North Devon Liaison Psychiatry Consultant Post 6PA Devon Partnership NHS Trust.

The team is friendly, supportive and well regarded for staff retention and highquality clinical work. The hospital is small and easy to navigate, enabling strong relationships across clinical and managerial teams, with a caring and relaxed culture.
The post is well supported by local Psychiatric Consultants, regular team supervision, links with Liaison Teams across the county and close collaboration within the team. The service works a Core 15 model, with the medical component providing care 9am5pm to people admitted to inpatient wards or attending the Emergency Department.
The team also contributes to the Integrated Psychological Medicine Service with Talkworks (IAPT), Health Psychology and Medical and Surgical Specialties, developing pathways for people with functional and chronic conditions.
The multidisciplinary team includes Nurses, Occupational Therapists, Social Workers, Liaison Psychologists, Consultant Psychiatrists and Junior Doctors. We work in a selfcontained unit that supports excellent communication while remaining closely connected to the general and psychiatric hospital on the same site. Strong administrative support is in place.
Main duties of the job
The role is to provide Consultant leadership to a multidisciplinary Liaison Psychiatry service, with opportunities for development and clinical leadership in the Integrated Psychological Medicine Service.
The clinical work is highly variable which makes it challenging at times but very interesting. This service is currently for patients age 18+ and would fit with either General Adult, Older Adult or Liaison training experience.
Teaching, as with all liaison teams, is an essential part of the role both locally, regionally, within the hospital, within the team and with medical students and is a rewarding part of the post.
The post holder will be expected to engage in the service governance programme and service improvement projects. There is an active and supportive QI team in the Trust.

Summary of position:

Clinical Practice
As a Staff Nurse working within Critical Care, the expectation is that, with adequate training, supervision and assessment, you will be able to;

Summary of position:

The Staff Nurse post incorporates working within both Cheltenham General and Gloucestershire Royal Hospital, working on a continuous rotational basis. Job summary
We are seeking an experienced Physiotherapist with a passion for providing high quality, patient-centred care for the medical and frail patient cohort in the acute hospital setting, to join our Medical Rehabilitation Therapy team.
They will primarily provide clinical and leadership support to the Medical Rehabilitation Therapy team on the discharge ready unit (DRU) and play a vital role in supporting patient flow and timely discharges. They will also support across the team's medical, care of elderly & frailty wards as required. The post holder will be expected to help other therapy teams as, and when required.
The ideal candidate will have a broad range of clinical experience in core areas including acute inpatient wards, critical care or respiratory, neurology and orthopaedics. Additionally they should have some experience of leadership- either through previous experience of supervising junior staff or students and being involved in service development projects.
As part of the role, the post holder will be expected to undertake respiratory on-call shifts and work as part of the weekend rota (which may include cover between the respiratory, orthopaedic and ED and Acute Medicine weekend rotas. This also includes Bank Holiday cover).
Main duties of the job
The post holder will manage a clinical caseload on our medical and frailty wards and discharge ready unit wards whilst also providing direct line management to junior Physiotherapists and Therapy Assistants. They will also be expected to take on students and share their expertise with the wider multi-disciplinary team as part of the in-service training rota. They will be expected to support rehabilitation of patients, prevent deconditioning of patients, as well as be involved in discharge planning of patients.
Additionally, the post holder will work alongside their Occupational Therapist colleagues to support the service and staff development across the acute medical therapy service.
As part of the role, the post holder will be expected to undertake respiratory on-call shifts and work in the respiratory and orthopaedic weekend rota (which includes Bank Holiday cover).

