This role offers an exciting opportunity for a motivated Occupational Therapist to join Bromley's evolving Experts at Hand (EaH) model and play a central part in the borough's SEND transformation. You will be part of a multidisciplinary team working to strengthen early identification, targeted intervention and inclusive practice across education and early years settings. As a School-Based OT, you will help shape a new, prevention-led offer that brings specialist expertise closer to classrooms, families and school communities. You will deliver evidence-based occupational therapy within a tiered Universal-Targeted-Targeted+-Specialist framework, providing coaching, consultation and modelling of OT strategies in real school environments. Day-to-day practice includes supporting whole-school approaches, delivering targeted interventions, implementing therapy programmes, and collaborating with teachers, SENDCOs and families to improve functional outcomes for children and young people. You will hold a targeted+ and specialist caseload, completing assessment and intervention in context for children with occupational performance needs and functional difficulties.Main duties of the job
1. To work autonomously and without direct supervision in a defined clinical area, recognising professional boundaries and seeking guidance where necessary, but having accountability for own clinical actions.
2. To be responsible for a clinical caseload of paediatric occupational therapy including assessment, interpretation of assessment results, evaluation, treatment, and advice, through the use of evidence-based practice and outcome measures.
3. To work with education partners using evidence-based approaches such as coaching, modelling and guiding to enhance capacity in the educational context and support children's functional participation as part of the OT offer.
4. To provide support and training to educational partners via modelling and setting up of group interventions, workshops, or OT surgeries when needed as part of the OT offer.
5. To carry out assessments and evaluate the child's occupational performance in context. This may include the use of standardised and non-standardised assessments, observation, interview with parent or carer and/or parent and teacher questionnaires.
6. To develop, plan and implement specialist Occupational Therapy treatment programmes and strategies to enhance the child's ability to perform everyday activities and participate in multiple environments. This could include verbal and written advice/ recommendations, written advice sheets, individual treatment and/or group therapy.

An exciting opportunity has arisen for an experienced and motivated Occupational Therapist to take a leading role in delivering Bromley's Experts at Hand (EaH) model, supporting system-wide SEND reform and strengthening universal and targeted provision across education and early-years settings. As a locality lead within a tiered service delivery model, you will drive early identification, inclusive practice and workforce capability building. Working as part of a wider multidisciplinary team, you will shape whole-school approaches, lead consultation and coaching, and embed evidence-based OT strategies across diverse educational environments. Day-to-day practice includes coaching and modelling OT strategies in context, delivering targeted interventions, supporting staff training, and responding flexibly to emerging needs within your locality. Alongside these system-level responsibilities, you will maintain a small targeted-plus and specialist caseload, providing assessment, intervention and collaborative problem-solving for children with more complex occupational performance needs.

The Children`s Occupational Therapy Service provides high-quality occupational therapy to children and young people 0-18 years old, with difficulties in play, self-care and/or school occupations due to a variety of conditions. Occupational Therapists within the service work autonomously, seeing children in a variety of community healthcare and educational settings including clients' homes, clinics, schools, nurseries and specialist educational settings. As an experienced practitioner, the Highly Specialist Children`s Occupational Therapistwill manage a team of occupational therapists who will work to build capacity in mainstream schools through universal and targeted work, while also holding a complex clinical caseload of specialist cases. The Occupational Therapy lead will be expected to oversee as well as support the team in managing the offer for schools and early year settings while ensuring children who need specialist care get timely assessments. Our Clinical Homecare Nursing team really love what they do! Working in Partnership with our NHS colleagues to provide individualised quality nursing care to patients requiring Parenteral Nutrition and other IV therapies in the comfort of their own homes.
This is a unique opportunity for an experienced nurse to work autonomously while delivering high-quality clinical care outside of a hospital setting.

Key Details:
Location: Working with trusts and patients in Liskeardand Surrounding areas.
Working hours: Monday to Sunday, 40 hours over 5 days per week on a rolling rota.
The shifts are structured as split shifts to accommodate patient needs. This means you will typically work around 5 hours in the morning to disconnect patients, and another 5 hours in the evening to reconnect them, with a break in the middle of the day. Flexibility is essential; some days may be longer or shorter depending on patient visits, but overall, average out to 40 hours per week.
Main duties of the job
- To administer prescribed IV therapies and associated care and support to patients who are unable to be trained to independence.
- To provide training and education to support patients/carers in the connection and disconnection of Parenteral Nutrition and any other prescribed therapies to independence.
- To provide training and education to support patients/carers to use infusion devices to deliver their therapy/therapies independently.
- To support the provision of a phlebotomy/cannulation service for patients.
What you need to succeed:
- Registered Nurse with current NMC registration (adult).
- Current clinical experience minimum of least 1 year.
- Competent in the management and delivery of IV Therapies.
- Excellent communication skills, time management skills and the ability to work well against strict timescales.
- The ability to make correct clinical decisions in a clinical emergency situation.
- Flexibility to travel and visit patients outside local area.
- Current Valid driving Licence.

