This is a full-time vacancy of 44 hours per week. As a Senior Care Assistant at HC One, Kindness will be at the core of everything you do. You will get to know our residents and their families helping to ensure we deliver truly personalised care plans. Leading a team of Care Assistants, you will be responsible for maintaining the highest standards of care and youll be a real influence on the rest of the team. Whether you are showing someone how to use a walking frame, helping someone at the end of their life to eat a meal or talking to a residents family on the phone, youll do so with a sense of kindness, above anything else. Assisting the Registered Nurse and Care Manager, you will be involved in providing essential physical and mental stimulation through a range of social and recreational activities, as well as administering medication. One of your key responsibilities as Senior Care Assistant is to maintain accurate records of the job you do and build strong relationships with the rest of your team. Ideally, youll have completed a Level 2 Diploma S/NVQ in Adult Social Care and are prepared to work towards Level 3 qualification in Adult Social Care or equivalent, a background in person centered care planning and knowledge of caring for people with dementia. Whats essential, though, is your brilliant positive energy and natural ability to get along with people. Youll know what good record keeping looks like and be able to manage your time effectively. And, whenever you communicate with a resident or co-worker, youll be truly considerate and kind.

We are pleased to offer an exciting opportunity for an administrator to join our Primary Medical Contracts team.
The Primary Care Contract administrator will undertake key functions within team and is fundamental to the smooth running of the contracts function. We are looking for registered nurses to join our dynamic, supportive and innovative team, working in a new state of the art Critical Care Unit!
A multi million pound project to expand to 34 beds and refurbish the existing unit was completed in 2021. This gives an improved working and patient environment along with facilities and rest areas for relatives/carers.

Job summary
An exciting opportunity has arisen for a band 3 HCA to join our friendly enthusiastic and committed team. The large multidisciplinary team covers East Cheshire providing Integrated Sexual Health and Contraception services under our brand "Axess".
Experienced HCA's will be considered for a band 3 position if they hold an NVQ level 3 or equivalent, a national care certificate, have IT skills and have experience of working within a sexual health / contraception service.
Candidates without experience will be considered, all training can be provided.
The role is based at the Crewe site. Applicants may be required to travel on occasion to other locations throughout East Cheshire and be flexible to work a range of early and evening shifts.
We offer a structured induction plan and on-going supportive preceptorship using our whole team consisting of our HCA team, assistant practitioner, band 6 and 7 specialist nurses, specialist doctors and consultants. Main duties of the job

We have an exciting opportunity for a Spiritual Care Co-ordinator to join our compassionate team. This is a part-time role working 30 hours per week, based at our Hospice in Cambridge. We are happy to discuss working patterns at interview; however, the needs of the role require the successful candidate to work on Tuesdays.
As our Spiritual Care Co-ordinator, you will co-ordinate and provide spiritual, religious and pastoral care to patients, families, carers, volunteers and colleagues across our services. You will support patients approaching the end of life, undertake spiritual assessments and contribute to holistic care planning, helping us to respond sensitively to each persons individual religious, cultural and spiritual needs.
Main duties of the job
- Working as part of the PFST to facilitate reflective discussions and provide pastoral support.
- Co-ordinating volunteers and developing links with community organisations, including local faith groups.
- Facilitating prayers, blessings, rituals, memorial services and acts of worship when requested.
- Offering spiritual support to families and carers before and after a death.
- Arranging and conducting remembrance events, such as Light Up a Life and Sunflower Memories, as well as memorial services.
- Recruiting, training, supervising and supporting spiritual care and chaplaincy volunteers.
- Planning and delivering education and awareness sessions on spirituality, spiritual assessment, cultural competence, grief, bereavement and compassionate care.
To be successful in this role, you will have a good understanding of spirituality within palliative and end-of-life care, along with experience of providing spiritual, pastoral or chaplaincy support in a healthcare, hospice, community or faith setting.
You will hold a degree-level qualification in Theology, Religious Studies, Chaplaincy, Pastoral Care, Counselling, Healthcare, Social Care or a related field, or be able to demonstrate equivalent experience.
You will also need experience of conducting significant life-event ceremonies, such as marriages, and of working within and co-ordinating multidisciplinary teams.
As the role involves travel within our hospice catchment area, you will need a valid driving licence.

The Patient and Family Support Team (PFST) at Arthur Rank Hospice Charity provides practical and emotional support to patients and their families. This includes psychological and social support, spiritual care, counselling, and support both before and after bereavement.
There may occasionally be a need to work outside normal hours to support events. This will always be agreed in advance, and our time off in lieu (TOIL) system ensures that any additional hours are recognised.

You will have the opportunity to work with adults with a wide range of complex conditions. You will be supported to further develop your skills in complex dysphagia and communication, including the management of tracheostomised patients.
We have regular team meetings and are committed to continuing professional development, including via CPD sessions and MDT projects. We have a clear supervision structure, and advice is available from our Clinical Specialists within the department.
Band 6 employees are required to supervise Band 5 and assistant staff and deputise for clinical team leads. You will be encouraged to support service development through participation in internal and external projects.
You will have access to expert practitioner SLTs working with complex dysphagia, critical care, respiratory and voice. We have a well-established inpatient FEES and videofluoroscopy service.

We currently have a vacancy for a Band 3 Healthcare Assistant to work as part of the team at Beech Step-Down, based at the Lightwood site.

Beech is a 10-bedded mixed gender environment which supports individuals who have experienced a mental health crisis that has resulted in a hospital admission, however, are well enough to be discharged from hospital but due to their social circumstances do not have a suitable discharge destination.
We offer a 24-hour service and expect a high degree of flexibility within our staff team, with shifts covering mornings (Early Shift 07:00 - 15:00), afternoons (Late Shift - 13:30 - 22:00) and Night shifts (21:30 - 08:00).
We are a small team and expect a high level of ingenuity within our Healthcare Assistants in supporting service users within a homely setting, as well as working well as part of the team. We are activity-focussed so to prevent boredom and develop the skills of those under our care.
It is also expected for the staff team to be supporting our service users by identifying their needs and risks in a collaborative manner so to support them in a safe and supportive manner.

We are looking for a caring, organised and proactive Frailty Care Coordinator to join our team at Shap Medical Practice on a fixed-term basis until 31 March 2028, working 16 hours per week. Salary will be dependent on experience.
We are ideally looking for someone with a background in healthcare, social care or the third sector, although we are also interested in people who can demonstrate the right skills, experience and approach. Job summary
We are looking for a Band 5 physical health nurse to work within our district nursing teams to cover our 8am-8pm 7 days a week model. The service is undergoing significant developments as part of our plan to deliver integrated community care for patients on a neighbourhood footprint across Knowsley.
Working to the Mersey Care values, you will have excellent empathy, communication skills and a person centred approach. The service operates 24 hours per day, 365 days per year to cover our 8am-8pm 7 days a week model.

The post holder is responsible for the delivery of clinical care to a defined population. Supported by senior practitioners, they will deliver care within the boundaries of their role. They will work effectively and collaboratively within the team to meet the needs of patients, support the delivery of policy and procedures and provide clinical leadership. Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.