Job summary
Elton Rehabilitation Unit is a 8 bedded facility that is located in Clevedon, North Somerset. The unit is one of a number of community units providing support and active rehabilitation for those people being supported on Discharge to Assess pathway 2 (please see below).
Following a stay in the acute setting, the ethos for all people being discharged is 'home is best' and a large number of people are supported to return home utilising the robust and established ~Discharge to Assess Pathway 1. For those people that are identified as having on-going rehabilitation needs, they are considered to 'stepdown' into a Pathway 2 Rehabilitation, Reablement and Recovery bed at the Elton Rehabilitation Unit.
On the Unit, they will receive therapy intervention and support around complex discharge planning, to enable a timely and safe discharge to their usual residence or to a more suitable location if appropriate.We actively promote a multi-disciplinary model on the Unit, where nursing staff work closely with therapist, social workers, GP's with Special Interest in Older People's Care as well as other Acute and Community colleagues, to ensure the person we are supporting is kept at the heart of every decision
Main duties of the job
The Health Care Assistant role compliments and forms a key part of the multi-disciplinary healthcare services within the organisation, implementing and providing high standards of individualised care for the service user. You will be part of an integrated multi-disciplinary team (MDT), which includes nurses, occupational therapists, physiotherapists and other allied healthcare professionals.o You will provide holistic, person-centred care and support for service users of all ages in a variety of settings. As a member of the MDT team you will support the registered nurses in the delivery of care to service users and contribute towards the safe and seamless delivery of holistic care, working safely within your scope of practice and competence at all times.o Your primary purpose will be to carry out routine care and health tasks which will be set out in agreed care plans as directed by the registered nurse. Job summary
The opportunity has arisen for a motivated Speech and Language Therapist with an interest in working with people with a learning disability to take a Band 6 specialist role in the Sirona Adult Learning Disability Health Service (SALDHS).
This is a full-time position based in North Somerset, with opportunities to also work with colleagues covering South Gloucestershire and Bristol. The service hours are Monday to Friday, 9am to 5pm.
You will work within a locality multi-disciplinary team, with the opportunity to work with specialist SLTs across Bristol, North Somerset and South Gloucestershire delivering person centred care, and training to care and support staff.
The post offers excellent support with opportunities to develop skills in:
- Assessment and management of communication, including lining with AACWEST
- Developing resources and accessible information
- Developing skills in Signing, using Talking Mats, and AAC
- Delivering training to carers and family members
- Opportunity to develop skills in managing eating, drinking and swallowing issues
- Excellent supervision and peer support available as well as regular Journal Club and opportunities to develop care pathways
- Regular team meetings and MDT working
Main duties of the job
You will hold a clinical caseload of people with learning disabilities who have communication and/or eating and drinking needs. Our service users have a broad range of communication needs including supporting the implementation of AAC, the use of signing, Talking mats, Social Stories and Visual support strategies. Management of complex dysphagia, including triage, prioritisation, assessment, risk assessment and the development of individualised care plans as well as care and support staff education to support understanding and implementation of guidance given.
Your work will be in the community seeing people in their homes or other familiar and appropriate environments. You will also be working collaboratively within our multi-disciplinary team which provides a holistic approach to meet the service users health needs. You will receive support from clinical specialist SLTs and the SLT Clinical lead to develop in this role We actively encourage skill development and case discussion to ensure the team deliver a high-quality service that is innovative, safe and continuously improving for a positive and meaningful service user experience.
Alongside your clinical caseload there will be opportunities to provide advice, education and training to health staff, social care workers and family carers. and information in a range of formats to meet service user needs.

We currently have a vacancy for a Clinical Development Fellow at ST1/2 level in our Emergency department.
This is an exciting opportunity to join a dynamic department. We are looking for enthusiastic, positive individuals with a commitment to improving the care given to our patients.
You will be part of a team of doctors looking after patients on the paediatric ward working alongside the multi-disciplinary team.
Main duties of the job
Under consultant supervision, provide care for patients in the department with responsibility for the prevention, diagnosis and treatment of illness or injury.
Refer patients to appropriate in-patient facilities where required and work effectively as part of multidisciplinary teams and meetings.
Teach and supervise junior staff as appropriate and contribute to relevant administration duties, including rota schedules.
Participate actively in audit, clinical governance and the day-to-day management and administration of the department.
Deputise for absent colleagues where this is consistent with the discharge of duties; where this occurs outside normal contracted duties and commitments, equivalent time off duty will be given.
Be available for irregular commitments outside normal reserved duties where essential for continuity of patient care, including public holidays as required by the rota with time off in lieu.
Take responsibility for your own continuing professional education in accordance with the appropriate college, faculty or professional body guidance.
The post is based at King's Mill Hospital but may include activity at Newark Hospital if required.

As a junior sister/ charge nurse you will have responsibility for monitoring standards and quality in the development, implementation and evaluation of patient care. You will monitor and support staff training and education, risk management reporting at ward level and budget control including allocation of resources.
A strong clinical background in acute general medicine is essential to this role and knowledge of caring for patients with impaired cognition issues, to support the nursing team and ensure our patients receive the right standard of care.
As a junior sister / charge nurse, you will work closely with the multi professional team members to enhance service delivery. As an individual new to the role, you will be well supported by your peers within the medical division.

Job summary
As part of the wider Healthcare of Older People (HOP) team, the post-holder will be based in The Harbour service at Newton Abbot Hospital, providing senior clinical decision-making and clinical leadership to enable people living with frailty to receive timely assessment, treatment and care closer to home. The role incorporates frailty coordination and telephone triage, Harbour @ Home (frailty virtual ward), same day emergency care (SDEC) with direct access to community hospital beds at Newton Abbot Hospital.This post includes contributing to evening and weekend rotas.
Main duties of the job
o Be part of the frailty coordination function, providing clinical telephone triage to referrers including SWASFT, Primary Care, community teams, acute hospital wards, the Emergency Department and Acute Medical Unit (AMU), identifying the most appropriate way to meet the patient's acute needs.o Hold clinical responsibility and accountability for agreed patients while they are under the care of The Harbouro Provide senior clinical decision-making and leadership to the multidisciplinary Harbour team and deputise for the GP Lead in their absence.o Work across acute care, The Harbour and existing community services, particularly Urgent Community Response (UCR), accessing same-day interventions where appropriate.o Work with intermediate care, Primary Care and the wider community multidisciplinary team.o Deliver high-quality specialist assessment and intervention for older people living with frailty, using the principles of Comprehensive Geriatric Assessment (CGA).o Use point-of-care testing, digital monitoring and other appropriate clinical support to facilitate care deliveryo Where appropriate, contribute to the training and supervision of other Harbour clinicians including trainee and qualified advanced clinical practitioners and resident doctorso Contribute to service development, quality improvement, data evaluation and teaching.
