Job summary
We are looking for an experienced, motivated and caring health care assistant to join our friendly Neighbourhood Teams.
These posts are offered on a six-month fixed-term basis. There may be an occasional requirement to work at other locations across the county to support service delivery, depending on operational capacity.
We pride ourselves on providing outstanding care to patients in the Worcestershire area. Working within the Neighbourhood team is both rewarding and varied, giving you the opportunity to widen your skill base and achieve great job satisfaction. Your role within the team is essential in allowing patients to achieve optimum function within their own home.
The Neighbourhood Team operates from 07:30-18:30 each day and you will work 07.30-15.30, 08:30-16:30 or 10:30-18:30 shift patterns on a roster basis.
Our Neighbourhood team consists of Community Nurses, Physiotherapists, Occupational Therapists, Healthcare Assistant's and administrative staff; with GP and social care colleagues working closely alongside.
We aim to deliver an effective, high-quality service that puts patients at the centre of their own care.

Main duties of the job
As a Band 3 Healthcare Support Worker, you will play an important role in delivering high-quality, person-centred care to patients in their own homes, helping them to maintain their independence, dignity, safety and wellbeing.
Working both independently and alongside colleagues, under the direction of registered nurses and therapists, you will complete delegated care and clinical tasks in accordance with individual care plans, Trust values, policies and procedures. Following appropriate training and competency assessment, your duties may include supporting personal care and activities of daily living; taking, monitoring and recording clinical observations; providing wound and catheter care and simple dressings; undertaking venepuncture; administering medication; supporting safe moving and handling; and fitting and demonstrating the safe use of equipment.
You will develop positive working relationships with patients, carers and families, recognise and promptly report any changes or concerns relating to a patient's condition, contribute to care planning and reviews, and communicate effectively with the wider multidisciplinary team. You will also maintain accurate, timely and contemporaneous records using EMIS Community, while promoting patient choice, confidentiality, infection prevention and consistently safe, compassionate care.

We are looking for an experienced, motivated and caring health care assistant to join our friendly Neighbourhood Teams.
These posts are offered on a six-month fixed-term basis. There may be an occasional requirement to work at other locations across the county to support service delivery, depending on operational capacity.

Main duties of the job

An opportunity has arisen for a Band 6 Assessor/Keyworker in the Bristol Early Intervention for Psychosis Service. This is a fixed term contract for 6 months.
The Early Intervention for Psychosis Service will provide help and support to young people over 14 years and under 35 years of age for 3 years following a first episode psychosis or for a limited period where there may be a possible psychosis.
The team is multidisciplinary, consisting of Peer Support Worker, Physical Health Worker, Mental Health Support Workers, Individual Placement Support Workers, Care coordinators with Occupational Therapy, Nursing or Social work registrations, Administrators, Consultant Psychiatrist, Team Manager, Senior Practitioners, Non Medical Prescribers, Psychologists, Cognitive Behaviour Therapists in Psychosis and many of the team are trained in Family Intervention for Psychosis.
The team act as a resource to the adult mental health community, CAMHS and inpatient services, by raising awareness of the needs of this client group, providing information and consultation about psychosis.
The successful candidate with be able to bring relevant skills and experience to the role, and also be able to develop further due to the opportunities for in house learning and a wide range of supervisions that the team offer. The successful candidate will be able to manage a complex caseload of 20 people.
Main duties of the job
The successful candidate needs to have a current registration with NMC, Social Care England or HCPC.
The role includes Keyworking to provide support to our Service Users and their friends and families. Linking in with the MDT, ensuring support, treatment and interventions are offered within good time, and build relationships with the teams and organisations the EI service works closely with, E.G, Crisis teams, Partnership organisations, Hospital inpatient units etc. In addition there will be opportunities to co-facilitate the service's wellbeing, social and therapy groups.

Pharmacy Technical Services is a dynamic and evolving area of practice, that plays a vital role in the safe preparation and supply of complex medicines to patients. Our team prepares and supplies systemic anti-cancer therapy (SACT) to patients across all four hospital sites within the Health Board and delivers a growing clinical trials service, providing investigational medicinal products to each site. Main duties of the job
Recording of accurate and up to date ECT treatment figures for use in management reports, reviews and audits. Additionally these will aid the ECT Manager to guide service needs.
Responsible for the procurement of office and medical supplies via Business World and NHS Supply Chain. Liaising with colleagues to monitor inventory levels. Maintaining accurate records of procurement transactions.
Distribution and collation of post ECT treatment follow up questionnaires to patient and carers.
Maintaining adequate number of ECT care pathways for referrals and other related documents; generating and posting appointment and discharge letters; general filing and the design and reformatting of clinical documents as required.
Attend meetings relevant to post and cover for team member absence.
Collation and recording of data in relation to physical health in the seriously mentally unwell (SMI) population. Circulating relevant information to internal and external medical professionals. Working within the boundaries and confidentiality and GDPR.
Provide general support and assistance to the ECT manager and team with ad hoc projects.
Supporting the ECT Manager in answering Freedom of Information Requests.

An opportunity has arisen for a part time 3 day a week permanent Band 6 Speech & Language Therapist in our friendly and innovative Speech and Language Therapy team. We are looking for a dynamic SLT to provide assessment, advice and intervention for children within our core service working in preschool, clinic and school based services. Rotherham is a friendly and dynamic Speech and Language Therapy department. The department offers a formal mentoring, peer support and a Personal Development Review system which ensures that training and learning needs can be identified and met appropriately.

Main duties of the job
This role will be working with children in preschool, clinic and school based teams . Duties will include taking case histories, assessment and intervention, writing advice for EHCPs and programmes of intervention , working with a referring on to other practitioners in health and education.
There will also be opportunities to work alongside other colleagues running groups and providing training for schools.
You will also be part of our digital project which may involve delivering pre written scripts , training and voice overs or being available to man our pre referral helpline.

With You in Kent for Young People is the commissioned provider of substance use support for young people aged between 10-25 years old. We offer a range of free and confidential services to young people, professionals and their families. As a service, we offer both short term and long term one to one work with young people and a range of group work options. All interventions stem from the service's ethos of providing accurate, individualised and non-judgemental support and guidance around drugs and alcohol.
Main duties of the job
This role involves working one to one with young people aged 10-18 years old delivering a range of harm reduction interventions in relation to drugs and alcohol including ketamine, cannabis, alcohol, cocaine and benzodiazepines.
Our young people require a holistic approach and may be experiencing mental health challenges, missing episodes, offending and familial breakdown. As such, you will work closely with external agencies and families to help our young people achieve the best outcomes. You will carry and manage a caseload of young people, engaging them in sessions in the community such as at school, their home or in community buildings. You will be supported with case management through line management and group reflective practice sessions in addition to team meetings and practice development days.
