Job summary
The post holderprovides specialist social work support to older adults with functional mental health needs admitted to MHSOP inpatient wards. As a key member of the MDT, the post holder leads on social care assessment, care planning and discharge coordination, ensuring safe, timely and person-centred transitions from hospital to community settings.
Using a strengths-based and recovery-focused approach, the post holder promotes independence, wellbeing, choice, dignity and least restrictive practice while safeguarding vulnerable adults. Acting as a key link between inpatient and community services, they work collaboratively with service users, carers and partner agencies to coordinate discharge arrangements, overcome barriers to discharge and secure sustainable outcomes.
The role includes care coordination, risk management, advocacy, safeguarding, professional consultation and maintaining accurate records. Through effective partnership working and social work leadership, the post holder contributes to reducing delayed discharges and improving recovery-focused outcomes for older adults.
Main duties of the job
Provide advice and advocacy to service users, carers and families. Lead discharge planning from admission, coordinating safe, timely and discharges by identifying and addressing barriers and arranging appropriate support, including domiciliary care, residential and nursing placements, supported living, reablement, equipment and Section 117 aftercare.
Undertake Carer Assessments, Care Act Assessments, Mental Capacity Assessments and contribute to Best Interests decision-making. Promote safeguarding practice, undertake safeguarding enquiries where required, and contribute to risk assessment and management that balances protection, independence and least restrictive practice.
Provide professional advice to multidisciplinary colleagues on social care legislation, safeguarding, NHS Continuing Healthcare, ordinary residence and charging responsibilities. Represent social work within MDT meetings, ward rounds and discharge forums, advocating for the rights, wishes and wellbeing of service users and carers.
Maintain accurate records and statutory documentation, contribute to quality improvement, audits and integrated working, participate in supervision and continuing professional development, support students and newly qualified staff and comply with Trust policies, procedures and professional standards.

Trauma & Orthopaedics Senior Trust Doctor (ST3+ equivalent)12 months fixed term contract,
Applications are invited for a Clinical Orthopaedic Fellow (at registrar level) this is an exciting opportunity to gain comprehensive experience and training in Trauma & Orthopaedics at University College Hospital.
As each of our consultants has their specific sub-specialty interest, the candidate may be able to preference the area of their sub-specialty interest to match that of their consultant supervisor (depending on availability), giving the fellow exposure to clinics and elective operating in that particular area.
The Fellow will be working alongside a team of sixteen consultant surgeons, who collaborate closely with musculoskeletal radiologists, anaesthetists, physiotherapists, and occupational therapists.
The candidate must be post MRCS, have at least 36 months Orthopaedic experience and have obtained full registration with the General Medical Council of the United Kingdom.
Main duties of the job
Your job plan is anticipated to contain 9-10 clinical sessions per week on average to be carried out on UCLH sites.
UCH Fellowships are clinically based specialist posts intended to give candidates hands on experience and a wide exposure to current techniques in trauma and orthopaedic surgery. UCLH has a busy trauma operative workload is delivered on all day Consultant supervised trauma lists during the week, with excellent experience to be gained in routine and complex cases. You will also have the opportunity to gain exposure to our highly specialist elective workload delivered by consultants who are experts in their given specialist areas.

We are looking for an experienced and motivatedElective Care Training and Validation Manager to support the delivery of high-quality elective access services across the Trust.
This is a key role within the Elective Access Team, supporting the delivery of national 18-week Referral to Treatment standards, improving waiting list management, and strengthening data quality across elective pathways. The post holder will work closely with operational, clinical, administrative, information and training teams to ensure patient pathways are managed safely, consistently and effectively.

