Job summary
We are looking for an experienced and motivated Clinical Pharmacist to join Jurassic Coast Primary Care Network on a 12-month fixed-term contract to provide maternity cover.
Working across our member GP practices, you will be part of a supportive multidisciplinary team, providing patient-facing clinical pharmacy services and helping to improve the safe, effective and person-centred use of medicines.
The role will include Structured Medication Reviews, medicines optimisation, medicines reconciliation, support for patients with complex polypharmacy and long-term condition management. Experience in areas such as type 2 diabetes, hypertension and lipid management would be particularly desirable.
You will work closely with GPs, practice teams, community pharmacy, care homes and secondary care colleagues, contributing to high-quality clinical care across the PCN.
This is an excellent opportunity for a pharmacist with strong clinical skills who enjoys autonomous patient-facing work and collaborative working within primary care.
Main duties of the job
The post holder will provide a patient-facing clinical pharmacy service across Jurassic Coast PCN practices, working closely with GPs and the wider multidisciplinary team.
Main duties will include undertaking clinical medication reviews and Structured Medication Reviews, medicines optimisation, medicines reconciliation following hospital discharge and supporting patients with complex polypharmacy.
The role will also include supporting long-term condition management, including areas such as hypertension, lipid management and type 2 diabetes, where this falls within the post holders competence and scope of practice.
The Clinical Pharmacist will provide medicines advice to patients and healthcare professionals, support safer prescribing, contribute to medicines-related quality improvement and audit work, and help implement local and national guidance.
They will also work collaboratively with community pharmacy, care homes, secondary care and other local healthcare services to support safe and effective use of medicines across the PCN.

We are looking for an enthusiastic and hardworking Housekeeper who will work as an integral part of the team ensuring a clean, safe and comfortable environment at all times.
Applicants must have excellent communication and organisational skills and be able to work under non-direct supervision, prioritising your own workload. It is necessary to have a flexible approach within this role and be able to respond appropriately to the demands of an ever changing workload.
You will need excellent interpersonal skills, as you will be liaising with and helping patients throughout the working day. You will also be required to communicate with all levels of staff both face to face and over the telephone.
The post holder will be expected to take ownership of the stock levels within the department, to include ordering of stock. You will also be required to maintain a high level of cleanliness within the department.
Main duties of the job
Recent experience in cleaning duties and use of chemicals and equipment is essential along with an awareness of health and safety as you will be working in a demanding environment. You are also required to be IT literate. Previous experience with stock control and the use of oracle is desirable.

Job summary
Lung Cancer Screening (LCS) is the national NHS screening programme that is currently delivered within Cheshire and Merseyside (C&M) aimed to facilitate earlier diagnosis for patients considered to be at high-risk of developing lung cancer.
Liverpool Heart and Chest Hospital are the Single Point of Access (SPA) provider for Cheshire and Merseyside.
Eligible participants aged between 55-74 who smoke or have ever smoked will be invited to a lung health check with a lung specialist nurse/nurse associate who will assess and provide lung health advice; the results of the assessment will be used to calculate a person's individual lung cancer risk score. Anyone at high risk will be invited to have a low-dose CT scan.
The aim of the programme is to deliver a service that supports and facilitates the early detection and treatment of lung cancer and to improve the respiratory health of the population.
The Administrators will be responsible for providing administrative support for the LCS programme. Working as part of a multi-disciplinary team they will schedule appointments/invitations adhering to programme timelines, be a point of contact for participants, provide call handling services, and provide the nursing team with administrative support to ensure outcomes are issued in line with key performance indicator (KPI) targets.
The role will require a flexible approach, and the candidate will be required to work in a fast-paced environment, delivering to tight deadlines.

We are looking to recruit a Locum Consultant Neuro Interventional Radiologist on a 6-month fixed term contract.
Based in our Neuro Radiology Department at LTHTR, covering our 2 hospital sites: Royal Preston Hospital & Chorley District General Hospital.
Opportunities exist to join our regional neurosciences centre for Lancashire and South Cumbria; with the exciting opportunity to support the development and expansion of the Thrombectomy Service along with other recent major service developments which include accreditation as a major trauma centre and designation as a tertiary vascular centre.
We would like to invite you to be a part of our exciting personnel and recent service expansion of the Stroke Thrombectomy service, which includes installation of a 2nd bi-planar Angio- suite.
The post holder will be expected to provide support for our comprehensive Interventional and Diagnostic neuroradiology service and will work alongside our existing team of neuroradiologists (6 of whom perform neuro intervention) and 1 head and neck radiologist.
Any person who is unable to work full-time for personal reasons may still be considered for the post.
Main duties of the job
The post holder:
- will be expected to support the provision of a comprehensive Neuroradiology service and work alongside the existing team of neuroradiologists, 6 of whom perform neuro-intervention, along with supporting advanced neuro-imaging and head and neck radiology with reporting, MDT presentation.
- in conjunction with colleagues, an active role in the planning, development and delivery of the neuro radiology service for the Directorate and associated Network. There is also an ongoing commitment to teaching SPRs who rotate through attachments within the Trust.
- Support diagnostic neuroradiology colleagues in both IP and OP MRI/CT reporting. Also close working with colleagues in the wider neurosciences and radiology department is expected as part of the daily work.
- Support the stroke thrombectomy service expansion.
- Provide support for the local and Network neuroradiology MDTs.
There are also substantial opportunities for development or maintenance of other subspecialist interests.
The successful candidate will be expected to participate in a neuroradiology on call rota with 1:8 frequency [under review].
Experience in head and neck radiology and advanced neuroimaging techniques is highly desirable.

Main duties of the job

Job summary
The postholder will be responsible for working within a friendly team of four, efficiently and effectively managing the clinical rotas for Brownlow Group Practice. They will work closely with clinical and management teams to maintain the core clinical rota, ensuring all job plan changes, new starters, and staffing updates are accurately reflected. The salary for this role follows a fixed incremental pay scale, this starts at £30,250 per year, rising yearly in line with our salary progression bands, up to £34,900 per year. This includes a £4,000 per year inner London weighting payment.

Main duties of the job
Our Recovery Workers work with our clients to help them to determine what their goals, ambitions and passions are, working with them to create the best treatment plan for their needs and to get them on the path to recovery. This is an integral role for With You - as a frontline worker, you will deliver continuity of care through effective partnership working, care coordination, delivery of interventions and key work responsibilities. You will manage an active caseload of service users, connecting quickly with the people we support, treating them respectfully and with dignity.
You will be a compassionate and motivated individual, with an exceptional ability to build meaningful relationships. We're looking for a motivated and compassionate person with lived experience of the criminal justice system to join our Liaison and Diversion team part-time. You'll play an important role in supporting people as they move through police custody, court and prison, helping them access the support they need to improve their health, wellbeing and future.
Lived experience is essential as you will use this to manage a caseload of people using our service, supporting them for up to 12 weeks for 'Liaison and Diversion' cases and up to 6 months for 'Reconnect' cases.
You'll work alongside each person to understand what's important to them and help them access services that meet their needs. This may include support with; Mental health, Learning disabilities, Neurodiversity, Housing, Social isolation, Education and training, Drug and alcohol support and Employment.
Building positive relationships is a key part of the role. You'll work closely with service users, their families, police, courts and a range of partner organisations to ensure people receive joined-up support.
Main duties of the job
- To hold a caseload of service users for up to 12 weeks (for Liaison & Diversion cases) and up to 6 months (for Reconnect cases) to support them to access a wide range of agencies and services that can provide help and advice.
- To facilitate engagement with appropriate services, by identifying unmet needs including, but not limited to, mental health, learning disability, ASD, ADHD, housing, social isolation and exclusion, education, debt management, drug and alcohol, and employment problems.
- Support service user who are typically hard for services to engage with, to access and utilise community services in order to meet identified health and social needs.
- Build on professional and supportive relationships, including family, police, court staff, and other partner agencies etc.
Working pattern and service operating hours:
This role is based on Monday to Thursday, 9:00am to 5:00pm.
Our wider Liaison and Diversion service operates across police custody suites between 7:00am and 8:00pm, seven days a week, and in courts between 9:00am and 5:00pm, Monday to Friday.
You'll work across sites throughout Hampshire, so you'll need to be able to travel. Please note that some locations are not easily accessible by public transport.

Main duties of the job

Job summary
We are looking for an experienced, motivated and caring health care assistant to join our friendly Neighbourhood Teams.
These posts are offered on a six-month fixed-term basis. There may be an occasional requirement to work at other locations across the county to support service delivery, depending on operational capacity.

The Neighbourhood Team operates from 07:30-18:30 each day and you will work 07.30-15.30, 08:30-16:30 or 10:30-18:30 shift patterns on a roster basis.
Our Neighbourhood team consists of Community Nurses, Physiotherapists, Occupational Therapists, Healthcare Assistant's and administrative staff; with GP and social care colleagues working closely alongside.

Main duties of the job
As a Band 3 Healthcare Support Worker, you will play an important role in delivering high-quality, person-centred care to patients in their own homes, helping them to maintain their independence, dignity, safety and wellbeing.
Working both independently and alongside colleagues, under the direction of registered nurses and therapists, you will complete delegated care and clinical tasks in accordance with individual care plans, Trust values, policies and procedures. Following appropriate training and competency assessment, your duties may include supporting personal care and activities of daily living; taking, monitoring and recording clinical observations; providing wound and catheter care and simple dressings; undertaking venepuncture; administering medication; supporting safe moving and handling; and fitting and demonstrating the safe use of equipment.
You will develop positive working relationships with patients, carers and families, recognise and promptly report any changes or concerns relating to a patient's condition, contribute to care planning and reviews, and communicate effectively with the wider multidisciplinary team. You will also maintain accurate, timely and contemporaneous records using EMIS Community, while promoting patient choice, confidentiality, infection prevention and consistently safe, compassionate care.
