Job summary
This role is central to supporting the development of Integrated Neighbourhood Teams (INTs), which bring together professionals from across health, social care, and the voluntary sector to provide joined-up, person-centred care. Improving Health Limited (IHL) and Quay Health Solutions (QHS) GP Federations are working in partnership with Guys and St Thomas Foundation Trust to provide the integrator function for Integrated Neighbourhood Teams (INT) across Lambeth and Southwark. The Integrator partnership will actively support the transformative work of the development of neighbourhood health across the two boroughs, and is recruiting to posts within the newly created neighbourhoods. This role will be hosted by IHL or QHS on behalf of the Integrator but the postholder will be continuously working across organisational boundaries. We are seeking individuals who can work collaboratively with a range of stakeholders, embrace flexible and innovative ways of working, and commit to continuous improvement. This represents a significant cultural shift from a single-organisation focus to being part of a truly integrated team that places residents and communities at the heart of decision-making. Successful candidates will champion co-production, foster strong relationships with local partners, and help shape services that reduce health inequalities and improve outcomes for all.
Main duties of the job
The Neighbourhood Care Coordinator will act as the central point of contact for INT-eligible patients, coordinating their care within the Neighbourhood Integrated working model. The role will support proactive, personalised care for residents with complex needs, frailty, multiple long-term conditions, children and young people, and those who face barriers to accessing or engaging with services. The post holder will proactively identify and engage patients, undertake structured holistic reviews, capture patient goals and wider wellbeing needs, develop personalised care plans, coordinate access to clinical reviews and multidisciplinary team (MDT) meetings, and ensure agreed actions are followed through. In addition, they will support care coordination workflows, prioritisation and escalation processes, contribute to quality improvement activity, and provide supervision and support to care coordination colleagues.
About us
Quay Health Solutions CIC and Improving Health Limited, our two established GP federations in Southwark, are working in partnership with Guys and St Thomas' NHS Trust (GSTT) to provide the Integrator function for Southwark.
Integrators are organisations within each borough that will seek to provide the core infrastructure to support effective integrated neighbourhood team working as it develops, ensuring services are tailored to meet local community needs and operate smoothly across organisational boundaries. Their role is critical in creating cohesive, proactive, targeted, and sustainable services that place individuals and communities at the centre.
Southwark is a diverse borough made up of five neighbourhoods: Bermondsey & Rotherhithe, Borough, Peckham & Nunhead, Camberwell & Walworth, and Dulwich. Each neighbourhood has distinct population needs, requiring tailored approaches to health and care delivery.

More detail about the role
Our Senior Healthcare Support Workers (SHCSWs) form an essential and well-respected part of our healthcare teams across a number of dynamic settings; out in the community, in our acute hospitals or within our theatres teams to name just a few.
You will be a valued member of the team through our comprehensive training and induction programme and our commitment to learning and development will satisfy even the most ambitious person.
The South West Ambulatory Orthopaedic Centre (SWAOC) at Nightingale Exeter have an amazing opportunity for you to join our multidisciplinary team and be part of a fast-growing service.
The team at SWAOC are excited to be able to give patients a day surgery or short stay option for orthopaedic surgery which traditionally has meant an inpatient stay at an acute hospital.

We also complete pre-operative assessments and help run the non-sedated spinal and hip injections lists through diagnostics. We are also always looking to develop our service further and take on new challenges.
Our pathways are therapy and nurse combined so this is a fabulous opportunity for you to be part of something amazing.
It would be an advantage for applicants to have knowledge and clinical skills in an orthopaedic or day unit setting. This post is being offered on a permanent basis.

If you are as excited as we are to provide this amazing service to the patients from all over the South West then please submit your application by the closing date we look forward to welcoming you to our team!

Clinical Practitioner - Care Management Service
East Suffolk and North Essex NHS Foundation Trust View all jobs
Posted 1 month(s) ago
Reference: 432-HR720-26
Job summary
We are seeking an experienced, motivated, and compassionate clinical practitioner to join our Care Management Service. This is an exciting opportunity for a highly skilled professional to lead the provision of proactive, person-centred care for adults with complex health needs, multiple long-term conditions, frailty, and those at risk of hospital admission. We have two posts; Ipswich East and Ipswich West Integrated Neighbourhood Teams.
You will provide advanced clinical assessment, case management, and care coordination, working closely with patients, carers, primary care colleagues, community teams, social care, and voluntary sector partners. You will play a pivotal role in identifying individuals at greatest risk, supporting self-management, reducing health inequalities, preventing avoidable admissions, and enabling patients to remain safely and independently within their own homes.
You will demonstrate strong clinical leadership, excellent communication skills, and a commitment to delivering high-quality, holistic care. You will be passionate about integrated working and ensuring seamless care across organisational boundaries, while supporting service improvement and positive outcomes for patients and their families.
This role offers an excellent opportunity to make a meaningful difference within the community by supporting individuals with complex needs through effective care planning, risk management, and collaboration across the wider health and social care system
Main duties of the job
The CMS Clinical Practitioner is an experienced autonomous practitioner working within the Care Management Service, delivering proactive, personalised and coordinated care for individuals identified as being at high risk of unplanned hospital admission, emergency department attendance and escalating health and social care needs. The role supports the objectives of the Care Management Service through prevention, early intervention, care coordination and multidisciplinary working.
You will work within a multidisciplinary and multi-agency model, supporting individuals with frailty, multimorbidity, complex physical health conditions, mental health needs, functional impairment and social complexity. Using enhanced assessment, clinical reasoning and care management skills, the practitioner will undertake holistic assessments, develop personalised care plans, facilitate rapid intervention and support admission avoidance and timely discharge.
You will contribute to the CMS multidisciplinary team (MDT) process, participate in desktop triage, support proactive monitoring of patients, respond to deterioration identified through clinical review or Data Initiated Follow-Up (DIFU) processes and facilitate progression through active care management, step-down and discharge pathways.
The role may be undertaken by a registered nurse, paramedic, occupational therapist, physiotherapist or other appropriately qualified healthcare professional working within their professional scope of practice.
About us
We are ESNEFT and we provide hospital and community health services to almost one million people across east Suffolk and north Essex. Our dedicated staff deliver care from acute hospitals in Colchester and Ipswich, community hospitals, surgeries, community clinics and in patients' own homes.
We are one of the largest NHS organisations in England, employing more than 12,000 staff.
We pride ourselves on supporting our staff. We offer a wide range of training and development opportunities, as well as flexible working options.
Along with supporting you to achieve your career goals we offer a generous pension scheme, unsocial hours payments (where applicable), 27 days annual leave on commencement (pro rata) and access to a range of NHS discounts. Our Staff Health and Wellbeing programme offer a variety of services.
Our philosophy is that Time Matters to everyone. Across the Trust, we concentrate on improving the things we do and removing those which cause time delays for our staff and patients.
We are investing in our commitment to Time Matters with a partnership with leading electronic patient record (EPR) supplier Epic. This digital transformation will bring what's widely regarded as the world's best EPR system to ESNEFT, transforming life in hospital for staff and patients.
