an eating disorders providing advice to patients at all stages of their treatment. This will include patients at risk of re-feeding syndrome and patients with complex dietary needs. We offer dietetic care in a variety of setting including community clinics, groups and support acute hospital admissions