Main duties of the job

Job summary
Help shape the future of lower limb arthroplasty services in South West Wales as a Consultant in Trauma & Orthopaedics with a specialist interest in complex primary and revision surgery.
The successful candidate will join a supportive department delivering both elective and emergency care across Hywel Dda University Health Board. This role offers the opportunity to contribute to the continued development of our arthroplasty service, with a focus on delivering high-quality outcomes, patient safety and evidence-based care.
We are looking for a clinician with advanced surgical expertise in lower limb arthroplasty and experience in managing complex and high-risk cases, including revision surgery. You will play a key role in service development, work closely with multidisciplinary colleagues, and contribute to training, quality improvement and governance activities.
Based at Glangwili General Hospital, the post provides opportunities to help shape specialist arthroplasty pathways in line with Health Board and regional priorities, supporting the delivery of high-quality care for patients across South West Wales.
Main duties of the job
- Providing expert clinical care in Trauma & Orthopaedics, including the assessment, diagnosis, and surgical management of patients requiring complex primary & revision lower limb arthroplasty procedures.
- Contributing to the delivery and ongoing development of a high-quality, sustainable arthroplasty service aligned to Health Board objectives and regional orthopaedic transformation programmes.Sharing responsibility for the acute management of trauma patients, including participation in a 1 in 12 on-call rota, ensuring safe, timely and consultant-led decision making for emergency admissions.

Main duties of the job

As a SEND Assistant Practitioner / Nurse Associate, you will be working as part of pro-active skill mix team who are committed to delivering high quality services.
The successful candidate will work 3 days per week during term time only. The hours of work are 8.30am - 4.30pm, Monday - Friday.
The Children's service provides support to children, young people and their families within a progressive universal model of service delivery dependent on assessed need.
The service offers every child/family a programme of screening, developmental reviews and information and guidance to support parenting and healthy choices as defined by current School Health drivers.
The post holder will work as part of a team to promote and maintain the health and wellbeing of children and young people with mild, moderate and severe learning difficulties which could also include thosewith complex medical needs. This post will include providing clinical care within the school environment.
Main duties of the job
Responsibilities will include caring for children with urinary catheters, enteral feeds, and medication requirements along with an awareness of tracheostomy and stoma care within the school and community settings. Responding to emergency situations and contributing towards specialist individual care plans to meet the child/young person's needs alongside staff training is an important aspect of the role.
Public Health
The post holder will contribute towards undertaking holistic assessments of children, young people and families in planning and delivering the Healthy Child Programme, including health screening and immunisations, plus a requirement to safeguard and support the health needs of any vulnerable children/young people within the caseload across the city.

Job summary
This is a fantastic opportunity for a Data Manager / Research Assistant Psychologist for a Wellcome funded research study, SloMo2. SloMo is a digitally supported, cognitive-behavioural therapy for psychosis that improves fear of harm from others (or paranoia). The SloMo2 study is investigating if SloMo therapy can be delivered by real world clinicians, by conducting a clinical investigation of its implementation, effectiveness, and cost-effectiveness in three NHS Trusts.
The postholder will join a friendly study team that is led Dr Amy Hardy. The post will be supervised by Dr Katie Taylor-Ryan, Study Coordinator and Dr Sarah Swan, Therapy Lead. The postholder will work alongside a lived experience researcher, and liaise with routine care psychological practitioners to facilitate data collection, promote uptake, and for provision of technical support.
Main duties of the job
- To provide data management and reporting support for the SloMo2 study, ensuring data integrity and completeness.
- To support research delivery in line with Good Clinical Practice through quantitative and qualitative data collection, safety monitoring, and Patient and Public Involvement.
- To support SloMo as a therapy service through provision of technical support, liaison with therapists, and assistance to the Therapy Lead.
- To contribute to team functioning and project management through effective communication, organisation, and use of supervision.

