We are recruiting to our new Intensive & Assertive Team, launching across Norfolk and Suffolk to provide intensive, flexible and recovery-focused support for people living with severe and enduring mental illness. This is a unique opportunity to join a service from the outset and help shape its future.
As a Care Coordinator, you will work with people who often find it difficult to engage with traditional mental health services, delivering assertive outreach, intensive community support and recovery-focused interventions. You will help improve engagement, reduce hospital admissions and support individuals to live more fulfilling lives within their communities.
Posts Available and Locality Working
We currently have 12.5 WTE Care Coordinator vacancies across Norfolk and Suffolk. Successful applicants will have the opportunity to discuss their preferred locality, and wherever possible we will align staff to their chosen area to minimise travel and support strong local partnerships.
As a county-wide service, there may occasionally be a requirement to support colleagues and service users across your allocated county in response to service needs. Any wider working will be reasonable, proportionate and fully supported.
The team operates 7 days a week, 11:00am-7:00pm, with staff participating in a rota that includes weekends and bank holidays. We value flexibility, teamwork and work-life balance, and offer the chance to be part of an ambitious and innovative new service from day one.
Main duties of the job

- Provide care coordination to individuals with severe and enduring mental illness.
- Undertake comprehensive mental health, risk and psychosocial assessments.
- Develop, implement and review co-produced care, recovery and crisis plans.
- Deliver evidence-based interventions in accordance with national guidance and Trust policies.
- Work proactively with individuals who may struggle to engage with traditional services.
- Deliver assertive outreach interventions within community settings, including service users' homes.
- Promote recovery, social inclusion and independence.
- Support individuals to access physical healthcare, housing, welfare benefits, employment, education and community resources.
- Undertake risk assessment and formulate effective risk management plans.
- Respond to crises and support admission avoidance where appropriate.
- Work collaboratively within a shared caseload model.
- Develop effective partnerships with carers, families and external agencies.
- Maintain accurate and timely clinical documentation.
- Contribute to service development, quality improvement and clinical audit activity.

Main duties of the job
To ensure the reception and administrative functions of the practice run smoothly, providing a high level of service to patients, doctors, nurses, and the wider practice team.
The post holder will be responsible for answering telephone calls, booking appointments, registering new and temporary patients, dealing with prescription requests and queries, giving out results in line with practice protocols, arranging ambulance transport and home visits, greeting and directing patients, handing out prescriptions, collecting payments for non-NHS services, managing general enquiries and complaints, filing and retrieving patient records, inputting and extracting information from the practice computer system, supporting admin tasks such as recalls and cytology, maintaining confidentiality and security, observing health and safety procedures, keeping reception and waiting areas tidy, and communicating effectively with patients and team members. Other reasonable duties may be required.

We are looking for someone with excellent communication and customer service skills, good organisational ability, basic IT skills, and the ability to remain calm under pressure while maintaining confidentiality. We have an exciting opportunity to join our friendly, innovative, and supportive medicines management pharmacy team as a Band 5 Medicines Management Pharmacy Technician (MMT).
This post is suitable for a GPhC registered pharmacy technician wanting to move sector and from any clinical background. This roles base isHellesdon Hospital in Norwichbut travel to different sites may be required.
The ideal candidate will work closely with our pharmacists and MMTs, carrying out various medicines management activities to provide a patient-orientated, high quality and comprehensive clinical medicines management service to designated inpatient wards primarily at Hellesdon Hospital. You will hold an Accredited accuracy checking qualification and be proficient in medicines reconciliation.
You will have a flexible approach to the needs of the service and have a strong team work ethic. You must be well organised and able to prioritise your own work, have good interpersonal skills and be confident working independently, and as a fully integrated member of the multidisciplinary team.
Please note this is a permanent position and 30 hours per week.Ideally, we would like the working hours to cover Monday, Tuesday, Wednesday and Friday with Thursday being a non working day. However, for the right candidate we might be in a position to negotiate these hours.
Main duties of the job
You will be part of a team delivering clinical pharmacy services to:
- Adult mental health inpatient wards (Norwich and Kings Lynn)
- Kingfisher Mother and Baby Unit (Perinatal mental health service - Norwich)
- Crisis Resolution and Home Treatment (Norwich and Kings Lynn)
- Psychiatric Intensive Care Unit (Norwich)
- Drug monitoring clinic (Norwich)
As a medicines management technician, you will:
- Work with other medicines management technicians and team managers to provide adequate cover for activities across dispensary and clinical services, prioritising and directing as required to ensure that optimum service delivery is maintained.
- Order medicines for stock and individual patients at ward-level in preparation for discharge using the appropriate IT systems.
- Participate in medicines management schemes on the wards including Patients Own Drugs, Self-administration, medicines reconciliation and non-stock top ups.
- Maintain a good stock control on the wards.
- Undertake patient counselling about their medicines and provide individualised information and advice about medicines to patients, carers, and other health professionals e.g., medical and nursing staff.
- Involve in discharge planning for service users ensuring a seamless medicines pathway into primary care.
- Participate in Trust wide medicines management initiatives.
- Participate in medicines management audit projects.
- Support the education and training for student pharmacy technicians and trainee pharmacists.

