Job summary
Join our innovative Community Ageing Well Service (CAWS) and make a meaningful difference to older people living with frailty, complex health needs and long-term conditions.
As a Specialist Occupational Therapist, you will work within a supportive multidisciplinary team to help people remain independent, safe and confident in their own homes. You will use your specialist skills to assess needs, provide person-centred occupational therapy interventions and help people achieve what matters most to them.
CAWS provides proactive, preventative and holistic care, working closely with GPs, community rehabilitation, social care, care homes, voluntary sector organisations and Integrated Neighbourhood Teams.

- Supporting admission avoidance
- Enabling people to remain safely at home

Main duties of the job
As a Specialist Occupational Therapist, you will:
- Complete comprehensive occupational therapy assessments for people with complex physical, cognitive and functional needs.
- Develop and deliver personalised treatment and rehabilitation plans based on individual goals and strengths.
- Assess and recommend appropriate equipment, adaptations and moving and handling solutions.
- Support people to maximise their independence, function, confidence and quality of life.
- Work collaboratively within a multidisciplinary team to develop coordinated, person-centred care plans.
- Provide specialist occupational therapy advice to patients, carers and other health and social care professionals.
- Contribute to falls prevention, frailty management and admission avoidance initiatives.
- Provide supervision, guidance and support to therapy assistants and junior colleagues, where appropriate.
- Contribute to service development, quality improvement and clinical governance.
- Build effective working relationships across health, social care and voluntary sector services to support joined-up care.

We are looking for a dedicated, compassionate, and enthusiastic Community Nurse Practitioner to become part of our supportive multidisciplinary team, working with adults experiencing eating disorders within our specialist Community Eating Disorder Service.
RESPONSIBLE FOR:
- Working in an identified adult community eating disorder pathway.Delivering a high-quality clinical service to patients and their families.
- Managing a caseload.
- Care coordination of an identified caseload with access to regular and robust supervision.
- Delivering therapeutic interventions, in accordance with an agreed care plan, to an identified group of people experiencing eating disorders.
- Working alongside the OTA and Peer Support Workers to develop and support therapeutic groups
Main duties of the job
Main duties and responsibilities
- 1. Take responsibility, with supervision for the management of a complex caseload, which includes, specialist assessment (within service specification), and care planning in conjunction with service user and carer, evaluation of individual treatment plans.2. Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.3. Assess patient's individual care needs, develop, implement and evaluate programmes of care.4. Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance. This will include physical health interventions/ monitoring and psychologically informed interventions. This may take place in clinical or community settings.5. Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.6. Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.

This is a unique opportunity for a motivated individual who has personal experience with the Criminal Justice System or prison custody.
The position is perfect for someone eager to use their Lived experience in prison and the Criminal Justice Service to assist others who have faced similar challenges. You will help them connect with support networks.
The outreach Service covers Shropshire, with its base being in Shrewsbury.
Training will be given and Experience of support work is desirable but not required as the role depends more upon your ability to use your personal experience to support others and your desire to do so whilst being part of a NHS team that is supportive, approachable and dedicated to the role.
The ability to commute independently across the Shropshire area is essential.
Peer support is based on the recognition that there is no better person to support the path towards recovery than someone who has walked the same path as that individual. We actively seek peers who have lived experience of both the Criminal Justice System who have a desire to use their own experiences to help others.

As a Peer Support Worker, you would be working across the area with people who have come into contact with Liaison & Diversion and then referred over to our Outreach service, building a good working relationship with them and helping them identify their needs alongside a senior Team Member. You will be then working with a caseload of these clients to support them in accessing health and support services for up to three months. You will be working as part of a larger team and under the supervision of Team Lead and Senior Peer Support Worker, you will also be working alongside our Navigators and Support Time Recovery Workers and participate in the client's journey from referral to discharge.

As our new Healthcare Assistant, your responsibilities will vary. Some of your duties will include:

Our community and outpatient services play a crucial role in providing specialised patient-centred rehabilitation focusing on Early Supported Discharge and longer-term recovery. We are based at the Haywood Hospital, with our community service working across 7-day shift pattern within patient home environments around Staffordshire and Stoke-on-Trent.

Main duties of the job
The Rehabilitation Assistant will work as part of a multi-disciplinary team and will have specialist experience, knowledge and skills to work within the community or an outpatient setting.

Perform at a high level of competence within agreed protocols under the supervision of a registered professional.
Take responsibility for their own workload within a broad framework overseen by registered staff to implement, deliver and evaluate specialist stroke rehabilitation programmes.

