Job summary
An exciting opportunity has arisen for a Band 7 Registered Nurse in Learning Disability (RNLD) to join the established CALDS (Child and Adolescent Learning Disability Service) which is a community based multi-disciplinary team within the Children, Young People (CYP) and Family Health Service. We are seeking a dynamic, compassionate, and experienced clinician to join our service in a Team Lead role for our tertiary response pathway (called Cyfuno). This is a key leadership role, providing clinical, operational, and strategic oversight to a specialist multidisciplinary team. You will play a pivotal role in ensuring that vulnerable children and young people receive timely, coordinated, and effective support during periods of crisis.
Main duties of the job
We are looking for a forward thinking RNLD who has in-depth knowledge of factors effecting the health and wellbeing of children with LD, reasonable adjustments that may be required, behaviours that are challenging and complex needs, along with an interest in supporting the development and improvement of the service. The post holder will have the ability to support CYP and their families; delivering good quality, individualised and evidenced based care. The post holder will have the skills to work in a team and as an individual, be confident to work within their competencies and be able to adapt to the changing needs of the service. Roles will include leading on risk assessment, decision-making and crisis planning, liaising with Safeguarding and Risk & Governance Leads, chairing and contributing to multi-agency meetings for complex and high-risk children, driving quality improvement, audit, and governance processes, working with professional leads to manage workload, co-developing service delivery and outcome measures, and fostering strong inter-agency relationships to support integrated care. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job
This role will work closely with other PCN roles: Pharmacists, Physiotherapists, Dietician and Social Prescriber in addition to the core practice teams.
You will assess, investigate, manage foot conditions and refer patients who have been directed to your service by a primary healthcare worker or directly by reception where appropriate. You will be an experienced practitioner who is comfortable working independently as well as part of a wider multidisciplinary team. You will be supported by the clinical team at each GP surgery and work with the wider clinical team to deliver holistic care to our patients. You will be able to assess and assist in adapting the service in response to local population needs

This is an opportunity for a Band 6 role on ward 203/Royal Derby Spinal Centre within Trauma and Orthopaedics.
We are looking for a highly motivated individual to join our management team, who possess excellent communication and leadership skills, and promote excellent quality of care, whilst supporting junior staff on the ward to do the same. The role will entail working weekends, bank holidays and nights to provide unit bleep cover.

You will have the full support of the management team and your peers, enabling you to succeed in this role.
Main duties of the job
The majority of your role will be clinical, and you will be a clinical lead on the ward supporting and mentoring junior members of the team with your experience and knowledge.
The job will also entail all aspects of people/staff management, for example, ensuring staff wellbeing, completing appraisals, dealing with conflict and more.
You will be expected to complete, audits, rosters, reports, attend meetings and liaise with all members of the MDT as well as many other tasks.

Main duties of the job
You will provide a support, signposting, and ongoing referral service to patients with low level mental health issues. You will do this through referrals from GP Practices and other PCN services. You will be the first point of contact for people accessing mental health services through their GP and help them navigate and understand their care pathway. You will do so through active listening, guidance, explaining onward referral routes and encouraging participation in the community through signposting to local mental health services. You will refer onwards to more specialised services and work with these services to support patients to navigate their care and promote autonomy, self-help and provide resources and action planning to improve outcomes for patients who are struggling with low level mental health issues.

Job summary
We are seeking a Pharmacy Dispensing Assistant to join our friendly Pharmacy team at Watford General Hospital.
You will assist, under supervision, with routine duties of a technical nature, including the dispensing and supply of medication to in and outpatients, the receipt, processing, storage and stock control of medicines.
You will respond to enquiries and requests from staff, patients and carers as appropriate.

Job summary
Working within Surrey and Sussex Integrated Care Board Primary Care Training Hub (primarily for Sussex), the post-holder will act as the point of contact for specific activities and will be responsible for ensuring operational delivery of NHS England Workforce Training and Education requirements and associated primary care workforce programmes and projects across the health and care system.

