An exciting vacancy has arisen for an enthusiastic and motivated individual who is interested in developing their Critical Care skills.
The purpose-built Critical Care Unit in Glangwili General Hospital is looking for a Band 5 Registered Nurse. This post is full time which will include days/nights/ weekend and bank holidays.
Main duties of the job
Successful applicants will be fully supported whilst gaining experience in the care of critically ill patients undergoing a variety of critical care interventions.
The successful candidates will ideally demonstrate some knowledge of Critical Care Nursing, but this is not essential and have a willingness to develop their skills further, playing an integral part in integrating critical care across Glangwili.
The applicants should be able to demonstrate the use of evidence-based practice and be proactive in managing change. Excellent communication and interpersonal skills are essential, together with a readiness to innovate and develop nursing within a team approach.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

If you would relish a challenge in becoming a Medical Laboratory Assistant in Microbiology, then we would love to hear from you. This is a full time role Monday to Friday and weekend/ bank holiday working is also essential.
You must have an interest in science, be self-confident, proactive and highly motivated. You must possess a wide range of skills demonstrating good numeracy and literacy, a high standard of verbal and written English, keyboard data entry skills and attention to detail is essential.
The successful applicant will support the Biomedical Scientists by performing a wide variety of tasks associated with the processing of samples. Duties vary between departments and include the sorting, preparation, processing and handling of blood, urine, tissue and bodily fluids.
Main duties of the job
You will also be required to perform data entry onto the laboratory computer system, undertake housekeeping duties, maintenance of equipment and disposal of clinical waste therefore the ability to multi-task and work flexibly is essential.
You will be working as part of the Derbyshire Pathology Service within the Microbiology Laboratory based at Royal Derby Hospital.
- To work in an effective and efficient way following the appropriate Trust and directorate policies, procedures and guidelines including the departmental Standard Operating Procedures (SOPs).
- To prepare and process patient specimens including -
- Preparation of urine samples for microscopy
- Preparation of mycology specimens for microscopy
- Inoculation of patient specimens onto culture media for incubation
- Separation of blood samples and setting up of tests
- Processing of swabs for screening tests
- Performing specimen reception tasks including the accurate entry of patient/specimen details onto the departmental computer system maintaining a valid database.
Maintaining and supporting the effective use of available resources (equipment and stock) in an efficient way.

We are seeking an enthusiastic and skilled Clinical Psychologist to join the Older Adults Community Mental Health Team (CMHT) in King's Lynn covering the West Norfolk Locality. This is an exciting opportunity to work within a well-established service supporting older adults with a broad range of psychological, emotional, and cognitive difficulties. The role offers experience across both functional mental health problems, including depression, anxiety, psychosis and complex emotional presentations, as well as organic conditions, including dementia and related neurocognitive disorders.

Our Service
The West Norfolk Older Adults CMHT provides comprehensive, multidisciplinary assessment and intervention for older adults experiencing a wide range of mental health difficulties. The team works closely with service users, families, carers, primary care colleagues and wider health and social care partners to deliver person-centred, psychologically informed care.
Main duties of the job
As a Clinical Psychologist within the West Norfolk Older Adults Community Mental Health Team, you will:
- Provide specialist psychological assessment, formulation and intervention for older adults presenting with a wide range of functional and organic mental health difficulties.
- Deliver evidence-based psychological interventions to individuals, families and carers, drawing upon your specialist training and expertise.
- Work collaboratively with service users, carers, multidisciplinary colleagues and partner agencies to support recovery, wellbeing and quality of life.
- Provide psychological consultation, advice and formulation to multidisciplinary team colleagues working with complex presentations.
- Contribute to multidisciplinary assessment, care planning, risk management and decision-making processes.
- Undertake work with older adults experiencing cognitive impairment and dementia, including supporting carers and systems around the individual.

Job summary
Whiston Core 24 is a specialist assessment of service user's mental health to those who present to the Emergency Department in times of crisis or, those who are referred from the acute general hospital wards and the formulation of treatment and risk management plans based on assessment of needs with the appropriate triage and diversion form the service.
The service operates on a shift system of long and short days (currently under review), inclusive of nights to provide 24/7 365 days a year service. The service has recently undergone Organisational Change to make the service more flexible across the Urgent Care footprint, providing support to Mersey Cares additional Core 24 services including Royal Liverpool, Warrington, Whiston and Southport Core 24 services where required.
We are looking for an experienced practitioner with a wealth of experience working in mental health services, inclusive of organic services.
Main duties of the job
To provide high quality, evidence based care within Division for service users and their carers within inpatient and community services.
To work in a socially inclusive way and in line with the recovery model.
To function as named nurse/care coordinator to service users undertakingassessment, planning and evaluation and review of care, within a recovery focused model of care.
To participate as an active member of the multi disciplinary team.
To provide advice, expertise and leadership within the team.
To supervise care for service users presenting with complex and high risk needs.
To work autonomously, reaching clinically appropriate decisions based oninformation received and using the knowledge and experience of the multidisciplinary team.
To be qualified to act as a mentor for students.

Main duties of the job
As a Care Navigator for the Bexley Care SPC, the post holder will manage all incoming communications including electronic, telephone, letter and/or any other forms of communication. The post holder will use information systems (RIO and Liquid Logic) and make judgements regarding referrals using algorisms to establish the urgent status of potential new referrals and ensure people are seen by the most appropriate team at the right time to meet their needs. Job summary
This role focuses on tracking and managing the referral and treatment process for cancer patients, ensuring that those referred under the Suspected Cancer Pathway are diagnosed promptly and that newly diagnosed patients begin treatment within national timescales- Cancer Guidelines V12.1. The individual will be responsible for maintaining accurate tracking systems and producing weekly reports to monitor patient progress. They will also liaise with MDT Navigators across SWL network to ensure that patients requiring treatment are flagged and treated accordingly.
In addition to referral and treatment tracking, the MDTC are expected to be involved in capturing and updating patient information in Trust databases, ensuring all relevant details are recorded and up-to-date. The role requires collaboration with the Senior MDT co-ordinators to develop and monitor national and local datasets, including tracking waiting times for newly diagnosed cancer patients. Timely reporting on these metrics is essential to ensure all patients are receiving care in a timely manner
This post is for Monday-Friday between 08:00am and 17:00pm on a rota basis. Flexible working cannot be considered for this role.
Main duties of the job
Operational Responsibilities/Support to Departments
1. Assist with the regular review of Multidisciplinary Team Operational Policies and Procedures.
2. Supporting the Cancer Services Management team in the delivery of Cancer National Waiting times and Breaches workshops.
3. To support Cancer Services Management Team in delivering Cancer audits, COSD, staging target CQUIN for the year.
4. Responsible for investigating Breaches of the Cancer Waiting times and for providing the feedback to involved parties.
5. Responsible for producing the resulting Breach Reports, which are submitted to the CSU (Commissioning Support Unit)/CCG(Clinical Commissioning Group) in accordance with reporting procedures.
Data Co-ordination and Tracking Cancer Patients
1. To track all Two-Week Rule referrals to ensure that suspected cancer patients receive prompt diagnosis, and to work with other Trust staff to bring forward appointments if necessary.
2. To update tracking systems daily to indicate whether Two Week Rule patients have a benign or malignant diagnosis.
3. To work with Clinicians, Clinical Nurse Specialists, schedulers, diagnostic staff and other cancer data co-ordinators to ensure identification and recording of the Trust's newly diagnosed cancer patients.
4. To track all newly diagnosed cancer patients to ensure that they are treated within national timescales, and to work with staff from this Trust or others

We are looking for 1 fulltime innovative and motivated Community Support Worker to work in the Reconnect Service across Nottingham City and Nottinghamshire County.
The Reconnect Service aims to improve the continuity of care for vulnerable people leaving prison by working with them before they leave and supporting their transition to community-based services, safeguarding health gains made whilst in prison.
The Reconnect Service does not provide clinical interventions but instead offers liaison, advocacy, signposting and support to men and women leaving the detained estate, to support engagement with community-based health and social services. This approach aims to support the wellbeing of people leaving prison, reduce inequalities and address health-related drivers of offending behaviours.
The Reconnect Service aims to increase access to and uptake of healthcare or relevant support services for individuals who have health vulnerabilities, who would otherwise struggle to engage. Key to the success of the service is close partnership working with justice agencies, health and social care and third sector agencies.

Main duties of the job
This is a relatively new development within Notts Healthcare and is a great opportunity to work on developing and establishing a vital service for service users leaving prison, creating the best opportunity for supporting their engagement and helping to break the cycle of offending.
The Community Support Worker role is an integral part of the Reconnect Service and will enhance the work of the Reconnect Team in supporting people whilst in custody in preparation for their release and once in the community. The community support offered will aim to support service users to engage with services such as physical health, mental health, substance misuse, and will include support to attend appointments with other agencies, supporting an effective transition into the community and health equalities. All work will be carried out under the supervision of the Team Manager/Reconnect Practitioners.
The staff member will be expected to support individuals who are residing in Nottingham and Nottinghamshire and potentially being released from HMPs across the country who are returning to the area. Main duties of the job
The post holder will provide administrative support to the Long-Term Ventilation (LTV) and Sleep service, supporting children and young people with respiratory and sleep-related conditions. The Paediatric Neurodisability Nursing Team provides co-ordination and case management of children with highly complex and unstable long-term conditions in partnership with health, education and social care organisations.

Main duties of the job
Working alongside the Paediatric Neurodisability Nursing Team the Special School Nursing Service, provides nursing care, co-ordination and case management for children with acute, highly complex and unstable long-term conditions, in partnership with health, education and social care organisations.
The service facilitates children and young people with healthcare needs, between the ages of 2 and 19 years of age, to access the education curriculum. The Band 6 staff nurse will provide nursing care to children ad young people with complex medical needs and learning disabilities will manage a defined caseload and act as a specialist resource for professionals and families providing them with specialist advice in order for them to make informed choices about their health.
The role also includes training and supervision of junior staff and non-health care professionals and line management responsibilities. The post holder will work between the special schools in Croydon and be based at St Giles School. The post includes a range of clinical care and the training and competency assessment of education staff.

Job summary

We have access to Bespoke CPD training and offer regular supervision to our staff to ensure you are valued and supported in your career development.
Our NHS Talking Therapies teams include a Clinical Leads, Operational Leads, Psychological Therapists, Lead Psychological Wellbeing Practitioner, Senior Psychological Wellbeing Practitioners Psychological Wellbeing Practitioners and Assistant Psychological Wellbeing Practitioners. We also train significant numbers of trainee therapists.

We can offer contracts on either our Trust Bank or contracted hours from 22.5 hours a week up to 37.5 hours.
Main duties of the job
- Accept referrals via agreed protocols within the service.
- Assess and supports people with a common mental health problem in the self-management of their recovery.
- Deliver LI treatments, primarily on the telephone and in groups, with some face to face work.
- Undertakes patient - centred interviews which identifies areas where the person wishes to see change and or recovery and makes an accurate assessment of risk to self and others.
- Make decisions on suitability of new referrals, adhering to the department's referral protocols, and refer unsuitable clients on to the relevant service or back to the referral agent as necessary or steps-ups the person's treatment to high intensity.
- Provide a range of information and support for evidence based high volume low - intensity psychological treatments. This may include guided self - help computerised CBT, information about pharmacological treatments. This work may be on the telephone, via other media or face to face.
- Educate and involve family members and others in treatment as necessary.
- Adhere to an agreed activity contract relating to the number of client contacts offered, and clinical sessions carried out per week in order to minimise waiting times and ensure treatment delivery remains accessible and convenient.
- Attend multi-disciplinary meetings relating to referrals or clients in treatment, where appropriate.
- Complete all requirements relating to data collection within the service.

We are a small rural practice which prides itself on its friendly and caring ethos. The successful candidate will work as part of the administration team and will have suitable customer service experience.
The ability to deal with patient queries in a professional, friendly and caring manner is essential. Flexibility is required in order to cover annual leave, absences and any additional hours required to meet the needs of the practice.
Main duties of the job
Provide general administrative assistance to the practice team to ensure that patients receive the high quality service that the practice aims for and that the practice runs in an efficient and effective way. Project a positive and friendly image to patients and other visitors, either in person or via the telephone.
Previous experience working in the NHS is desirable but not essential as comprehensive training will be provided. Excellent interpersonal skills, good IT skills, excellent telephone manner and good communication skills are essential to the role. The candidate will need to have a flexible working approach and have the ability to work with a wide range of professionals, people and patients.

The primary aim of the therapy service is to ensure patients can live well and as independently as possible in their own homes following acute hospitalisation. This will be achieved through:
