Job summary
The Community Nursing serving St Austell and surrounding area in central Cornwall is offering a fantastic opportunity for highly motivated and enthusiastic Community Healthcare Assistants to join our service. We have a vacancy based in the St Austell locality (although occasional cross cover in central maybe be required). The successful candidates must be able to demonstrate a passion for delivering nursing care to adults in their own homes, including those who are approaching end-of-life.
The role is predominantly lone working with the support of a team and requires varied clinical skills such as simple dressings, venepuncture and personal care. The role will encourage professional development through the use of our community matrix to progress your career according to your areas of interest. The trust is committed to personal learning and development within Community Nursing with many opportunities available.
We believe in integrated ways of working to achieve the best possible patient outcomes. Previous care experience is essential and a knowledge of local developments in healthcare is desirable.
Due to the nature of the role it is essential that applicants have a valid UK driving licence or are able to travel independently on a daily basis to and from the base, patients' homes and other venues around the geographical area. Mileage is paid at agenda for change rates and travel time between visits is incorporated into your working shift.
Main duties of the job
To carry out assigned tasks including personal care to our patients at the end of their lives, wound care, venepuncture, while regularly working without direct supervision involving direct care to patients and carers to support the registered nurse.

Key Dimensions of the post are:Working as a member of the community nursing team within a given geographical area. The District Nursing Team work on a flexible rota shift system covering the period from 8am to 10pm which includes an on call system offering a high standard of service.

Main duties of the job

DIUCS brings together NHS 111, a clinical assessment and treatment service including mobile clinical visiting services, urgent treatment centres, minor injuries units, and a single point of access for professionals, integrating these elements, presents the opportunity for more people to receive seamless urgent advice and/or care, and reduce the strain on primary care, our EDs and the 999 service.
You will undertake high quality, holistic, patient centred clinical assessments of patients via the telephone, which always considers people's safety, privacy and dignity.
Provide an excellent standard of clinical practice including safely managing the call back queues through the systems, providing clinical support and advice to our non-clinical Health Advisor team and specialist advice to others regarding the management and care of patients. Plan, implement and review health improvement programmes in a range of settings. Provide supervision, mentorship, training and clinical support to students, trained practitioners and non-clinical colleagues.

There will be opportunity to work in multi-disciplinary clinics with both our paediatric Ophthalmologist and Optometrists. The successful candidate may be required to work at any of our Trust sites and will therefore require excellent organisational skills. Our service has an active Orthoptic stroke service and there may be opportunity to gain experience in this area.

The locum GP role is an essential member of the clinical team with the Extended Access Service. You will provide high quality clinical care to the patients attending the services, dealing with a diverse range of health needs. This role is expected to be a mix of locum shifts on site across 5 locations. Hours of operation are:
- Monday - Friday 6:30pm - 8:30pm
- Saturday - 9am - 1pm and 1pm - 5pm
- Sunday - 9am - 1pm
The role has opportunity for personal development as well as support other clinical areas within The Confederation as needed.

It is essential that the post-holder should hold an MRCP (UK) or equivalent qualification, on the GMC specialist register and hold either a respiratory/GIM CCT (or be within 6 months of obtaining this at time of interview). Candidates with a CESR/portfolio pathway in respiratory medicine will also be accepted.
The positions described in this job description are for fixed term locum physicians in respiratory medicine based at Northampton General Hospital. You will be expected to participate in ward cover 2 weeks in every 8-10. There are plans to develop a respiratory on call rota which will provide 7 day cover and telephone advice overnight, but this is not yet in place.
We are open to discussions about flexible and part time working. We are also open to discussions about different working options such that if the candidate wishes to include procedural sessions (bronchoscopy / EBUS / pleural) or other specialist interests we will consider modifying the job description as such.

An exciting opportunity has arisen for an experienced and highly motivated nurse to join our Breast Services team as aBreast Reconstruction Specialist Nurse.
This is a senior leadership role for an autonomous, advanced practitioner who will provide expert care and support for patients undergoing breast reconstruction following breast cancer surgery. You will work closely with the breast and plastic surgery teams to deliver a seamless, patient-centred service throughout the reconstructive pathway -- from diagnosis and surgical planning to long-term recovery and survivorship.
The post holder will act as the clinical lead for breast reconstruction nursing, driving forward high standards of care, service development, and patient experience in line with national best practice.
About You:
We are looking for an experienced senior nurse with substantial knowledge and clinical expertise in breast surgery, plastic/reconstructive surgery, or oncology. You will have advanced assessment and communication skills, with the ability to work autonomously while contributing to a dynamic MDT.
You should be able to demonstrate leadership, innovation, and a commitment to developing services in line with evolving patient needs.

- Provide specialist advice, education, and support to patients considering or undergoing breast reconstruction.
- Lead nurse-led clinics for pre- and post-operative assessment, education, and follow-up care.

We are seeking to employ a highly motivated, appropriately skilled, reliable, and enthusiastic Estates System Support Worker to provide administrative and systems support within Estates Services. The successful applicant will be a valuable member of the Estates Services team undertaking administrative, and estate systems support to the estates team across the organisation. The post holder will provide an efficient telephone helpline service for the E&F Helpdesk, maintain estates database systems including processing financial, procurement and asset information and accurately input data to the Maintenance Reporting System (fmFirst). The post holder will also carry out reception duties for visitors and contractors and deliver high-quality routine administrative support as required within the Directorate, responding to requests for information promptly and efficiently using a variety of communication methods, ensuring that Trust Customer Service standards are adhered to. The post holder will demonstrate excellent organisational skills, must be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity.
Main duties of the job
As part of this role, you will be responsible for providing accurate information and data from the Computer-Aided Facilities Management (CAFM) system. Acting as the first point of contact for staff and contractor enquiries, you will offer guidance, information, and signposting, ensuring queries are referred to the appropriate department or individual when required.
You will deliver high-quality administrative and clerical support to managers across the team, as well as processing orders, resolving invoice queries, and supporting helpdesk operations.
