Job summary
Health Harmonie is seeking an experienced and motivated Consultant ENT to join our growing community-based service. This is an exciting opportunity to deliver high-quality, consultant-led care closer to patients homes, helping to reduce NHS waiting times and improve access to specialist ENT services.
You will provide expert assessment, diagnosis and management of a wide range of routine and moderately complex ear, nose and throat conditions. Supported by advanced outpatient diagnostics and modern clinical facilities, you will provide senior clinical decision-making and, where specifically commissioned, selected definitive ambulatory and day-case treatments.
Working within a patient-centred environment, you will play a key role in delivering timely, effective care while contributing to innovative NHS pathways that improve patient outcomes and experience.
Main duties of the job
Key responsibilities will include:
Providing consultant-led assessment and management of patients referred through central triage or escalated from Tier 2 services.
Undertaking enhanced outpatient diagnostics, including nasendoscopy, otomicroscopy, audiovestibular testing and imaging review.
Providing senior clinical decision-making for uncertain diagnoses, persistent symptoms or cases requiring specialist ENT assessment.
Managing moderately complex chronic ear disease, recurrent otitis, selected dry tympanic membrane perforations, hearing loss and tinnitus where tertiary otology is not required.
Managing persistent nasal obstruction, chronic rhinosinusitis, uncomplicated nasal polyps and septal deviation, with definitive treatment where appropriate and commissioned.
Managing persistent globus, benign tonsillar disease and low-risk dysphonia, including laryngeal examination.
Undertaking biopsy or excision of known benign lesions within the commissioned ambulatory scope.
Providing selected commissioned low-risk ambulatory/day-case procedures, including septoplasty and turbinate reduction, grommet insertion, selected myringoplasty, day-case tonsillectomy, limited polypectomy, benign lesion removal and selected FESS, subject to appropriate facilities, workforce and anaesthetic governance.
Working within agreed governance and referral pathways, redirecting patients outside the commissioned scope appropriately.
Undertaking other duties appropriate to the role as reasonably assigned.

- Tier 2 escalations where diagnostic uncertainty remains, symptoms persist despite adequate treatment, or consultant-level clinical decision-making is required.
- Moderately complex chronic ear disease, including recurrent otitis, selected dry perforations, hearing loss and tinnitus requiring specialist review but not tertiary otology input.
- Persistent nasal obstruction, chronic rhinosinusitis, uncomplicated nasal polyps and septal deviation requiring consultant assessment and consideration of definitive treatment.
- Recurrent epistaxis requiring enhanced specialist assessment and/or an agreed outpatient procedure.
- Persistent globus, benign tonsillar disease and low-risk dysphonia requiring laryngeal examination and consultant-led management.
- Known benign lesions requiring biopsy and/or excision within the commissioned ambulatory procedure scope.
- Selected low-risk ambulatory/day-case procedures, where explicitly commissioned, including:
- Septoplasty with or without turbinate reduction
- Grommet insertion
- Selected myringoplasty
- Day-case tonsillectomy

Exclusions and Onward Referral
The Consultant will ensure that patients whose clinical needs fall outside the commissioned service are appropriately referred to the relevant hospital, tertiary or multidisciplinary pathway.
The following presentations are outside the scope of the service:
- Urgent, suspected cancer or red-flag presentations, including patients requiring a head and neck cancer pathway.
- Complex airway, swallowing or voice disorders, significant aspiration risk, or conditions requiring a hospital-based multidisciplinary service.
- Complex dizziness or balance disorders, central neurological signs, or conditions requiring specialist neuro-otology beyond the commissioned service.
- Patients requiring inpatient admission, overnight care, emergency support, complex or multistage surgery, or long-term/lifelong specialist follow-up.
- Tertiary ENT conditions, including cochlear implantation, skull-base disease, complex otology, major head and neck surgery and reconstructive surgery.
- Patients with medical, anaesthetic, safeguarding or functional risks that make the proposed ambulatory/day-case setting inappropriate.
- Procedures falling outside the agreed facility, equipment, workforce or anaesthetic scope, or unresolved diagnostic uncertainty requiring hospital-based MDT review.

