Job summary
An exciting opportunity to join H&F Partnership as a Probation Services Nurse to deliver healthcare interventions in probation settings, helping to address health inequalities and improve outcomes for individuals involved in the criminal justice system. The role involves conducting health assessments, providing health education, identifying unmet healthcare needs, and coordinating referrals to relevant services.

Main duties of the job
The following are the core responsibilities of the Probation Office Nurse.
- act as main health contact for patients on the probation caseload.
- complete an initial health screening assessment/template.
- complete ADHD screening assessment where necessary.
- ensure patients are supported and appropriately signposted to relevant primary care services.
- develop, implement and embed health promotion and wellbeing programmes.
- identify, manage and support patients at risk of developing long-term conditions, helping prevent adverse effects on their health.
- oversee chronic disease clinics, delivering care as required and referring patients to secondary/specialist services where appropriate.
- maintain accurate clinical records in line with current legislation and professional standards.
- ensure Read codes are used effectively.
- support the appropriate maintenance and effective use of chronic disease registers.
- act as a chaperone where required.
- prioritise health issues and intervene appropriately.
- recognise, assess and refer patients presenting with mental health needs.
- liaise with external services and agencies to ensure patients, including vulnerable patients, are appropriately supported.

H&F Partnership is a large GP Partnership and a single organisation PCN. Formed in 2016 from the merger of 3 practices, it now extends to five sites, 32 GP Partners and 71000 patients in Hammersmith and Fulham. Our aims are to provide excellent patient care, education across all disciplines of professionals in Primary care (medical students, nurses, pharmacists and others) and primary and applied research and evaluation of new integrated systems of delivering care.

This position is a key leadership role within the Integrated Urgent Care (IUC) Directorate, leading the Clinical Assessment Service to ensure that patients receive a high quality, safe service. The CAS is a multi-disciplinary team of clinicians who provide telephone clinical assessments for patients who have contacted the service via 111 or have had their case transferred from the 999 service.
The role will be integral to the management of the CAS clinical workforce and the post holder will work within the senior management team to deliver, improve and design innovative service delivery models as part of the growth and expansion of the IUC within LAS and the wider London system.
The post holder will be the first line manager for the Clinical Team Navigators within the CAS as well as for the GPs and ACPs, working in conjunction with the CAS GP leads.
There will be a requirement, as part of the role, to provide 24/7 clinical oversight on an on- call basis.
This is a rewarding challenge affording you the opportunity to learn new skills and further utilise your clinical expertise and management skills.
Main duties of the job
As one of the Deputy General Managers you will be responsible for the delivery of KPIs and for implementing plans to improve them. You will be responsible for quality assurance and governance, as well as the day to day and long term management of your team.
This role requires excellent communication skills and the ability to work as part of a team, whilst making autonomous clinical and operational decisions.
As part of the role, you will be responsible for maintaining patient safety by managing and balancing the risk across all areas of the CAS queue, ensuring that appropriate skilled clinicians deliver a safe, effective and efficient 24/7 service to patients.
You will need to be a registered Nurse or Paramedic with previous or current experience working as a Navigator within a Clinical Assessment Service. You will also require a current clinical decision software qualification.
Successful candidates at shortlisting will be invited to attend assessment centres and interview, provisionally planned for week commencing 12th October.

The Thoracic Unit Services
The Unit deals with the full spectrum of adult thoracic surgery apart from lung transplantation and oesophageal surgery. Approximately 1000 thoracic procedures are performed annually. The unit has a national reputation as a large volume minimally invasive surgical unit with established robot and video assisted thoracoscopic surgery programs. Last year, over 95% of procedures were performed using a minimally invasive approach. The thoracic surgery service provides a comprehensive thoracic surgery practice, which includes anatomical (lobar/sublobar) and non-anatomical lung resections, mediastinal surgery, lung volume reduction procedures (LVRS, BLVR), interventional airway management (cryotherapy, laser ablation, stents), chest wall surgery (slipped rib syndrome, pectus correction), pleural procedures and thoracic trauma (rib fixation, emergency procedures). The unit is currently aiming to further expand its services by developing a robotic bronchoscopy program.
Due to the nature of St. George's hospital as one of the largest hospitals in the UK with, uniquely all services on site, Thoracic surgery works with and supports many other services. This includes combined ENT thoracic surgery operating lists and ad hoc operating lists with many other specialities including Plastics, Upper GI surgery and Neurosurgery.
Main duties of the job
Main Duties/Key Results Areas:
Clinical Fellow at SpR level (ST3 equivalent or above) in thoracic surgery duties are:
Caring for inpatients / outpatients and performing surgery. Care of patients in the CITU.
Care of patients on Caroline Ward.
1:7 on call 24 hour with non-resident cover overnight.
The individual is expected to take an active part in all the departmental academic audit meetings.
The post holder will, together with consultant colleagues, be responsible for the provision of thoracic surgical services to St George's University Hospitals NHS Foundation Trust, including:diagnosis and treatment of patients of the Trust in such hospitals, clinics or other premises as is required.