The Havens is recruiting a full time Band 2 Receptionist to join our dedicated team.
The Havens are London's Sexual Assault Referral Centre (SARC) located in Camberwell, Paddington (Ladbroke Grove) and Whitechapel, providing 24/7 (365 per year) forensic medical examinations and care to people disclosing rape or sexual assault.
This post will be predominately based at our Haven Paddington and Haven Whitechapel sites although regular cross site working is expected. Working pattern is Monday to Friday (excluding bank holidays) 9am to 5pm shifts, with a 12 noon to 8pm shift (maximum once per week, typically once every two weeks -at the time of publication, awaiting an implementation date)
The post holder will work as part of team in facilitating a high quality customer-focussed reception service including: taking and transferring calls (including client calls), registering clients, booking in clients on arrival, directing clients to the appropriate staff and preparing and administering clinical proformas and client files. The post-holder will be responsible for entering highly sensitive information onto the Havens systems. This involves sexual assault information which may be used in criminal investigations. The data entry needs to be accurate so attention to detail is crucial.

Job summary
This is an exciting role that provides essential administrative support to our Inpatient Unit. As the first point of contact for patients, families, and visitors arriving at Summerlee Ward, you will play a vital role in creating a welcoming and supportive environment.
Our service operates between 8am and 8pm Monday to Sunday, we are looking for someone to cover Wednesday 12 -- 8pm and Saturday 8 -- 8pm. There will be overtime available to cover sickness and leave across both Summerlee and Spinnaker Wards.
While your primary focus will be ward based reception and administration, there may be occasions where you are required to support the Community and Responsive Services (CRS) Administration Team, depending on service priorities. This offers a valuable opportunity to broaden your skills and experience across different areas of the service.
Please note this post is part time 19 hours per week.

Main duties of the job
*Welcome patients and visitors to the ward, ensuring they are directed appropriately.
*Answer telephone calls and relay messages to relevant staff promptly.
*Maintain patient records and update systems such as RiO or SystmOne as required.
*File, photocopy, scan, and distribute documents as needed.
*Manage incoming and outgoing mail.
*Support the ordering of ward supplies using Trust-approved systems (e.g., SBS, Clare's).
*Assist with booking transport, appointments, and other patient-related logistics.
*Maintain confidentiality and adhere to data protection policies.
*Work collaboratively with the multidisciplinary team to support ward operations.
*Ensure the ward environment is tidy and stocked with necessary materials.

Job summary
The Acute Elderly Unit Team in BHRUT are actively recruiting for a motivated and dynamic Band 7 Occupational Therapist to join our team at Queen's Hospital in Romford, Essex.
The team covers 3 Acute Elderly wards and are part of the Frailty service provision. Working in a fast-paced environment, you will have the opportunity to expand your clinical knowledge and be expected to supervise and educate more junior staff. The successful candidate will be required to demonstrate strong leadership skills and be confident to work as an autonomous practitioner with a focus on complex assessment and discharge planning with robust decision-making essential for efficient patient flow. You will be familiar the assessment of patients with diverse presentations and complex physical, cognitive and psychological health conditions within elderly and medical specialities to develop and deliver treatment and discharge plans. The team is an integrated service with Occupational therapy, with staff working to develop an enhanced therapy role. You will be supported in your role directly by the Clinical Specialist OT. Peer support will be provided by the existing Band 7 Physio's and OTs and plenty of CPD opportunities are available.

Main duties of the job
To provide specialist standard of Occupational Therapy service to patients within the acute elderly units with a special focus on the assessment of the acute elderly patients, facilitating and maximising patient potential within the service boundaries, using a holistic approach.
To give highly specialist guidance, advice and education to other members of the Multidisciplinary Team (MDT) on matters relating to your specialist area.

Single Point of Access (SPA) is a service that is vital to the delivery of adult community health care for housebound patients across South and West Hertfordshire.
In this role, you will:
- Manage electronic referrals from a wide range of healthcare professionals
- Handle patient telephone enquiries across multiple services
- Use an advanced electronic patient record system to support efficient service delivery

Location: BRE, Bucknalls Lane, Watford, WD25 9NH
Working Pattern: This is a 7-day service operating from 8:00am to 8:00pm. Shifts are scheduled across these hours, including weekends and Bank Holidays, as the service runs 365 days a year.

