Job summary
Swaffham and Downham Market PCN is seeking a motivated and enthusiastic Pharmacy Technician to join our friendly team. This is an exciting opportunity to play a key role in supporting safe, effective and high-quality patient care within Plowright Medical Centre
Working alongside their GPs, Clinical Pharmacist, Nursing Team and Administrative Staff, you will support and help deliver excellent patient care.
Main duties of the job
The successful candidate will:
- Support medicines optimisation and medication safety initiatives.
- Process medication requests and undertake medication reconciliation following hospital discharge.
- Assist with prescription queries from patients, community pharmacies and other healthcare professionals.
- Conduct medication reviews within your scope of practice.
- Support the management of repeat prescribing systems.
- Carry out audits relating to prescribing and medicines management.
- Support with QOF
- Maintain accurate clinical records using the practice clinical system.

Job Purpose
The Pharmacy Technician will work as part of the multidisciplinary team to support the safe, effective and efficient delivery of pharmaceutical services within the practice.
The role requires a proactive individual who can work independently while maintaining excellent communication with clinicians, patients and external healthcare providers.
Key Responsibilities
Medicines Management
- Process and manage repeat prescription requests in accordance with practice protocols.
- Undertake medication reconciliation following hospital discharge and outpatient appointments.
- Support medicines optimisation to improve prescribing quality and patient safety.
- Identify prescribing issues and refer complex clinical matters to the GP or Clinical Pharmacist.

The post will support the weekday running of the Acute Medicine Unit (AMU), Same Day Emergency Care (SDEC) and enable a senior acute physician weekday presence within the emergency department (ED), this will be in conjunction with the 10 existing acute physicians on a rotational basis. The successful candidate will join the team at an exciting time for the service having been rated as best in the country for overall satisfaction with doctor training in the 2026 GMC national training survey and advanced plans to introduce a Frailty SDEC and take, and to restructure the AMU including proposals for NHS England funded capital improvements.
Main duties of the job
The post holder will provide the full range of specialist diagnosis, treatment and care to patients with acute medical problems as outlined in the proposed programme. We are interested in appointing a motivated and highly competent doctor to these posts, as they will take on the task of developing acute medicine services within the broader vision of 'a high quality, constantly improving Trust, providing excellent clinical services'.
It is envisaged that the successful applicants will, in conjunction with the current post holders, be responsible for:
- consultant supervision of the acute medical unit
- consultant supervision of the existing ambulatory emergency care unit
- Provide acute medicine presence and support within the emergency department
- Participation in the medical on call rota (currently 1:21 weekdays and 1:7 split weekends).
- Provide consultant level care for the winter pressure escalation beds under the care of acute medicine

The post-holder would be expected to provide Consultant input and clinical leadership to the Adolescent Mental Health Pathway four days per week as well as contribute to the assessment clinic and the consultant duty rota. The role of the Consultant is to provide psychiatric assessment and treatment, contribute a psychiatric perspective, provide senior clinical leadership to the multidisciplinary team, offer consultation to other professionals and agencies and facilitate the development of the team and wider service
You will join 4 (3.2 WTE) other Consultant Child & Adolescent Psychiatrists based in C&H CAMHS. In addition to the above clinical duties, clinical supervision, teaching, training and research, the service takes part in the Trust's active and extensive Healthcare Governance and Quality Improvement agenda.
Main duties of the job
The AMHT is based at Homerton Row Site and provides assessment and treatment to City & Hackney Adolescents presenting with psychosis and severe mental illness, and those requiring more intensive support for serious mental health difficulties associated with risk to self. The role of the consultant is to provide psychiatric assessment and treatment where appropriate, contribute a psychiatric perspective, provide senior clinical leadership to the multidisciplinary team, offer consultation to other professionals and agencies and facilitate the development of the team and wider service.
The team, is clinically led by the CTL and the consultant psychiatrist together with an 8b clinician. It is a small cohesive team comprising of clinical psychology, family therapy, nursing, CAMHS Practitioner and an OT who has a joint role leading on internal duty system, as well as a Specialist Registrar from the GOSH/RLH rotation and an F2 doctor from HUH.
The Team caseload is around 45 - 50 cases including approximately 10 Early Intervention for Psychosis young people.

We have a fantastic opportunity for a General Practitioner to join our healthcare team at HMP Lewes, a category B remand prison based in Lewes, East Sussex. The prison houses around 692 patients aged between 18-65 years of age. Within this role you will be working in receptions supporting the intake of new patients into the prison. You will work closely with the multi-disciplinary team including the substance misuse nurses, and early days mental health team to deliver high quality healthcare services. Main duties of the job
You will be an experienced ECP (prescribing Nurse or Paramedic) who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. This includes patients presenting with minor illness and minor injuries within both general practice and a Minor Injury Unit setting.
They will demonstrate safe clinical decision making and expert care, including advanced assessment and diagnostic skills, for patients within general practice and the Minor Injury Unit. Job summary
BHRUT would like to invite applications for a whole time Speciality Doctor in Anaesthetics.
This is a whole time post based on 10 Programmed Activities (PAs) per week Specialty Doctor Contract. Post offers 8 PA's dedicated to clinical duties in Anaesthetics, 1 Supporting PA and additional 1 PA's for special interests and personal development.
You will be joining a dynamic and forward thinking team of doctors and the division of Anaesthesia at BHRUT provides anaesthetic, critical care and acute and chronic pain services for the local population. This takes the form of elective and emergency anaesthesia supporting the broad range of surgical specialties that the Trust provides critical care services including a consultant lead critical care outreach team acute pain ward rounds and outpatient clinics for chronic pain, anaesthetic pre-assessment and critical care follow-up.

Main duties of the job
- Contribute (in co-operation with the Audit Committee) on a regular basis to the clinical audit process undertaken within the Trust, and where appropriate at a broader regional level.
- Encourage and participate in appropriate research initiatives approved by the local Ethical Committee. Experience in research and audit is advantageous.
- Maintain an up to date knowledge of current anaesthetic practice. Undertake administrative duties associated with the care of patients.
- Attend departmental meetings. Support the Divisional Director, Specialty Lead, Associate medical Director and other professional colleagues in the management of the department in order to ensure the maintenance of high standards, a good quality of patient care and the development of the service within the Clinical Governance framework.

Job summary
An exciting opportunity has been developed to join the Geriatric Medicine Department based at UHW working within our acute inpatient wards. We are currently developing our acute frailty pathway to ensure older patients that present to UHW have timely access to comprehensive geriatric assessment (CGA) alongside treatment for their acute condition and this post will enable us to deliver excellent care for older people through their admission journey.
Main duties of the job
We have a long-embedded team based at the 'front door' of the hospital to prevent unnecessary admission of older people, alongside and Older Person's Acute Medical Unit to ensure those that need a short stay can access frailty attuned care. We plan to further enhance this pathway by ensure those that need a longer stay have access to further specialist care and are re-designing the services across our inpatient wards to support this. This speciality doctor post has been created to provide robust support and continuity for the care of patients that are cared for in this pathway.
The successful candidate will be based one of our specialist geriatric medicine inpatient wards. The post holder will work alongside consultant geriatrician, resident doctors and a wider MDT to support the routine care of older inpatients and delivery of Comprehensive Geriatric Assessment in this area.

Job summary
After a restructure of the Echo department at Harrogate District hospital we are looking for BSE Level 2 Accredited Highly Specialised Echocardiographer(s). Part-time and flexible working proposals will be considered. We are a District general hospital with a busy Cardiology service providing a wide range of investigations including physiologist led Stress echo, valve clinic and contrast.
As a positive and supportive management team we are open to any working patterns that support the role including compressed hours, job sharing, full or part time working. There is no on call commitment however the successful candidate will be expected to rotate to the CDC hub at Ripon.
Full training and support will be given in advanced techniques including strain and 4D.

Main duties of the job
- Independently perform ultrasound scans on a range of outpatients and inpatients, including adult congenital heart disease patients (ACHD), recording and interpreting images obtain
- You will work to British Society of Echocardiography standards, and adhere to all protocols and guidelines of the society
- Vetting and triage of report
- Monitor wait times and alert service managers to issues in a timely manner
- Partaking in the team quality control of reports and images
- Formulate detailed Echo reports to be personally verified and issued to consultants
- Undertake clinical supervision and training of junior and trainee physiology staff and other professional group
- Participate in Continuing Professional Development and other developmental activities, including research and clinical trial
- Work with Team Leaders and the Service Manager in further developing the clinical physiology service