Job summary
At present, University Hospitals of North Midlands is unable to offer visa sponsorship for Band 2 to Band 4 roles, as these positions do not meet the minimum salary and skill thresholds required under UK Visas and Immigration (UKVI) regulations.

Main duties of the job

Job summary
At present, University Hospitals of North Midlands is unable to offer visa sponsorship for Band 2 to Band 4 roles, as these positions do not meet the minimum salary and skill thresholds required under UK Visas and Immigration (UKVI) regulations.

Job summary
To work as a specialist nurse in Cardiology clinics, working as part of the MDT to provide assessment and treatment planning. This role will support the delivery of Atrial Fibrillation, Valve and Rapid access chest pain clinics.
You will demonstrate an extensive cardiac knowledge with competence in assessment and consultation and registered prescriber as well as the ability to work independent and autonomously.
Provide care for patients in a holistic manner, incorporating physical and psychological care to enable them to live with a cardiac condition.
To provide education and support to the cardiology patients.
To be the point of contact for patients in the CNS clinics to give ongoing support virtually and to assess and liaise with other members of the MDT on their behalf.
To work alongside the MDT to deliver the highest standard of cardiac care and liaising closely with community teams to provide a joined up and effective service delivery. Engage with the MDT.

Main duties of the job
Manages clinical risk taking responsibility for the assessment of patient needs and the development, planning, implementation and evaluation of care within the ward and post discharge.
Draws on specialist knowledge to act as an autonomous member of the CNS team, and provide specialised management and care of the cardiac patient.
Effectively deliver a high standard of care to the cardiac patient.
Carefully documenting and sharing information with the MDT in a useful and constructive way. Using methods available to communicate effectively with other health professionals including GP, community nurse specialists and consultants.
To be able to communicate effectively with the consultants and colleagues if there is a patient concern and understand the correct escalation process.

Job summary
We are looking for a dedicated, compassionate, and professional Occupational Therapist Clinical Specialist who wants to make a genuine difference in the lives of people.
You will work as part of our team, liaise and work closely with therapists in the acute hospital and community rehabilitation, as well as our transfer of care hub (TOCH) and community prescribers.
You will lead lead and manage the team of OTs at the Community Hospitals, providing support, mentoring and supervision. You will also manage the internal rotation of the Band 6s within the community hospitals, as well as supporting their rotation whilst they are at the community hospital. You will also be involved in the planning for the OT needs and provision of a new ward at Clacton Hospital opening in the coming months. The aim is for this ward to become an dedicated Assessment Unit, providing rapid assessment, diagnostics, and treatment forappropriate patients, reducing avoidable admissions to the Colchester acute site.
You will support the Physiotherapy Hospital Lead, in managing therapy staffing, rostering, and where necessary, covering in the PT Band 7 Lead's absence.

Main duties of the job
Key Responsibilities
To work as an autonomous practitioner, to be professionally and legally responsible and accountable for all aspects of own work including the management of patients in your case.
To ensure a high standard of clinical care for the patients under your management, and support more junior staff to do likewise.
To gain consent and undertake highly specialist occupational therapy assessments, clinically diagnose and to formulate and implement treatment plans using specialist knowledge and experience. This will include the treatment of complex patients in a manner that respects people's choice, privacy, dignity and individuality.