Job summary
An exciting opportunity has arisen on Moorgate ward at the Harbour for a fixed term / secondment for 9 months to the ward manager band 7 position. The secondment is ideal for those who already have established experience as a band 6 RMN and who wish to build on their development and experience, as well as being a fantastic opportunity for already established band 7 who want to experience a challenge on a busy male PICU.
The successful candidate will bring excellent role modelling skills, a passion for supporting and developing junior colleagues, including student nurses and they should have a keen interest in quality improvement projects that can lead to improvements in patient care and staff wellbeing.
You should bring with you experience of nursing people with acute mental health needs, have experience of supporting colleagues and students with development, and a "can-do" attitude.
Main duties of the job
The successful candidate will be expected to work across a shift pattern, being part of a team that provides 24/7 care and treatment to male service users on a busy, 8 bedded PICU ward at the Harbour. There will be occasions when you may need to support other wards, including our male and female adult and older adult wards at the Harbour.
Responsibilities will include supporting and developing learners in practice; shift leading, administering medication, shift work, engaging with patients to support recovery, including leading on dialog+ assessments; ensuring that the clinical environments meet high infection control and cleaning standards; representing the ward at meetings which may involve preparing and presenting reports; administering medication and monitoring for side effects; completing physical health checks on patients and escalating concerns as and where required; supporting colleagues with supervision and appraisal conversations; rostering and daily oversight of staffing levels; leading on MDT meetings; facilitating team meetings and away days; oversight of staff sickness and absence including forging close working relationships with teams such as HR.

The Countermeasures Development and Evaluation Centre is based at Porton and has approximately 250 staff, performing a range of research activities, including high containment level work. The department's work covers a broad range of projects involving vaccine and therapeutic, discovery, evaluation, development, and research.
The Discovery team sits in the Countermeasures Development, Evaluation and Preparedness Division in the Public Health Microbiology Directorate within the Chief Scientific Officer Group. Its work focuses on the establishment of host-relevant microbial models, evaluation of antimicrobial therapies directed at UKHSA priority pathogens, and development of molecular and immunological methods for characterising pathogen responses and host-pathogen interaction. This scientist post will contribute to the development of develop functional mucosal immune assays towards the establishment of in vitro assays that provide correlates of protection for TB vaccines.
Main duties of the job
- Work at CL-3, growing and manipulating Mycobacterium tuberculosis
- Carry out studies to develop functional mucosal immune assays using flow cytometry
- Analyse biological data and develop new tools to perform those analyses e.g., large flow cytometry data sets
- Contribute to internal and external reporting.
- Adhere to ISO9001 in both practical and computer analyses, and in the storage of data

Main duties of the job

The Macmillan Head and Neck Clinical Nurse Specialist (CNS) provides expert, patient-centred care for individuals diagnosed with Head and Neck and Thyroid cancers throughout the cancer pathway. The Good Neighbour Scheme is SCCCCs longest running project having started in 1966. The scheme offers a range of services to older people and their carers through volunteers who offer the kind of support a Good Neighbour might give. The service is provided free of charge with the basic aim of enhancing quality of life and helping to combat loneliness for people over 65 living in Sheffield. Over 190 active volunteers regularly visit or telephone an older person with the focus being on the social aspect putting the world to rights, usually with a cuppa!
We consider this to be an important and sensitive service as many of the people who are referred to the Good Neighbour Scheme have complex health needs and may be housebound and or socially isolated.
Main duties of the job
Coordinators accept requests for help from a wide range of referrers across health and social care including District Nurses, GPs, social work teams, Care Managers, Community Support Workers and various intermediate care teams. The service also takes self-referrals and from friends and family too.