Main duties of the job
Flexible working is an essential part of this role, including providing cover for central booking and reception services in line with service needs. The post holder will also be expected to work evenings where required.
The successful candidate will maintain strict confidentiality at all times, recognising the sensitive nature of the service. They will use their initiative to manage day-to-day tasks effectively and respond to routine queries and challenges.
Key responsibilities include:
Process new referrals in line with local Standard Operating Protocols (SOPs)
Book clinics and appointments, managing cancellations and amendments promptly
Maintain health records as required in accordance with local SOPs
Accurately enter and retrieve patient information using electronic patient record systems
Provide accurate confirmation of appointments to patients
Maintain the integrity of patient data on electronic patient records, including the creation and update of records
Work closely with relevant departments to ensure that notes are prepared and available and that necessary clinical information has been obtained in time for clinics, where appropriate
Work with telephonic equipment and computer software used to support the Single Point of Access service

Job summary
We are looking for an enthusiastic and dedicated Salaried GP for our two partners and three salaried GP practice in Brentford. We are looking for someone to join our team for 4 sessions, Tuesday all day and Thursday all day. We run a total triage appointment system.
Currently, our practice is fully staffed by a friendly and caring team who work together to provide excellent patient care. We have a team of skilled professionals consisting of 5 GPs, 1 ANP, 1 nurse, 2 HCAs, 1 pharmacist, and a pharmacist technician.
Our list size is about 8100 with an interesting mixed demographic. We are active within our PCN and our PCN staff consists of pharmacists, first contact physio, a social prescriber, mental health advisor, a paramedic and a diabetes specialist.
We are rated Good by CQC and we use SystmOne. We value best practice, support one another, and focus on professional development and flexibility for our staff. We have several meetings including clinical and practice meetings. We encourage progress in leadership and special interests.
Informal visits or discussions with GP partners are welcome.
Main duties of the job
The appointed Salaried GP will provide medical services for the practice population to a high standard, including day-to-day activities such as Online triage and advice, face-to-face consultations, telephone consultations, Home visits, checking and signing off electronic prescriptions, processing test results, scanning and appropriate administration to ensure accurate records, patient continuity and quality care.

We are within a purpose-built health centre with good facilities, including spacious and air-conditioned clinical rooms, a free staff car park and bicycle parking. The surgery is located opposite Brentford train station, near Kew and Brentford High Street. - Number of patient contacts mix of f2f/tel A minimum of 15 booked appointments per clinical session but more if demand requires.
- Home visits A proportion of visits up to a maximum of 2 visits per working day, unless undertaking a triage day.

- On Wednesday per month afternoon PLT sessions when the practice is closed, if you are not required to attend, then this session would be owed back to the practice, using any holiday/ bank holiday allowance upon agreement between yourself and the practice (dependent on working days)
Clinical responsibilities:
In accordance with the above timetable, you will undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and electronically signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
In consultation with patients and in line with current practice disease management protocols, developing care plans for health
Providing counselling and health education
Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to agreed standards
Collecting data for audit purposes
Compiling and issuing electronic acute and repeat prescriptions
Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

Completion of cremation certificates and medical reports (NHS and private) when required (fees to be retained by practice (unless prior agreement with GP partners and completed outside of normal working hours/sessions)

Job summary
We are looking for a Care Navigator to join our friendly practice on a part-time basis.
Hours: 19 per week
- Monday 08.00 - 14.00
- Wednesday 14.00 - 17.00
- Thursday 14.00 - 18.45
- Friday 13.30 - 18.45
We're open to patients from 08.00 to 18.30, Monday to Friday, and shifts are scheduled within these times. From time to time, you'll also be required to work a late shift finishing at 8pm.
The hours listed in this advert are the exact hours we need to cover, so please only apply if you can commit to these as stated. We cannot offer flexibility around start or finish times.
Main duties of the job
You will be the first point of contact for patients and other contacts, you will help patients navigate our patient triage system, book appointments, deal with general enquiries, and support the day to day running of the team.
