Job summary

The Nurse will assist the Manager and fellow staff in the provision and maintenance of a seamless service & highest quality care to patients in accordance with Clinical Governance.
To assist senior nursing staff in the assessment, planning, delivery and evaluation of paediatric care for the children and young people who attend the Paediatric Assessment Unit (PAU) from their initial attendance through to discharge or admission to the inpatient ward.
To contribute to the reduction of healthcare acquired infection by ensuring a high standard of infection control practice in the light of research current best evidence and legislation.
The Nurse will collaboratively function as part of the multi-disciplinary team within the Paediatric Department working within the Trust's policies and procedures.
The Nurse is expected to work with and support the Paediatric Directorate as a whole, inclusive of Paediatric Outpatient Department, Neonatal Intensive Care Unit (NICU), Neptune Children's Ward and the Paediatric Emergency Department.
It is expected that the Nurse as a reflective practitioner will continue to develop their own capabilities and Nursing knowledge in line with the Trusts career pathway where education is supported.
This job description is not meant to be exhaustive and will be reviewed as part of appraisal or when necessary. The post holder may be required to carry out other relevant duties as required.
Main duties of the job

To establish effective communication with children/young people, parents and siblings who attend the Paediatric Assessment Unit (PAU)
Liaise and communicate with all members of the multidisciplinary team on matters affecting patient care.
To act as an advocate for the child/young person in all discussions relating to Paediatric services
To answer telephone calls in a professional manner as per Trust guidance, taking full information and caller details to ensure accurate escalation to the appropriate person
To act, at all times, in a courteous, dignified and respectful manner when dealing with patients, staff and visitors to the Trust
To ensure the experiences and views of child/young person and parents/carers are listened to.
Ensure that there is adequate provision of written / verbal information for patients to support their safe discharge from the PAU department

To communicate effectively any patient care to others, using the SBAR model
- Analytical and Planning
Prioritise care delivery within the PAU setting, acknowledging the impact of the decision on other departments and ensuring patient flow is maintained
To undertake and record patient observations using the Patient Early Warning Score (PEWS) and escalate any concerns in a timely manner
To identify and make direct referrals to other health professionals and agencies according to individual patient needs.
Having knowledge of, and act within current Mid and South Essex NHS Foundation Trust Policies and Guidelines of Clinical Practice
Be aware of personal responsibilities regarding risk management procedures e.g. fire, health and safety.
Promote a positive departmental image by showing a professional and helpful approach to all visitors to the ward
To participate in the process of complaint management.
Be familiar with the steps taken when a medical or untoward incidents arise, and escalate and act accordingly
To be aware of current developments required by professional bodies.
To ensure continuous, professional development by the attendance at study sessions/days, courses and development of skills. To be able to provide evidence of this by the maintenance of a professional portfolio
- Clinical Duties

To co-ordinate and liaise with multidisciplinary agencies to give care to patients and their families with specific needs
Learn and perform new skills in relation to the Paediatric Assessment Unit specialty
To act as a supervisor and/or an assessor, preceptor, supporting student nurses, nurses, medical students and Return to Practice Nurses, during their training
- Policy and Service Development
To measure the effect of nursing care by participating in the quality assurance cycle, including the development of practice guidelines, protocols, policies and clinical audit.
Maintain an interest in current research issues and using critical analysis, assess suitability for application of these and new ideas within the Paediatric Assessment Unit.
Positively contribute to the process of change within the PAU.
To contribute to meeting high standards of care, including the reporting of untoward incidents and ensuring compliance with the Risk Management Strategy
In conjunction with senior members of the team, ensure the appropriate collection, evaluation and dissemination of audit data, and ensure the data quality meets Trust requirements.