They will make use of workforce and project data available to understand the needs of the workforce now and in the future to embed recruitment, development and retention strategies that relates to the respective integrated care system (ICS) and Training Hub population health needs.
Main duties of the job
The post-holder will work closely with a range of local stakeholders to support developing the Training Hub agenda within their governance arrangements, enabling existing, and new, workstreams as required by the Surrey & Sussex PCTH programme and activities specific to individual workforce roles.

An exciting opportunity has arisen for a Community Mental Health Nurse to join our Memory Team in Redcar, based at Reed Marsh House. Main duties of the job
The ideal candidate will be highly motivated and be able to demonstrate best practice in community mental health service delivery for older people, their families and carers. The ability to work independently, effectively and collaboratively within Multidisciplinary Teams, with external partners, and most importantly - with people, their families and unpaid or paid carers, is an essential core competency.
In the role you will be responsible for making the initial contact to support service users and their families and carers in their understanding of the memory pathway. You will undertake a detailed history using a holistic memory assessment framework, with the initial collation of all clinically relevant individual service user information (clinical history) and involving families and carers as part of our genuine commitment to co-creation. In this way you will promote positive experience and clinical outcomes from the beginning of the person's service journey.
There is a requirement to accurately record and document clinical discussions and assessment details on our clinical system and to ensure that all individual service user documentation is up to date and available to inform internal clinical meetings and aid consistent, safe and high quality decision making by members of the MDT and wider colleagues.

The Royal London Hospital is one of the UK's busiest and oldest teaching hospitals, operating in purpose built facilities close to the historic site in Whitechapel, East London. As well as Europe's largest Neuro-Trauma service, the Department of Neurosurgery provides a full range of sub-specialties, including Neuro-Oncology, Complex Spine, Neurovascular, Functional, Neuro-Modulation for Pain, Complex Hydrocephalus and Craniofacial Reconstruction. The Neurosurgeons are supported by a comprehensive Clinical Neurophysiology department (including intraoperative monitoring), dedicated Consultant Neuro-Radiologists, Neuro-intensive care (including a Neuro-Monitored unit within the department), Neurologists and a Hyper-Acute Thrombectomy Service.
Main duties of the job
The post holder will work within the firm of ST1-2. The successful candidate will work within a team providing neurosurgical care for neurosurgery in patients and conduct the daily ward round with them. Additional time within the weekly rota will be made available for attending theatre lists and outpatient clinics, depending upon the training requirements of the individual candidate.
The post holder will participate in the ST 1-2 (SHO) on-call rota which is currently as a 1 in 11 full-shift on call which is fully compliant with EWTD.

Main duties of the job
The post is locality team based in Amersham and the Chiltern South Bucks area. If you are a newly qualified therapist you would be given a mainstream school aged caseload for 4 days of your week, alongside 1 day CPD to develop competencies and shadowing for the first year with the aim to develop interests in different parts of the service. Caseload allocations are caped to allow NQTs the ability to acquire necessary competencies within the first year.
This is a jointly commissioned service that will give you the opportunity to use your innovative and dynamic SLT skills to influence the delivery of high quality, evidence based interventions for children and young people in Buckinghamshire.
We use a wide variety of evidence-based therapy approaches with children. Therapy is delivered individually, in groups or flexibly as the child and setting require.
Award winning and evidence based programmes by Speech and Language Link, are commissioned to every mainstream school setting across Buckinghamshire and supports a graduated approach to therapy delivery alongside our school advice sessions and webinars.

Job summary
An opportunity has arisen for a Senor Mental Health Nurse to join our multi disciplinary team.
The Crisis Response Team provide interventions to young people at the point of mental health crisis.
The team provide a 24/7 service which allows for flexibility from a 9-5 working pattern as a Senior Mental Health Practitioner, the post holder will provide high standards of evidence based client centred care to children and young people with mental health [CYPMH] problems within the THRIVE Framework for service delivery.

The role of the call handler/booking co-ordinator is a pivotal post in the Trust Access Centre.

The post holder is required to provide an efficient and effective support service to the residents in staff residential accommodation on the Ysbyty Gwynedd Hospital site, including accommodation booking, security of property, key holding, and record keeping.

Alrewas Surgery has a fantastic opportunity for a salaried GP 6x sessions per week.
A long-standing, well respected GP practice covering a semi-rural area between Lichfield and Burton on Trent, with easy vehicle access as we are situated just off the A38 close to the National Memorial Arboretum, Alrewas Surgery aims to offer a welcoming, high quality GP service to all its patients.
Main duties of the job
Working 6x sessions per week, you will be responsible for assessing and treating patients and undertaking all clinical duties associated with GP work in line with GMC and RCGP standards and working with our multi-disciplinary team.
As a Salaried GP, the successful candidate will be responsible for managing their own caseload, dealing with a wide variety of health needs in a primary care setting. They will work both independently and collaboratively as part of our team to deliver a high standard of care for all registered patients, while carrying out the associated clinical and administrative duties.
Patient care will be delivered face-to-face, over the telephone, and through our online dashboard system. The core surgery hours are 8:00am to 6:30pm.
You will also hold Duty Dr on-call responsibilities once per week, undertaking triage and same day needs assessment and response.
You will receive a competitive salary dependent on experience, 30 days annual leave pro rata and an NHS pension.

Job summary
We have an exciting opportunity for a Band 5 development post within the Sandwell Sexual Health services. The successful candidate will be given the opportunity to undergo training and development to become a Band 6 senior Contraception and Sexual Health Nurse.
We are looking to recruit an enthusiastic passionate nurse who wants to develop professionally to work within a motivated team of professionals and deliver education, sexual health advice and management.
We are a multi-professional progressive team providing a comprehensive, evidence based and patient focused service for Contraception and Genitourinary Medicine specialising in integrated sexual health and HIV services. They will be working at the main hub at the Lyng Centre and our outreach services across Sandwell.
The post holder will work alongside consultants and senior specialist nurses to gain appropriate training and skills in order to provide clinical care for presenting groups of patients including history taking, clinical assessment and examination, safeguarding, diagnosis, treatment and evaluation of their care.
The post holders must be motivated to provide all aspects of sexual health and contraception to vulnerable and hard to reach groups who would otherwise not engage in Sexual health services
Main duties of the job
-To assess, plan, implement and evaluate individual care based on consultation, assessment and individual needs
-Undertake some higher-level clinical duties under the direction/supervision of a senior nurse/practitioner
-Collect specimens as directed by senior clinical nurse or doctor, ensuring its safe storage/transport or disposal as appropriate. Ensure appropriate labelling and recording of specimens and request forms.
-Collect blood samples (peripheral or venous) ensuring its safe storage, transport or disposal, as appropriate.
-Manage a defined group of patients/clients, participating in all aspects of care from initial contact through to discharge, including transferring from one environment to another if applicable
-To undertake administration of prescribed medication

We are looking for an enthusiastic and forward thinking individual with appropriate exposure and experience, particularly in High risk and Vascular Podiatry. You should champion quality care,to join our specialist team to help innovate the High risk and Vascular Foot care agenda.
As a practicing Clinician you will perform specialist podiatry assessments of patients with diverse presentations and complex physical and psychological conditions, to provide a diagnosis and to develop and deliver a specialised individual treatment program across various settings, including the domiciliary caseload to benefit the patients needs.
Main duties of the job
- Management of the high risk and vascular caseloads with the lead as well as the wider team
- Coordination of the acute and community integrated vascular and high risk podiatric pathway in a safe and effective manner
- Working with the wider directorate team, including the diabetes and vascular leads
- Supporting and implementing the quality agenda in all aspects of work
- Regular evaluation and development of the high risk and vascular podiatry service
- Clinical supervision of staff and students, taking part in peer reviews and training sessions
- Working alongside the lead for advice and support when required
- Routine evaluations of clinical work and documentation
- Participating in local and national audits and service improvement activities to demonstrate adherence
- Management of a domiciliary caseload - a driver with daily access to a car is essential