Management Role
Ensure that patient documentation is completed accurately and contemporaneously using SystmOne and paper records, in accordance with professional standards, hospice policy and legal requirements. The Trust seeks to appoint full-time Trust ST3 to ST5 doctors at Middle Grade level, based in the Emergency Department (ED) at Hinchingbrooke Hospital. The postholder may also be required to work cross-site at Peterborough City Hospital dependant on operational needs.
The post is designed to provide support to the Consultant staff and to enhance the clinical supervision of junior doctors (FY2 and ST1-2's). The post holder will join middle grade rota and make a significant contribution to the service delivered to patients within ED.
The applicants should have at least 6 years post registration experience, of which 24 months will have been spent in an Emergency Medicine setting at ST3 level equivalent or higher with a commitment to a career in Emergency Medicine, demonstrated by completing parts of MRCEM/ FRCEM.

Main duties of the job
- The post holder will be part of the Middle Grade shift system providing 24/7 on site cover for the Emergency Department.
- Post holder will work across the full range of emergency services delivering patient care.
- Initial assessment and treatment of adult & paediatric patients arriving in the department by ambulance or other means.
- To carry out manipulation of fractures and dislocations, together with other minor procedures such as suturing and regional block anaesthesia
- An integral part of in-service training for the junior doctors.
- Supervision of FY2 and ST1-2 doctors. In addition we have medical students from Cambridge, ACPs and ENP trainees who are provided supervision and shop floor teaching.
- Participation in the department's teaching programme and appraisal.
- Audit and administrative roles, e.g. Police Statements, Clinical Audits and service development (6hrs per week on average will be allocated for CPD, teaching and administrative work)
- Cover for colleagues during annual and study leave will be integrated in the rota.
- The post holders will work with the audit lead supervising the audit projects of the Junior Doctors and will be required to conduct their own independent audits together with producing action plans as required.

Job summary
We are pleased to offer a practice nurse role preferably with experience but not essential to join our dynamic team in a busy 2 site practice who is committed to excellent patient care. We are looking for highly motivated individual who can positively contribute to the care and wellbeing of patients, adapting to change and support a progressive team.
We have a large team of 4 practice nurses and 3 HCA's, 13 GPs 1 ANP. We also support nursing and medical students and GP registrars. We are a large 19,000+ patient surgery split across 2 sites.
Main duties of the job
Organise and coordinate the provision of nursing services for the practice including management of patients wiyh long term conditions, vaccination and immunisations, cervical screening, general treatment room duties and any other duties as required by the practice within your professional competencies.
Advise patients on general health care and minor ailments with referral to GPs as necessary.
In participation with the lead nurse, ensure the maintenance of equipment stock levels relating to patient care.
Following appropriate training, undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc, and ensure the cleanliness of any equipment used. Perform any investigatory procedures requested by the clinicians.
Participate in the administrative professional responsibilities of the practice team.
Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate
Ensure accurate completion of all necessary documentation associated with patient healthcare and registration with the practice.
Co operate and participate as required in any research projects within the practice.

As part of an effective team, you will be responsible for efficiently assisting in all food service areas/retail outlets as directed by your Team Leader /Line Manager and operating within established food hygiene and safety practices, methods and procedures and within the principles of a flexible team approach.