Over the last few years there has been a steadily increasing demand for the service for a number of reasons
more older people stay in their own homes for longer or move into supported, independent living schemes - including those with Dementia
a more diverse range of professionals in health and social care are aware of and value the services we offer
the service is free of charge
We are now in a position where we need to expand the staff team and also look at other methods of volunteer support and coordination. This will allow us to improve our processes, allow the further expansion of our volunteer base and offer support to more people in need, whilst considering service sustainability.
Purpose of the Job
This post has 3 main strands
- to contribute to the effective development and delivery of the Good Neighbour Scheme
- to work with the Co-ordinators of our other services which offer a range of support to older people and their carers and ensure the scheme contributes to the overall aims of SCCCC.
- To build and support a network of Volunteer Peer Mentors who will support other volunteers during their Good Neighbours volunteering journey.
Duties and Responsibilities
- Work closely with other team members to take referrals from a range of referrers, including health and social care staff, friends and family members and the individual themselves, and allocate volunteers where appropriate.
- Be available for enquiries from volunteer, service users and their families and respond to any requests for information in a timely and professional manner.
- Ensure that each service user receives a home visit prior to allocation of a volunteer, in order to confirm personal details, undertake a risk assessment and better understand them and their needs.
- Provide support to service users waiting for a dedicated volunteer by offering support in person or over the telephone where appropriate
- Offer support and supervision to a team of Volunteer Peer Mentors and wider volunteer team and ensure that the service is responsive to the needs of its volunteers.
- Ensure compliance with SCCCCs policies and procedures.
- Liaise with the fundraiser in order to maximise donations to the organisation.
- Publicise the scheme through all appropriate channels.
- Liaise with the Operational Lead to participate in volunteer training as appropriate.
- Maintain accurate records of work undertaken and produce oral/written reports as required.
- Ensure that appropriate monitoring and evaluation procedures are put in place and used effectively.
- Cover the work of other Good Neighbour Scheme Team Members during periods of leave.(As appropriate)
- Attend the meetings of the organisation and any other meetings as required.
- Attend appropriate training courses to enhance/develop own skills.

Job summary
We are looking for a person with secretarial experience to join our Trauma and Orthopaedic Admin Team.
This is a full-time permanent post and an exciting opportunity for someone with already proven secretarial experience, given that Medical Secretaries in this department are senior staff within the Admin Team. However, those with appropriate experience in a senior administrative role would be considered.
The Trauma and Orthopaedic Department is fast paced, with over 40 Consultants running a busy Elective and Trauma Service for the local and surrounding areas.
The post holder must have excellent organisational and communication skills as liaising with patients, colleagues and other departments is an essential part of the role. Interested candidates should look to gain an understanding of the specialism prior to interview.

Main duties of the job
- To act as a point of contact to ensure all enquiries are effectively managed and successfully resolved within a prompt timescale, ensuring all relevant personnel are informed as necessary and an appropriate record is maintain
- To ensure, wherever possible, that all clinical documentation is up to date, in an orderly fashion and available whenever clinical decisions are being made, reporting exceptional circumstances where necessary. This will require effective collaboration with all relevant personnel to maintain an efficient administrative system: prompt transcription, manipulation and distribution of appropriate correspondence.

Main duties of the job
As a valued member of the Dietetic & SLT department, the post holder will be involved in a wide range of activities to support the patient journey. The post holder will adhere to the following:
Clinical
- To work autonomously to provide dietetic advice to adult and paediatric patients, their carers and staff with a range of medical conditions within in-patient, out-patient and community settings f within agreed protocols and guidelines and with support from senior colleagues when necessary including: Carry out nutritional assessments (interpretation of biochemistry, anthropometrics, clinical condition and diet history) to determine nutritional problem. Development of treatment plans (calculation of nutritional requirements, analysis of nutritional intakes sometimes using specialist IT systems, client centred goals and discharge planning including patients on artificial nutritional support) Prescription of dietary treatment and ACBS products for conditions where there are limited professional / clinical guidelines available Facilitation of group sessions to support patient knowledge and behaviour change.

You will work closely with TRFT staff to develop their knowledge of Mental Health- including involvement in delivering training and supporting TRFT staff to develop positive working practices which put the patient at the centre of their own care and promote parity of esteem.
Main duties of the job
You will be a knowledgeable and a competent practitioner with excellent assessment and risk assessment skills, the ability to work well under pressure and demonstrate the confidence and ability to work autonomously and as a member of the wider MDT. You will have experience of working with adults with complex mental health needs and have a willingness to continue your professional development.
The team works over a 24/7 period, and this involves a flexible duty rota covering this period. Full-time applications will only be considered.

The Integrated Transfer of Care (ITOC) Team is a dynamic and multi-skilled team consisting of nurses, occupational therapists, physiotherapists, and Assistant Practitioners. We also work in alignment with social services and other Locala community services. Our purpose is to support a safe and timely hospital discharge of people who are medically fit, and no longer need hospital care, to a place more suitable to the person's needs. Assessments and care provision can then be tailored to support people to regain strengths and skills so that they can live as independently as possible. We also aim to avoid admission for patients in A&E and Frailty departments who require intervention to allow them to be discharged home.