We are looking for a proactive and enthusiastic dietitian to join us in a Band 6 role to provide support to inpatients with complex nutritional needs including mental health, eating disorders, disorder eating, oncology and parenteral nutrition.
You will become part of a large team of dietitians and dietetic assistants. The team works in a variety of inpatient specialties, community services, paediatrics and renal. There is also a culture of continuous learning for team members and our students and apprentice .
This role offers the opportunity to develop and influence service delivery in a variety of different specialist areas, specifically including mental health, eating disorders and oncology, working closely with team leaders in these specialist areas. You will be supported to gain competency in parenteral nutrition and will design and implement QI projects within their area of practice.
There is an expectation to contribute to our established student-training programme and support other team members in their development including preceptorship.
You will participate in a variety of activities to build your CPD portfolio including training provided internally and access to external training relevant to your role.

You could help us change lives.The role is diverse and will include , welcoming donors, testing donors to see if they are able to give blood on that day, taking blood, maintaining records and offering basic clinical advice and aftercare. You must be able to travel to undertake the duties of the post at various locations. WBS fleet vehicles, team minibus and equipment vehicles are provided.We will give you all the technical training you need to perform venepuncture (using needles to take blood), so you don't need a medical background. However, you will need to demonstrate excellent customer service skills and have a friendly and flexible approach which will make donors feel welcome at ease and valued.Work hours are varied and include long days, Bank Holiday and weekend working.There will be a requirement for you to work in various settings and also on the mobile donation unit.
Main duties of the job
The role will deliver both excellent customer service and direct care to donors throughout the blood donation process. You will be working as part of a multi-skilled team committed to creating a caring environment and excellent customer service experience whilst identifying and meeting the expectation of individual donors so that blood is donated safely and donors are encouraged to return. This will include delivering high standards in customer service, performing venepuncture and driving duties.

An exciting opportunity has become available for a highly motivated, autonomous and enthusiastic Clinical Endoscopist to join us in the Gastroenterology and Endoscopy Service at Whipps Cross University Hospital, part of Barts Health NHS Trust.
This is a full time, senior nursing post, the requirement of which will be to perform high quality Gastroscopy, Colonoscopy and Flexible sigmoidoscopy within a high-volume endoscopy service.
The post holder will play a key role in the delivery of a safe, efficient and patient-centred service within Endoscopy and Gastroenterology within our well-established Gastroenterology Clinical Nurse Specialist (CNS) Team, which includes a Nurse Consultant, Clinical Endoscopist, IBD, Lower GI STT and Upper GI cancer CNS's
Main duties of the job
The provision of a high standard of individualised holistic patient care to all patients attending the endoscopy unit and Gastroenterology Service.
Performing endoscopy procedures which will include diagnostic upper and/or lower GI endoscopy in line with the BSG, ACPGBI and AUGIS guidance on the scope of professional practice for clinicalendoscopists and to work as part of a large multidisciplinary team.
Attendance at MDT meetings in colorectal, hepatobiliary and upper GI.
The candidate will be expected to autonomously manage a caseload of patients using advanced assessment skills within the Gastroenterology Service, including face to face and virtual Nurse-Led clinics within the sub-specialities of upper gastro-intestinal, hepatobiliary and lower GI disease to support in the delivery of new and patient follow up in both routine and Faster Diagnosis Standard pathways.

Job summary
We are looking for an enthusiastic Nuclear Medicine Technologists/Radiographer to join our multidisciplinary team.
Our Nuclear Medicine and PET (Positron emission tomography) CT (Computed Tomography) departments are spread across two sites: Charing Cross Hospital and Hammersmith Hospital. The successful candidates would be expected to cover workload across both sites on a rotational basis. We do not have an onsite radiopharmacy, our radiopharmaceuticals are provided to us by an external supplier.
The trust wide PET/CT service is based at Charing Cross Hospital and utilises a Siemens Biograph Vision scanner. Our PET CT service includes 18F-FDG, 68Ga-Dotatate, 68Ga-PSMA, 18F-Choline, 18F-Amyloid studies and numerous research studies using a wide range of radioisotopes.
At Hammersmith Hospital there are two Siemens Intevo Bold SPECT CT scanners which provide a wide variety of hybrid imaging as well as supporting an ever expanding therapy service. Examples of these are bone, parathyroid, stress and rest myocardial perfusion, DAT scans, 177Lutetium, SIRT and Therasphere therapies.
The successful post holder would be joining an exceptional team who have shared values and provide high quality patient care. We work collaboratively to consistently improve our services, as well as supporting staff to develop and explore their own interests within the nuclear medicine field.