This is a newly created post in a busy community-based CAMHS service which serves a population size of around 330,000. The service is commissioned to provide specialist mental health support for children and young people aged 5 to 18 years presenting with moderate to severe mental health difficulties. Depression, anxiety related difficulties, self-harm, difficulties associated with neurodiversity are frequent presentations leading to referral.
Main duties of the job
Provide Medical leadership for MDT meetings.
Carry out comprehensive psychiatric Assessment and provide treatment for inpatients on the ward.
Support ward-based staff to manage psychiatric emergencies.
Conduct patient reviews and lead Multidisciplinary ward reviews, CPA reviews and multi-professional meetings.
Be the responsible clinician for the purposes of the Mental Health Act 1983(amended 2007) and carry out duties in accordance with the code of practice.
Renew and maintain Responsible Clinician status according to agreed procedures.
Carry out comprehensive Risk Assessments and participate in Trust's risk management processes.
Provide medical leadership to the inpatient ward team.
Maintain a high level of effective communication and work flexibly and cooperatively with other parts of the service including other wards and inpatient units, community mental health teams, services, and primary care.
Liaison with Families and Carers
Complete reports for Mental Health Tribunals with the support of the Specialty doctor
Attend Mental Health Tribunals or supervise other members of the medical teams in Tribunal work Assess for use of the Mental Health Act, complete consent to treatment and authorise Section 17 leave.

Job summary
From spring 2026, our services have extended to include children and young people's mental health services and all-age eating disorder services within the Trust.
An exciting opportunity has arisen for a Consultant Psychiatrist to join our dedicated Tunbridge Wells CAMHS team.
The Tunbridge Wells CAMHS Community team provides a service to children and young people (under the age of 18 years) living in Tunbridge Wells. Tunbridge Wells has a CYP population of 27 910.
The team is a generic Tier 3 CAMHS team who undertake assessment and treatment of a range of disorders in children and young people including ADHD, ASD, Mood disorders, Psychosis, PTSD, Anxiety disorders, emerging personality disorders and challenging behaviour.
We have specialist teams within Kent for the treatment of eating disorders and moderate to profound learning disability. The services run a pathway model with clear treatment pathways for Mood & Anxiety, Conduct & Behaviour, Neurodevelopmental cases and Complex cases. We are able to offer a range of evidence-based treatments including CBT, DBT, Family therapy, NVR and Child psychotherapy.
Successful applicants will ideally have MRCPsych or equivalent and be on, or within six months of inclusion on, the Specialist Register. You will be fully registered with the GMC, and have Approved Clinician status and S12 approval under the Mental Health Act (or have eligibility for it and be able achieve this on appointment).
Main duties of the job
To act as an autonomous, registered practitioner who is legally and professionally accountable for own unsupervised actions guided by the professional code of conduct and Trust guidelines and protocols.
The post holder will have full registration with a licence to practice on the General Medical Council (GMC).
Further professional knowledge will have been gained throughaccredited courses, workshops, study, and in-house training programmes.
To be responsible, and accountable, for service delivery toclients/patients.
To be able to assess and develop care plans to meet thecomplex needs of patients with a variety of conditions. This includes chronic, acute, and palliative care within own competencies, recognising own limitations and seeking advice when necessary. This will include continuously evaluating and acting on outcomes.
To be able to initiate referrals to other health professional specialist services and agencies.
To provide patients and relatives with information and educationthus ensuring they have meaningful choices that promote dignity,independence, and quality of life.
To ensure practice is supported by research, evidence-based practice, literature, and peer review.

Main duties of the job
1. To act as an autonomous, registered practitioner who is legally and professionally accountable for own unsupervised actions guided by the professional code of conduct and Trust guidelines and protocols.

3. Further professional knowledge will have been gained through accredited courses, workshops, study, and in-house training programmes.

We are looking for a dedicated consultant to join the South Kent Community CAMHS team.
The South Kent CAMHS Community team provides a service to children and young people (under the age of 18 years) living in Dover and Folkestone. Dover has a CYP population of 24 908, and Folkestone has a CYP population of 22 301.

As a Medical Director, you will play a pivotal role in providing person-centred care to our inpatients. St Giles Surgery has been establish in Camberwell for over 30 years. The team moved to a purpose built premises which are shared with our neighbouring practice Brunswick Park. Many patients have been registered with us since the onset. We are patient focused and always try to offer the best possible patient care within our control. The whole team is passionate about making a difference for our patients and improving local population health and living.
Our list has grown significantly over the years and has continued to grow. We regularly review our catchment area, ensuring we are inclusive and offering services to our immediate population.
We also offer many additional services and are always keen to pilot any new service that could benefit our local population. We work closely with our federation and are active within our Primary Care Network. We try to host as many services as possible as we believe having the services in-house, enables our patients to feel comfortable accessing the range of professionals available. We currently offer appointments with: Mental Health Practitioner, Wellbeing Coach, Social Prescriber, First Contact Physio and a Care Coordinator. In addition to this we have two PCN pharmacists as well as an employed pharmacists supporting the clinical team and patients with medication queries, compliance and monitoring. In addition to the help and guidance we provide to our own colleagues, we are a training practice and regularly host trainee placements.

EPUT is looking to appoint an enthusiastic Consultant Psychiatrist to join our Home First team based at Derwent Centre, Harlow, Essex.
The Home First Team provides crisis care in the community and sits within the Trust's Urgent Care Pathway. The team works closely with the three community mental health teams in West Essex based in Epping Forest, Harlow and Uttlesford, who may refer patients who are in a mental health crisis and requiring more assertive management and intensive monitoring in the community. The Home First Team also plays an important role in helping the Trust achieve its Flow and Capacity targets which are in place to ensure effective utilisation of inpatient hospital beds. The Home First Team will "gatekeep" admission to mental health wards with a view to assessing whether the patient could be supported in the community with the Home First Team, thereby avoiding admission. Further, the Home First Team may support inpatient wards in proactively discharging patients who are recovering and now able to be supported in the community.
Main duties of the job
The post-holder is required to work independently and accountably as part of a team of clinicians offering Mental Health services. Working collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7 day a week, 24 hours a day service for 365 days a year.
You must have full GMC registration and be on the Specialist Register, with an appropriate CCT or equivalent or be within six months of obtaining your CCT on the date of the AAC.

This post would be suitable for individuals who are seeking a career as a specialist shoulder surgeon and has an excellent track record of candidates progressing to Consultant posts in the UK and overseas. Candidates should have completed higher surgical training in Trauma and Orthopaedics including relevant experience in the subspecialty of shoulder and upper limb. Possession of the FRCS(Orth) or equivalent and full GMC registration are essential. Candidates must have achieved the CCT and be eligible for entry on to the UK Specialist Register prior to commencement of the post. Individuals who do not possess FRCS(Orth) or equivalent and a CCT need not apply.
This post will work alongside our current Shoulder & Elbow consultants, Mr Patel and Mr Little.
Main duties of the job
The post offers instruction in the management of patients with a range of shoulder and upper limb problems and exposure to surgery from basic to complex including arthroscopic reconstruction, primary and complex arthroplasty, trauma and post-trauma reconstruction. The post provides ample training opportunities in surgery of the shoulder, elbow and upper limb. The post holder will be expected to complete a service improvement project and participate actively in audit, research and teaching. The post holder's duties will include delivering care for patients under the care of the Shoulder and upper limb service, providing support to junior medical staff and receiving support and training from Consultant staff. Specific responsibilities will include overseeing the upper limb trauma service as well as the outcomes programme for shoulder and elbow surgery.
The successful applicant will be eligible to apply for the BOA Future Leaders Programme. All BOA Leadership Fellows must be members of the BOA, or join prior to commencing the scheme. The programme cost for 2025-26 is £3960/- (including VAT) and may be subject to change. Two thirds of the cost is met by the host Trust whilst the Future Leader Fellow will be responsible for one third of the cost (this does not include travel and accommodation costs). PLEASE NOTE: The full amount will need to be paid in the first instance by yourselves, and then two thirds claimed back via the Trust employee expense system.