The Patient Flow Coordinator plays a pivotal role in ensuring the safe, timely, and efficient movement of patients through our community hospitals the aim of reducing length of stay, preventing readmissions and optimising patient outcomes.
This is an exciting role which will require you to communicate effectively with patients, families and our multidisciplinary team in order to help patients get home as soon as they are ready. You will work with the teams on our wards at Abingdon Community Hospital. Your work will directly contribute to reducing length of stay, preventing readmissions, and improving patient outcomes.
Main duties of the job
- Planning and Coordinating Hospital discharges
- Effective communication with patients and families to involve them in preparing for discharge from hospital
- Working closely with colleagues in other organisations such as adult social care and voluntary sector organisations to arrange appropriate support for patients when they leave hospital
- Maintaining clear and accurate documentation
- Coordinating and contributing to daily board rounds and weekly MDT meetings.
- Contribute to the induction and training of new staff, including shadowing and mentoring opportunities relevant to the discharge process.

Are you a recently qualified OT looking to develop your skills and experience across a wide range of physical and mental health settings?We are looking for OT's to join our established and sought after Band 5 Occupational Therapy rotation.
Our rotation offers a unique opportunity to work in a mix of clinical areas across physical health, mental health and various specialist services. The rotation currently includes:
- Adult Mental Health (inpatient)
- Adult Learning Disability (community)
- Forensic Mental Health (inpatient)
- Stroke Rehabilitation (inpatient)

- Community Therapy Service (community)
- Community Hospital rehabilitation (inpatient)
The rotation is focused in and around the city of Oxford, with some rotations based in the community hospitals to the south of the county. Each rotation is for a period of 8 months (subject to change).
This role will require you to travel countywide across the Trusts sites on a regular basis. You must therefore be comfortable with driving for work purposes and able to travel freely within the hours of the role. Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.
Main duties of the job
- To provide Occupational Therapy that is evidence based and follows client-centred principles. This will include assessing, planning, implementing, and evaluating the outcome of interventions, documenting all stages of the process.
- Interventions may be carried out in hospital, community settings or during home visits.

This advert is recruiting for a Peer Support Worker (PSW) within the Oxford Health Buckinghamshire Individual Placement & Support Service (IPS). Peer Support Workersare people who have their own lived experience of mental health challenges or have supported/cared for someone with mental health challenges. They use empathy informed by their own experiences to support service users/families who are accessing mental health services. Peer Support Workers model the recovery principles together with personal responsibility, self-awareness and self-care whilst undertaking the duties of the role. The IPS service knows that being in employment can positively impact a person's recovery and wellbeing. The Individual Placement & Support service supports people receiving care from Oxford Health mental health teams who are unemployed and motivated to find paid employment, or whose current employment is at risk. Main duties of the job
We're looking for people who are compassionate, can work well in a team, highly motivated and have excellent written and verbal communication skills. You will be provided with training for the role and given the opportunity to complete the national recognised care certificate.
Oxford Health offer the Peer Support Training programme as a nationally recognised accredited Level 3 Apprenticeship (fully funded by Oxford Health) for those that are eligible and interested in completing a qualification in addition to their in-house training. Main duties of the job
To work as a clinical nurse specialist as part of the hospital-based specialist palliative care service, providing a 7 day a week nursing service.

This is a unique opportunity for an experienced nurse to work autonomously while delivering high-quality clinical care outside of a hospital setting.

Key Details:
Location: Working with trusts and patients in Readingand Surrounding areas.
Working hours: Monday to Sunday, 40 hours over 5 days per week on a rolling rota.
The shifts are structured as split shifts to accommodate patient needs. This means you will typically work around 5 hours in the morning to disconnect patients, and another 5 hours in the evening to reconnect them, with a break in the middle of the day. Flexibility is essential; some days may be longer or shorter depending on patient visits, but overall, average out to 40 hours per week.
Main duties of the job
- To administer prescribed IV therapies and associated care and support to patients who are unable to be trained to independence.
- To provide training and education to support patients/carers in the connection and disconnection of Parenteral Nutrition and any other prescribed therapies to independence.
- To provide training and education to support patients/carers to use infusion devices to deliver their therapy/therapies independently.
- To support the provision of a phlebotomy/cannulation service for patients.
What you need to succeed:
- Registered Nurse with current NMC registration (adult).
- Current clinical experience minimum of least 1 year.
- Competent in the management and delivery of IV Therapies.