Here at MSE, we are committed to providing a supportive environment for the development of Occupational Therapists. Our Southend Hospital department operates an integrated therapy service, where Occupational Therapists & Physiotherapists work alongside each other in a variety of the clinical areas. We are currently looking for a dynamic Occupational Therapist to join our band 6 inpatient rotations initially working across the elderly / frailty service. This position offers the opportunity to develop clinically and managerially in a fast paced environment working within our established Inpatient Therapy Team.
Our Inpatient Therapy Team is led by our Clinical Lead Occupational Therapist with the support of Band 7 Team Leaders who are highly experienced in their fields. We have regular Occupational therapy team meetings and actively encourage CPD.
In return we offer training and clinical development opportunities with a comprehensive in-service training program and regular clinical supervision. We also provide additional support in order for staff to attend recognised courses within the area of the post to support staff development. The Therapy Team is also committed to supporting the development of leadership skills at Band 6 encouraging staff to attend relevant courses provided by the Trust.
Main duties of the job
The post is part of our Inpatient Team which offers the opportunity to work across a variety of inpatient areas commencing on the elderly / frailty wards.
The role involves the rapid assessment and management of patients presenting to the hospital to facilitate safe discharge as soon as possible with the provision of any necessary equipment and care recommendations.
You will demonstrate an ability to be flexible and thrive in a fast paced environment. You will be responsible for day to day management of the service delivery and manage a complex caseload with support from your band 7 team lead. You will gain experience in supervising staff and become an accredited practice placement educator.
You will possess strong leadership skills and the ability to engage others, with a proactive approach to service development. You will also demonstrate a strong commitment to a person centred approach in line with the Trust Values and Behaviours.
Together with the team leaders in our in-patient specialities, you will by supported the Occupational Therapy Clinical Lead and the Therapies Manager in the strategic and operational planning for the department.

The Portering Supervisor / Working Porter is responsible for the delivery and day-to-day supervision of portering services across designated sites. Job summary
An exciting opportunity has arisen within the Health and Justice service for the role of Reconnect Navigator based within the Black Country to support the strengthening of the female pathway within the service.
Reconnect supports people leaving prison and immigration centres. We will work with people up to 12 weeks prior to release and up to 6 months after release unless their health needs are met before.
The post will be based in the community however forging strong links with the female prisons will be vital to support the transition for those females being released into the Black Country.
It is expected that the post holder will form a strong working relationship with the female pathway leads from local Reconnect services and with the female pathway champions across Health and Justice within MPFT to develop the pathway and support the development of others within the team.
Reconnect is part of an integrated service with Liaison and Diversion and Mental Health Treatment Requirement services as part of the Health and Justice Service.
This is an 'all age' service and so you will be required to work with people from the age of 10 years upwards.
Prison vetting will be applied for as part of this post however please discuss any concerns regarding this as we also welcome applicants with lived experience.

Midlands Partnership University NHS Foundation Trust has an opportunity to join our Older Adult Mental Health Bromley Ward at St Georges Hospital in Stafford.
Bromley Ward is an acute inpatient facility for individuals over 65 years presenting with a functional mental health condition or early stages of dementia, requiring admission for assessment and treatment with a planned discharge into the community.
We provide inpatient facilities for older adults with mental health problems and we are looking for caring, compassionate individual to work within our older adult service who are committed to providing high quality care and treatment to our patients. Working in an older adult environment can be challenging, but with the right team, positive attitude and resilience we are rising to the challenge and reaping the reward of being able to support people during their most difficult times.
Please note you must be able to work night shifts on rotation.
Main duties of the job
- Delivering a high quality clinical service to patients and their families.
- Acting as named nurse to a group of patients under the direction / supervision of the person identified with continuing responsibility.
- Participating in therapeutic group work programmes.
- Taking charge of the ward in the absence of the ward manager and deputy ward manager.
- You will be responsible for the assessment of care needs and contributing to; the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence based interventions.
- You will, in conjunction with the multi-disciplinary team, provide nursing interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical
