Job summary
Hello!
My name is Jane Yarnell, my role as the Lead Practitioner Development Technician for the OUHFT Pharmacy Department means that I am privileged to lead the training for our fantastic team of Pharmacy Level 2 apprentices, enabling them to gain the skills necessary to become a Pharmacy Assistant within one of our departments across the trust. Pharmacy Assistants are essential members of the pharmacy team working under the supervision of a Pharmacist and alongside Pharmacy Technicians to provide pharmaceutical care for the patient. The duties are varied, and it is not simply a matter of counting tablets. I am currently looking for a level 2 Pharmacy apprentice to join the Horton General Hospital Pharmacy department.
During your 15-month contract full practical in-house training will be provided, you will undertake a level 2 Specialist Btec in The Principles and Practices for Pharmacy Support Staff, this will provide you with the skills to work as a Pharmacy Assistant, alongside the knowledge you will need to support your pharmacy career. You must be a team player, keen to further your education, and have a good attention to detail as well as the ability to work accurately in a busy environment and be able to communicate clearly with patients and staff.
Main duties of the job
It is desirable that you have 4 GCSEs of 3 or above, but we can help with functional skills.

- To efficiently undertake basic dispensary duties which may include the following duties as directed:

Job summary
The Ridgeway Surgery in Harrow is looking for a Care Navigator to help patients access the right care and make their experience of contacting the practice as straightforward as possible.
Working closely with our clinical teams, you will review patient requests, coordinate appointments and follow-up, and guide patients towards appropriate services. You will use agreed practice pathways, with support from the duty clinician whenever advice is needed. Main duties of the job
Your main focus will be helping patients reach the most appropriate care through our online consultation system, telephone and face-to-face contact. You will clarify requests, check relevant information and use practice pathways to arrange appointments or direct patients to other services.
You will work closely with the duty clinician, promptly escalating urgent concerns, safeguarding issues and any requests that need clinical judgement. You will help maintain continuity of care and support patients who need additional assistance to access services.
Day-to-day work will also include coordinating routine reviews and follow-up, handling administrative queries and keeping accurate records. When required, you will support reception by welcoming patients at the front desk, answering calls, processing registrations and assisting with general enquiries.

Job purpose
To help patients access the most appropriate care, professional or service by reviewing requests, gathering relevant information and following agreed practice pathways.
The Care Navigator will work closely with the clinical team to coordinate appointments and follow-up, support continuity of care and assist patients who need additional help accessing services. The postholder will also support core reception services when required.
This is a non-clinical role. The postholder will work within their training and agreed procedures, seeking clinical advice whenever a request is unclear, raises concerns or requires clinical judgement.
Main responsibilitiesCare navigation and patient access
- Review patient requests received through the practices online consultation system, by telephone and in person.
- Read the full request and check relevant information in the patient record, including previous consultations, existing plans and outstanding appointments.
- Contact patients to clarify requests or obtain further information, using agreed questions and procedures.

We are excited to be able to provide an opportunity for a Nurse Associate to join our dedicated neonatal transitional care team. We are a transitional care unit within the postnatal ward that is commissioned to take 8 cots. You will be an integral part of this collaborative environment as a member of the neonatal team.
You will assist the neonatal nursing staff in the neonatal and transitional care units by providing individualised, family-centred care to infants until they are discharged. The successful candidate will work alongside the neonatal and postnatal teams. You will primarily work on the Transitional Care Unit but will also have the opportunity to rotate to the neonatal unit and work alongside the inpatient maternity team. You will play a crucial role in helping to provide care for the infants with their mothers, through routine observations and assisting doctors when needed.
The successful implementation of an NTC program depends on effective collaboration between community, midwifery, neonatal and paediatric teams. So, it is crucial that you can work collaboratively in a multidisciplinary and ethnically diverse team.
This role will play a key role in the ongoing provision of quality NTC services at the Trust, focusing on the needs of both mother and baby. This practice is in keeping with our philosophy of family-centred care in the maternity unit.

Main duties of the job

Apprentice Healthcare Science Assistants will undertake an initial structured departmental induction and then follow a training programme throughout the apprenticeship to enable them to:
- Work under the supervision of the Biomedical Scientist and other team members in the delivery of high-quality care.
- Provide assistance and support to a broad spectrum of service users, and the multi-disciplinary team.
- Perform a range of routine healthcare science activities under supervision.