The Primary Care Clinical Pharmacist is a clinical practitioner working as part of the multidisciplinary team to optimise medicines use, improve patient outcomes and support safe, evidence-based prescribing across general practice. Key Details:
Location: Working with trusts and patients in High WycombeandSurrounding areas.
Working hours: Monday to Sunday, 40 hours over 5 days per week on a rolling rota.
The shifts are structured as split shifts to accommodate patient needs. This means you will typically work around 5 hours in the morning to disconnect patients, and another 5 hours in the evening to reconnect them, with a break in the middle of the day. Flexibility is essential; some days may be longer or shorter depending on patient visits, but overall, average out to 40 hours per week.
Main duties of the job
- To administer prescribed IV therapies and associated care and support to patients who are unable to be trained to independence.
- To provide training and education to support patients/carers in the connection and disconnection of Parenteral Nutrition and any other prescribed therapies to independence.
- To provide training and education to support patients/carers to use infusion devices to deliver their therapy/therapies independently.
- To support the provision of a phlebotomy/cannulation service for patients.
What you need to succeed:
- Registered Nurse with current NMC registration (adult).
- Current clinical experience minimum of least 1 year.
- Competent in the management and delivery of IV Therapies.
- Excellent communication skills, time management skills and the ability to work well against strict timescales.
- The ability to make correct clinical decisions in a clinical emergency situation.
- Flexibility to travel and visit patients outside local area.
- Current Valid driving Licence.

We are currently looking for a Band 5 Community Nurses to work across the Hounslow areas based at Clayponds Hospital. The service operates from 19:00 until 07:00 seven days a week. The service has 2 shifts, 19:00 until 12 midnight (Twilight) and 19:00 until 7:00 AM (Overnight)
Main duties of the job
To provide high quality, skilled nursing care and to assist the Community Nursing Team Manager in the on-going planning, implementation, and evaluation of the holistic needs of the patient/client, family, and/or carer.
Deliver high quality nursing care to housebound patients with complex needs to enable them to remain in their own homes. Working closely with out of hours service providers, specialist nurses, allied health and social care professionals to meet the needs of the patients without unnecessary attendance in A&E or be admitted to hospital.
This will involve working independently but as part of the Community Nursing Team covering a particular geographical area. This will include first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care. Candidate will be expected to be flexible to support corporate approach to patient care as and when required.

We are looking to recruit a permanent Band 7 Applied Psychologist or Band 8a Highly Specialist Psychologist to join our LD Child and Adolescent Mental Health Team in Teesside covering all four localities (Stockton, Hartlepool, Redcar and Middlesbrough). The successful candidate will join a supportive and collaborative multi-disciplinary team, leading on and delivering psychological assessments, formulations and therapies within the LD Getting More Help CAMHS Team.

The post holder will have a key role in providing both psychological assessments and psychological therapy.

Main duties of the job
In addition to direct clinical work, and where necessary clinical supervision to other team members regarding patients is an important element of the role.
The post holder will provide highly specialist psychological assessments, formulation and therapies with individuals, families and teams as part of an MDT approach. Alongside direct clinical work, providing consultation, training and where required clinical supervision to other team members as well as trainee and assistant psychologists are all integral part of the role.
As a senior member of the team, the post holder will be expected to demonstrate good leadership skills in supporting MDT colleagues to work psychologically and use person-centred principles across their work. This includes supervision, advice, consultation and training. The post holder will work within a compassionate, collective leadership approach and will be a key member of the leadership within the team. Health Visitors will provide evidenced-based interventions to meet the health-related needs of individuals, families, groups and communities and be able to assess and evaluate their effectiveness. Health Visitors lead and deliver the Healthy Child Programme (HCP), providing support from pregnancy and throughout childhood until school age; identifying early those children and families who require additional/specialist intervention and support.
Accountable for the quality and professionalism of their work and advice, role holders answer questions about what needs to be done and how to do them, in specific (and often complex) situations. Clinical interventions at this level horizons may be short term or extend beyond weeks/months. Their work will be based on a theoretical understanding of their technical/professional field, but they will be operating within well-established professional procedures and defined Council policies. Work will involve a broad range of complex problems and role holders will need to manage changing priorities and use professional judgement to respond to differing situations. They relay technical advice and guidance, working with members of the community to ensure that their needs are met.