The PCN Transformation & Development Manager will lead, coordinate, and drive delivery of the PCN DES and wider transformation programmes across the Federations three Primary Care Networks. Extended Hope is a support service for young people in mental health crisis in Surrey. It is based in a small children's home in Redhill but covers the entire County. This home provides a short-break service for up to two children who experience social, emotional and mental health difficulties. It is operated by Surrey County Council and the local NHS trust. Report to the Manager of Extended Hope and provide leadership in the development and delivery of service.
To provide timely assessment of young people who present in mental health and social care crisis within the acute hospitals, Health Based Place of Safety (HBPoS) or other community spaces across surrey. Young people may present with deliberate self-harm behaviour, suicidal ideas, suicide attempts, family breakdowns, complex family dynamics, breakdown of adoption, foster and children home placements. Assessments of the above presentations with appropriate signposting, care planning, safety planning, risk formulation/management & community planning applied.
To work collaboratively with the Crisis pathway provision within CYPS and Social care and offer community-based assessments with emergency services when there could be a possibility of supporting young people in their communities as an alternative to being placed on a section of the Mental Health Act (MHA) or removed to a place of safety.
To provide support, advice and education to the Emergency Departments, Paediatric Wards or HBPoS to build knowledge and skills of staff within these settings in the care and treatment of young people. Our hospital colleagues will be involved in care provision and planning throughout any Paediatric or HBPoS inpatient journey.
To work closely with the CYPS Crisis Pathway to provide a streamlined response to meet the needs of young people and their family/carers. This includes the Crisis Intervention Team, Paediatric Liaison Service, Hope Service/Hope Day Programme, Adolescent Team, Childrens Crisis Service (CCS provision SCC) Youth Workers and the Emergency Duty Team (EDT).
To be part of the Mindworks provision within Surrey aiming to deliver a Thrive approach to care and treatment. This is a partnership between CYPS (formally known as CAMHS) and the Alliance partners.
Key Responsibilities
To work closely with Mindwork services during office hours, responding to emergency referrals of young people presenting in crisis.
Work as an autonomous practitioner, undertaking clinical assessment of young people who may have complex co-morbid mental health and/or social care needs and present in escalating crisis. You will be working alongside medical, nursing, social care and other professional colleagues to ensure that the holistic approach to meeting the needs of the young person and family/carer is considered. As a practitioner at the Extended Hope Service you will consider how your service can complement the short-term plan that can allow the least restrictive options for the young person at that time.
To co-ordinate and liaise with ED, Paediatric wards, PLNs, or HBPoS when young people with mental health and/or social care needs attend Acute General Hospitals or are admitted to the Paediatric wards or HBPoS. As this role in out of hours you will have access to senior colleagues should support regarding decision making be required.
Work closely with the Emergency Departments, CYPS Crisis Line, Paediatric Triage Line, the Adult and CYPS Psychiatric Liaison Services, Mental Health Crisis Teams, On Call Duty Psychiatrist, On Call Managers, ensuring young people are seen in a timely manner and risk is assessed and a management plan is developed.
To work alongside the Hope Community service to support young people who are required to stay (for emotional or psychological reasons) in acute care settings through in-reach visits and engagement insitu. This will be an adjunct to the work of that service and constitute an additional responsibility in alignment with that services Care Coordinators.
To support formal Mental Health Act assessments of young people when required, in order to guide care and treatment planning by AMHPs and s12 Drs.
To manage own caseload from referral to discharge/transfer, holding medium to high risk/needs cases.
Ensure that outcome measures for the service are implemented and audited. Contribute to relevant research and ensure that interventions on the pathway are evidence based.

There are 3 full-time posts with SABP bases across the county.
Main duties of the job
To manage a caseload of young people referred to the service, working collaboratively with other services both internal and external, to provide consultations and mental health assessments to assist young people overcome the obstacles they face and to achieve their potential. These young people will already be working with the Youth Justice Service.
To build a trusting relationship to develop a deeper understanding of the needs of young people and be able to signpost or refer to more appropriate services. This role will involve outreach into the community to meet young people in their own or choice of environment.
To work in partnership with young people, families/ carers and other services regarding decision making and planning to help them to make informed decisions, to enable them to clarify and express their needs and to co-produce solutions.