Locum Consultant Ophthalmologist - Corneal
Applications are invited for the Fixed Term Locum post ofConsultant Ophthalmologist in Cornea.
This is Locum post which will enhance the current establishment of NHS and academic staff of the Department of Ophthalmology, Alliance Hospitals and community practices. The successful appointee will be expected to participate in undergraduate and postgraduate teaching and clinical audit. They will be required to undertake management duties and work in a multidisciplinary team. Opportunity for permanent position post 12 month fixed term pending consultant application and interviews.
The appointee will have a principle interest in Cornea, Corneal Graft operating and they will be expected to contribute to the Corneal service on the Leicester Royal Infirmary, Leicester General Hospital site (including the East Midlands Planned Care Centre EMPCC) and associated community hospitals or GP practices. Participation on the on call rota and in the eye casualty department is essential.

We are looking for an Advanced Clinical Practitioner to join our Virtual Ward and Hospital at Home Service. Our service is rapidly expanding and we are looking to further enhance the acute care that we deliver. This will be a fantastic opportunity to be part of a new venture, continuing to develop innovative care pathways utilising remote technology to provide acute care at home.
As an Advanced Clinical Practitioner you will take a lead in advanced assessment, prescribing, management and treatment of patients.
You will be based within the Virtual Ward and Hospital at Home service and work with part of the multidisciplinary team across primary, secondary and acute services. The aim of virtual ward is to reduce length of acute hospital stay and/or avoid hospital admission; this leads to quicker recovery and reduces disruption to patients', and their carers' lives. This means we can proactively manage people such as those living with long term conditions and frailty at home, for those that have a sub-acute episode we can improve a patient's experience and outcomes. In addition, this means we have more space in our hospitals for acutely unwell patients and reduces the pressure on urgent care.
You will be responsible for coordinating the provision of specialist interventions to meet the urgent health care needs of people who would have traditionally been hospitalised or those requiring facilitated timely discharge from emergency departments and step down from acute wards.
Main duties of the job
Delivering evidence based advanced practice skills to ensure the appropriate management of patients needs within the Virtual Ward and Hospital at Home Service.
Using skills of assessment, diagnosis, interpretation of tests, forward planning of care & treatment.
Review of newly admitted patients into the Virtual Ward and daily reviews including participating with speciality teams reviews and MDT's.
Timely recognition of deteriorating patients, appropriate response, escalation & documentation.
Routine writing of drug charts to prescribe medication for patients whilst in the Virtual Ward and in preparation for discharge.
Participate in, initiate, and deliver audit & research to identify current practice, drive continuous improvement & contribute to the development of new knowledge, theory & practice to benefit patients & colleagues.
Review of patients within the Virtual Ward and Hospital at Home and supporting the initial assessment of patients in the high acute beds (this will include in their own homes of patients with a range of conditions within various specialties.
Flexible approach to working within the Virtual Ward, Community Urgent Care and within the Accident and Emergency Department (front-door model) to step-up/step-down patients.

The post of Consultant Radiographer in Breast Imaging will deliver autonomous clinical practice of breast services in the Queen Elizabeth Hospital NHS Trust in accordance with the NHS Cancer Plan. There is a requirement to work as part of a clinical team lead by the Director of Breast Screening to provide and develop consultant level radiographic leadership and practice in the field of diagnostic breast imaging.
The post will play a key role as an expert professional, providing advice to Medical Consultants and other health professionals and multi-disciplinary team (MDT) groups, contributing to improved outcomes for patients, seeking to develop further the patient focused approach to care. The post holder will provide cover for Radiology Breast Consultant duties working flexibly where required.
The post requires provision of diagnostic imaging to the same level of competency as a Consultant Radiologist, to include screening and symptomatic mammographic interpretation, diagnostic breast ultrasound and percutaneous interventional breast procedures under ultrasound, stereotactic or clinical guidance. Responsibilities are also required related to the provision of education, training, supervision, mentorship and ongoing support for under and post graduate radiographers, and specialist radiology trainees in the study and development of knowledge and skills in the field of breast imaging.

Independent breast screening and symptomatic mammographic interpretation including formulation of radiology report

Main duties of the job
This Fellowship Programme is designed to provide you with experience in Paediatric Intensive Care and the transport of critically ill children. This post is suitable for clinicians keen to enhance their paediatric critical care skills and knowledge to complement their chosen career path, as well as preparing interested applicants for a career in PICM to be excellent, ambitious clinicians and leaders.