You will take part in the preparation of raw food materials as part of the process to make our patients nutritional meals throughout their healthcare pathway during their time at CoCH. Applications are invited for the post of Locum Consultant Anaesthetist with special interest in Neuroanaesthesia, based at Addenbrooke's Hospital, Cambridge.
This post is available from February 2027 for a period of 12 months.
Main duties of the job
The successful candidate will join an expanding, innovative and dynamic department of 85 substantive Consultants, 44 Residents and 15 Senior Clinical Fellows, providing anaesthetic services in this thriving modern teaching hospital.
You will be part of a team of 15 Neuroanaesthetists and will be expected to undertake neuro-anaesthesia, interventional neuroradiology, and general anaesthesia lists along with on-call in the Trauma service, as described in the job description.
The trust currently provides a 24/7 stroke thrombectomy service to the whole of East Anglia.
You will need to be fully registered with the GMC and ideally have a CCT in Anaesthesia or be within six months of award of CCT at the time of interview. Applications are welcomed from those unable to work full time for personal reasons or those wishing to job-share.
You will be required to have completed an SIA in Neuroanaesthesia (or equivalent), and must be able to work independently at Consultant level.

An exciting opportunity has arisen for an enthusiastic and motivated Pharmacy Technician to join our Cancer Pharmacy Team on a fixed-term basis to provide maternity leave cover.
This role offers the opportunity to gain valuable specialist experience within oncology and haematology pharmacy services while supporting the delivery of a comprehensive, patient-centred medicines optimisation service for patients receiving cancer treatment at Great Western Hospital.

Specialist Biomedical Scientist
12-Month Fixed Term Contract (Maternity cover)
This role is an opportunity for an experienced, well-motivated Biomedical Scientist with Health and Care Professionals Council registration to join the Great Western Hospitals Chemical Pathology section within Blood Sciences department.
The post holder will join a team of dedicated professionals working to deliver high quality test results and excellent care to patients across Swindon and the surrounding area. The Great Western Hospitals Blood Science department handles over 6 million test requests per year and offers a large repertoire of tests for our patients.
The post holder will be expected to work at a competence level equivalentto the Institute of Biomedical Scientists Specialist Portfolio in Clinical Chemistry and contribute to the effective running of the Chemical Pathology department by performing equipment maintenance, quality controls and troubleshooting procedures. The laboratory operates a quality management system (QMS) which all staff members engage with to ensure all processes are followed according to SOPs and documented according to the departmental policies.

Main duties of the job
The Chemical Pathology department performs analysis of a variety of clinical biochemistry tests through the automated equipment. This is currently provided by Beckman-Coulter and consists of two sets of DxI 800 analysers linked to DxC AU 700 analysers via a Powerlink and a single stand alone DxC AU 700. The department also provides services for xanthochromia, drugs of abuse analysis and HbA1c in a dedicated area of the lab.
Senior Biomedical Scientists support this role and other BMS roles in the department.
There is a requirement for out of hours shift cover, including lates, nights and weekend shifts. This is distributed to all staff via a rota.

Job summary
To support the Facilities (Hotel Services) team in delivering catering services to patients, staff and visitors in accordance with the Health Board Food Safety policy and all related and relevant policies, procedures and legislative requirements.
To assist in developing, maintaining and promoting the highest standards of service delivery, supporting, team working practices and the Facilities Unit commitment to EFQMS; and helping to maintain open and effective communications.
The post holder will work either as part of the ward based catering team or as part of the Main Kitchen catering team and will be responsible for food preparation or regeneration, meal service and cleaning of all catering areas.
Main duties of the job
- To prepare food and beverages in accordance with Health Board policy, procedures and legislative requirements;
- To assist nursing staff with meal service at ward level;
- To serve beverages at ward level in accordance with patient requests;
- To serve staff and customers in the dining room providing a professional and polite service;
- To operate the cash register accurately;
- To ensure all catering areas are cleaned in accordance with daily and weekly cleaning schedules.
- In the event of a major incident or to support adverse weather or other Facilities services contingency, the post holder may be required to be called in outside of normal working hours to support the Facilities Services in the contingency actions when and where required.
The duties of this post are not inflexible or exhaustive and will be reviewed periodically and may be changed to be consistent with the grading of the post as the organisation or department develops and following consultation with the post holder.
Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn't mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates.