Job summary
This is an exciting opportunity for a motivated Support Worker to join a dynamic multidisciplinary community rehabilitation service in Southampton. The Community Therapy Service is integrated with Community Nursing and forms the East Locality, delivering programmes of rehabilitation working in partnership with other social and healthcare providers.
The role will allow individuals to gain experience in falls assessment, education and prevention, complex community rehabilitation and goal focused rehab planning. Additionally, the role will involve provision of equipment and minor adaptations to support independence, providing advice on health and wellbeing and signposting to other community services. The team promotes inter professional working which supports the single assessment process for patients. HIOW Healthcare is looking for an inspirational nurse to deliver a high standard of quality care to patients within the community setting. Community Nurses work autonomously and as part of a team and care is delivered in the patient's own home environment. Our nurses are responsible for a manageable caseload, working across the city ensuring our patients are receiving the best quality care from the comfort of their own homes and thereby avoiding hospital admission.
HIOW Healthcare Community Nurses work with adults who need support for a multitude of health conditions. These include:
Long term conditions management
Acute wounds and complex wound care
Palliative and end of life care
Facilitating discharge from hospital, including drain management, administration of prophylactic medication
Continence care and bowel care
Administration of a variety of medications
Although a lone worker, you will be supported by a team who are always available, to allow you to develop your existing skills into becoming an autonomous practitioner in the community. This is a part time position (27.5 hrs pw) working within a team based at Bitterne Health Centre, Southampton. Our working patterns are across 7 days. Our core shifts are 0800 to 1600 and 1400 to 2000 hrs. There is also a requirement that you will work 1 in 3 weekends as well as Bank Holidays.
Main duties of the job
A registered nurse working as part of a multi-disciplinary Locality Health Team affiliated to a cluster of GP Practices providing nursing care to the Practice population both in the patients home or clinic setting based on individual care needs.
Delivering on the organisational objectives to meet individual health care needs through high quality, safe and effective services to avoid unnecessary admissions to secondary care, support early discharge from hospital, effective case management for patients with complex long-term conditions or following an acute health episode and individualised care for the patient and carers at the end of life.

Working within the Quality and Safety Directorate, you will support the Team Manager, Claims in the effective management of Clinical Negligence and Personal Injury Claims. o Manage a caseload of Inquests, from commencement to conclusion, liaise directly with the Coroner's Office, support staff, draft and amend witness statements, action plans and work to strict deadlines.
o Prepare staff who are called to give evidence and inform and guide staff on Inquest process and to attend Inquests, and report back to senior staff the outcome of inquests and advise on remedial action if required.

Job summary
Southampton Early Intervention in Psychosis Service (EIP) is currently looking for a highly motivated and skilled professional to join the multi-disciplinary team in delivering individual and group interventions to people aged between 14 to 65 year olds who are presenting with first episode psychosis.
The successful candidate will have quality improvement skills and the vision to continue with the development of the service. They will champion a culture based on principles of recovery, choice and personalised care, providing a role model to junior members of the team. They will be compassionate and committed to providing patient centred and evidence based care and will have a awareness of current national drivers relating to working with individuals with a psychotic illness.
Experience of working as a CPN or in this field will be an advantage.
Car driver and use of car within work hours is essential.

Main duties of the job
To maintain a high standard of professional practice in the assessment of care needs, treatment and evaluation of programmes of care.
To undertake initial team assessments of referred clients and report to the referral agents after discussion/supervision from the appropriate Team Meeting. To also undertake joint assessments with colleagues from other disciplines and agencies as and when appropriate.
To complete an accurate assessment of risk with all new clients and provide the initial management plan. All risk management plans will be reviewed and updated as appropriate.
To provide Social Circumstances Reports for Mental Health Act Tribunals and attend as required by the Team Manager.
Where appropriate, to communicate and liaise with other agencies involved in providing health care and social care.
To promote and maintain a close working relationship with medical staff, other nurses and therapists.
To assist in establishing and maintaining lines of communication with wards, and hospital departments, G.Ps, voluntary agencies and other organisations.
To continue to support carers and relatives and engage in carer support and family work interventions.
To contribute to multi-professional assessment and treatment.
To liaise with ward staff/crisis team and provide care co-ordination to clients in hospital
To participate in meetings and to offer suggestions on the development of the Mental Health and Community Psychiatric Service.

Job summary
The post holder will join the RTT (Referral to Treatment) Validation Team within the Patient Administration Contact Centre, supporting a range of administrative and patient pathway activities.
We are looking for motivated, detail-oriented individuals with experience in RTT validation, patient pathway tracking, waiting list management and elective pathway standards.
The successful candidate will have a strong understanding of NHS RTT rules, consultant-led pathways, clock starts and stops, and validation processes. You will help ensure the accuracy of RTT data, identify and resolve pathway discrepancies, validate patient journeys and support the Trust's elective access standards.
Experience of PTLs, PAS systems, data quality processes and managing complex patient pathways is essential, alongside strong analytical, organisational and communication skills and the ability to work independently within a fast-paced environment.
You must already have the right to work in the United Kingdom or fully meet the eligibility criteria for sponsorship under the Skilled Worker visa route.
