Job responsibilities
Clinical:
To undertake a comprehensive assessment of patients including those with diverse or complex presentations/multi pathologies; use advanced clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition.
Undertake video and telephone assessments as well as face to face sessions.
Formulate and deliver an individual therapy treatment programme using assessment, reasoning skills and treatment skills based on a sound knowledge of evidence based practice and treatment options. Formulate accurate diagnosis prognoses and recommend best course of intervention, developing comprehensive discharge plans.
To take delegated responsibility from the Band 7 therapist for managing patients with particular conditions and be responsible for providing specialist therapy assessment and treatment plans for patients with these conditions.
Evaluate patient progress, reassess and alter treatment programmes as required.
Work as a lone practitioner with telephone support from a senior therapist if required.
To demonstrate highly developed dexterity, co-ordination, palpatory and other senses for assessment and manual treatment of patients
To communicate effectively with patients and carers to maximise rehabilitation potential and to ensure understanding of their condition
To assess capacity, gain valid informed consent and have the ability to work within a legal framework with patients who lack capacity to consent to treatment
To communicate effectively and work collaboratively with medical, nursing, therapy staff and other health and social care and education professionals to ensure delivery of a coordinated multidisciplinary service. This will include discharged planning and other forms of verbal/written communication as necessary including report writing, referrals to other agencies
To communicate highly sensitive information effectively with all patient groups/carers were there maybe difficulties in accepting or understanding information
To maintain tact and diplomacy and use highly developed communication skill when working in situations which may become hostile/antagonistic

We are looking for an experienced and motivated Band 6 Physiotherapist to join our Fareham and Gosport Community Therapies Team.
Working as part of a supportive multidisciplinary team, you will provide high-quality Occupational Therapy assessment and intervention to adults within the community, supporting people to maximise their independence and achieve their rehabilitation goals.
You will manage your own caseload, using sound clinical reasoning to assess functional needs, identify and manage risks and develop individualised treatment and rehabilitation plans. Main duties of the job
Undertake Occupational Therapy / Physiotherapy assessments of patients with diverse and complex conditions, providing a clinical diagnosis (where appropriate), development, and delivery of individualised treatment programmes.

We are looking for 2 highly motivated Staff Nurse to join our established, busy 30 bedded Surgical Assessment Unit (SAU) at the Conquest Hospital, as maternity cover for 10 months.

Main duties of the job
Episodic care rather than continuity of care the pharmacist manages the patient's presenting clinical need during the Enhanced Access consultation.

We share a single patient list but our patients are attached to a specific site and are looked after by local multidisciplinary teams which include GPs, Nurses, Physicians Assistants, Paramedics, ANPs and Pharmacists. Our site based teams are supported by Managers and local administration staff. Our GP Partners have overall clinical and administrative responsibility for Nexus Health Group and are supported by a Senior Management Team and centralised HR, IT, clinical administration functions

We currently have a vacancy for a Band 3 Healthcare Assistant to work as part of the team at Beech Step-Down, based at the Lightwood site.

Beech is a 10-bedded mixed gender environment which supports individuals who have experienced a mental health crisis that has resulted in a hospital admission, however, are well enough to be discharged from hospital but due to their social circumstances do not have a suitable discharge destination.
We offer a 24-hour service and expect a high degree of flexibility within our staff team, with shifts covering mornings (Early Shift 07:00 - 15:00), afternoons (Late Shift - 13:30 - 22:00) and Night shifts (21:30 - 08:00).
We are a small team and expect a high level of ingenuity within our Healthcare Assistants in supporting service users within a homely setting, as well as working well as part of the team. We are activity-focussed so to prevent boredom and develop the skills of those under our care.
It is also expected for the staff team to be supporting our service users by identifying their needs and risks in a collaborative manner so to support them in a safe and supportive manner.

Main duties of the job
The successful applicant will be expected to implement identified plans of care for service users under the direction/supervision of qualified staff to meet the needs of the individual's identified care plans. We have an exciting opportunity for an experienced Mental Health Practitioner to train at Derby University for the initial 12 months to gain a qualification in Cognitive Behavioural Therapy as part of the Children's and Young People's Mental Health Support Teams (Reflexions). The successful postholder must be prepared to travel to Derby University for two days per week in order to complete the training.
Upon completion of the course there is an expectation for you to apply for accreditation with BABCP.

Main duties of the job
The Reflexions team can provide the opportunity to further develop work within the field of young people's mental health and wellbeing with a focus on development of whole school approaches to mental health and wellbeing and the development of the think family approaches within the schools environment.
The mental health support teams provide individual interventions, guided self-help, parenting and whole school approaches. Working with parents/carers and teaching staff is also part of the service offer.
The post holder will be knowledgeable and experienced in mental health problems and psychiatric disorders and will be expected to carry out in-depth mental health assessments including risk assessments where required. The postholder will also need to be the link worker between the team and the educational settings to ensure that children and young people receive the most appropriate care and interventions and well as being experienced in multi-agency working. Support, advice and consultation to families/carers and other professionals involved in the life of the child/young person will also need to be provided.
About us
The post holder will work in partnership with the Reflexions Team Leader and Clinical Lead to further develop the partnership working that is currently taking place between Education, Local Authority, Child & Adolescent Mental Health Service (CAMHS) and other external agencies.
Applicants should be experienced in mental health at a senior level with the required clinical skills to support the Mental Health Support Team. Experience of recent working with schools and delivering mental health promotional collaborative work in such an arena is vital in order to be able to fulfil the role.
Applicants must have excellent communication skills and be able to network with other agencies such as Public Health, Clinical Commissioning Groups and Local Authorities amongst others as and when required. It is beneficial for applicants to have experience of managing staff.

The Clinical Outreach Nurse is a key member of the Sexual Health Hertfordshire Outreach Team, responsible for planning, coordinating, and delivering innovative sexual health outreach services across Hertfordshire. Working collaboratively with community organisations, voluntary sector partners, education providers and healthcare professionals to identify needs, develop targeted outreach initiatives, and improve outcomes for vulnerable and underserved populations.
As a clinical expert, you will advocate for sexual health promotion, support prevention strategies, safeguard vulnerable individuals, reduce health inequalities, and strengthen engagement with communities. Main duties of the job

They will participate, directly and indirectly, in the assessment of patients referred to the service and in drawing up care plans which will address the patients' psychiatric, physical, and social needs.
They will carry out detailed psychiatric assessments of patients as clinically indicated to allow accurate diagnoses and contribute to their treatment plans, including pharmacological management of psychotropic medication.
The post-holder will forge good working relationships with other psychiatrists across the Directorate in order to encourage appropriate referrals and assure a cooperative approach to the care of patients. They will be linked in with other Consultants and supported to join peer groups. They will also provide consultation and advice to members of the MDT and primary care colleagues
